Activation of the maternal caregiving system by childhood fever--a qualitative study of the experiences made by mothers with a German or a Turkish background in the care of their children.

BACKGROUND: Childhood fever represents a frequent cause to consult a primary care physician. "Fever phobia" describes a fearful and irrational view of fever shared by many parents with different cultural backgrounds. The study aims to explain the experiences of mothers of children having a fever and to analyze the role of the mothers' cultural background with regard to their experiences by comparing the accounts of mothers with a German with those from a Turkish background. Disease and context specific knowledge about the influence of culture can be important for effective counselling. METHOD

[Psychosocial rehabilitation applied to children].

Based on a developmental approach, psychosocial rehabilitation (PSR) for children assumes that a combination of factors points to problems and, as for adults, focuses on recovery. It does not depend solely on the child but on a process involving supportive adults. Recovery in children supports the hope of the parents and builds a new form of parenthood to cope with the child's disability. To optimize psychosocial abilities of a child with a neurodevelopmental disorder (NDD), a personalized assessment and early interventions are needed. Promoting parental skills is the key to preventing additio

Mixed-methods evaluation protocol for the Nurturing Connections Programme: a new Australian perinatal and infant mental health service.

INTRODUCTION: Forming secure attachment relationships provides children with the best possible start to life. Children from families with high psychosocial vulnerability and complex mental health needs (eg, caregivers with lived experience of trauma, experiencing mental illness or substance abuse, current or past domestic violence, and/or current or a history of child protection issues) are at the greatest risk of experiencing attachment disturbances. Nurturing Connections is a new early intervention service launched by the New South Wales State Ministry of Health targeting both caregiver adve

Update on childhood sexual abuse.

PURPOSE OF REVIEW: Childhood sexual abuse (CSA) is a problem of epidemic proportion affecting children and communities worldwide. This review will examine the most recent published CSA literature emphasizing not only new findings in diagnosis, management, and prevention, but also practice implications for the medical and community-based provider. Sexual assault and rape, though forms of sexual abuse, are broader than the scope of this article and not discussed in detail at this time. RECENT FINDINGS: Prevalence rates for CSA are difficult to obtain. To identify populations at greatest risk an

Physical and sexual violence against children in Kenya within a cultural context.

The issue of physical and sexual abuse of children in Kenya is recognised as a highly prevalent and significant problem, occurring within a framework of social values that contribute to the exploitation of women and children. In order to understand the risk and protective factors for children who experience physical and sexual abuse, the cultural context must be taken into consideration. This paper will argue that patriarchy and traditional values underlie the norms and behaviours of parents and communities interacting with children. These values can be protective, eg the value placed on famil

Intergenerational transmission of child abuse and neglect: real or detection bias?

The literature has been contradictory regarding whether parents who were abused as children have a greater tendency to abuse their own children. A prospective 30-year follow-up study interviewed individuals with documented histories of childhood abuse and neglect and matched comparisons and a subset of their children. The study assessed maltreatment based on child protective service (CPS) agency records and reports by parents, nonparents, and offspring. The extent of the intergenerational transmission of abuse and neglect depended in large part on the source of the information used. Individual

The maltreated adolescent: patterns of physical abuse.

Although adolescents are severely and chronically physically abused, relatively little attention has been directed towards understanding this phenomenon. This study explored the family characteristics and the behavioral-emotional reactions of 77 adolescents who were physically maltreated. The youths were selected at a number of sites including a runaway shelter, a hospital-based child abuse team, and a protective service unit. Six different patterns of adolescent reaction to abuse were identified: acting-out, depression, generalized anxiety, extreme adolescent adjustment, emotional-thought dis

When the Victim is a Boy: Reframing Sexual Offences Against Male Children in Indian Law.

In India, sexual offenses against children are gender neutralized and addressed by the Protection of Children from Sexual Offences (POCSO) Act of 2012. Still, the medical/legal literature, clinical suspicion, and judicial discourse continue to be implicitly gendered, resulting in poor detection rates of sexual offenses against male children and inadequate interpretation thereof. The available evidence base is consistent in indicating that a large proportion of child sexual abuse victims are males. Social stigma, delayed presentation, and lack of physician knowledge complicate underreporting an

Mental health and psychosocial support in conflict: children's protection concerns and intervention outcomes in Syria.

Child protection and mental health during conflict intersects with a variety of adverse conflict-related factors, and intervention outcomes in the field are often difficult to predict. Using the casefiles of 376 school children registered in a Mental Health and Psychosocial Support (MHPSS) project in the Northwest governorate of Idleb in Syria, this study aimed to determine (i) the rates of various protection concerns (potential mental health conditions, psychosocial deprivation issues, and social, behavioural and emotional issues) for students enrolled in this project, (ii) whether the rates

[Violence in contact and custody proceedings-A case of child endangerment in the family court-Where are efficient child protection measures?].

Against the background of 30 years of discussion in medicine and law about unfounded accusations and the existence of reasonable suspicion of intra-family violence in contact and custody proceedings, this article emphasises the importance of focusing on child protection by all professionals.Based on a case report in connection with the Federal Ministry of Justice's handout "Dealing with violence in custody and contact proceedings", the article describes the relevance of child protection in Austrian case law and the means by which it is undermined. In the name of science, ideological diagnoses

Reducing Family Violence Through Child Welfare Intervention: A Propensity Score-Matched Study of Fathers for Change.

Intimate partner violence (IPV) is prevalent, costly, and detrimental to children's health and development. It often co-occurs with child abuse and neglect. Most children referred to child protective services (CPS) have witnessed IPV and are at increased risk for subsequent exposure, as well as repeat maltreatment. For CPS referred children, there is often a missed opportunity to interrupt family violence and prevent future occurrences. Fathers for Change (F4C) is a family level intervention designed to reduce IPV by improving emotion regulation and reflective functioning in fathers. To date, …

Understanding pathways from primary health care to universal health coverage outcomes: a realist review.

INTRODUCTION: Primary Health Care (PHC) rooted in community empowerment, integrated service delivery, and multisectoral action is central to efforts toward Universal Health Coverage (UHC), yet little is known about how PHC reforms influence UHC outcomes across contexts. This review examines how PHC approaches affect population coverage, service coverage, and financial protection by identifying what works, for whom, under what circumstances, and why. METHODS: A realist review was conducted using studies from countries with the greatest gains in the UHC Service Coverage Index between 2000 and 2

Mobilize to vaccinate: lessons learned from social mobilization for immunization in low and middle-income countries.

Creating and sustaining demand for immunization services is a global priority to ensure that vaccine-eligible populations are fully protected from vaccine-preventable diseases. Social mobilization remains a key health promotion strategy used by low- and middle-income countries (LMICs) to promote vaccination demand. In this commentary, we synthesize illustrative evidence on successful social mobilization efforts promoting the uptake of immunization services in select LMICs. The first example focuses on Sierra Leone's routine immunization program during the Universal Child Immunization initiativ
