Combined Chylothorax and Chylous Ascites Complicating Liver Transplantation: A Report of a Case and Review of the Literature.

Chyle leaks may occur as a result of surgical intervention. Chyloperitoneum, or chylous ascites after liver transplantation, is rare and the development of chylothorax after abdominal surgery is even more rare. With increasingly aggressive surgical resections, particularly in the retroperitoneum, the incidence of chyle leaks is expected to increase in the future. Here we present a unique case of a combined chylothorax and chyloperitoneum following liver transplantation successfully managed conservatively. Risk factors for chylous ascites include para-aortic manipulation, extensive retroperiton…

A large epithelioid angiomyolipoma of the kidney in a very young child with idiopathic chylothorax: A diagnostic dilemma.

Epithelioid angiomyolipoma (EAML) is a subtype of angiomyolipoma, the most common benign renal tumour and usually asymptomatic until adulthood. This paper reports a patient with EAML identified at 30 months, the youngest age of presentation with this tumour reported in the literature. In this case the tumour was extremely large andwas successfully surgically removed. Furthermore, the patient had no other TS features, presented with acute symptoms, and had bilateral idiopathic chylothorax which was managed by octreotide and diet.

A Cause of Bilateral Chylothorax: A Case of Mesothelioma without Pleural Involvement during Initial Diagnosis.

Chylothorax is characterized by fluid accumulation in the pleural cavity containing chylomicrons due to disruption of lymphatic drainage in the thoracic ductus and development of chylothorax. A 60-year-old male patient presented to our clinic with shortness of breath and displayed bilateral pleural effusion and diffuse mediastinal lymph nodes in his computed chest tomography images. There were no thickening and nodular formation on the pleural surfaces. PET-CT showed no pathological FDG uptake. Thoracentesis showed a chylous effusion. Drainage reduced during monitoring could not be stopped; th…

Chylous ascites and chylothorax due to constrictive pericarditis in a patient infected with HIV: a case report.

INTRODUCTION: Chylothorax and chylous ascites are uncommon and usually associated with trauma or neoplasms. To the best of our knowledge, constrictive pericarditis leading to chylothorax and chylous ascites in a person infected with HIV has never previously been described. CASE PRESENTATION: A 39-year-old Thai man was referred to our institute with progressive dyspnea, edema and abdominal distension. His medical history included HIV infection and pulmonary tuberculosis that was complicated by tuberculous pericarditis and cardiac tamponade. Upon further investigation, we found constrictive per…

Somatostatin - therapeutic option for chylothorax in preterm neonates. Report on two patients and review of the literature.

We report the use of somatostatin in two preterm neonates with chylothorax and discuss those cases in the light of current literature on the treatment of chylothorax in infants born preterm. Chylothorax, a severe complication in thorax surgery, is also a symptom of different diseases and may even occur spontaneously. Treatment is difficult, especially in preterm neonates with co-morbidities. The standard therapeutic strategy with non-invasive procedures (e.g. enteral diet free of long chain triglycerides or parenteral nutrition) is not always effective. Surgical interventions, like pleurodesis…

Successful management of severe chylothorax with argon plasma fulguration and fibrin glue in a premature infant.

Post-traumatic neonatal chylothorax is a rare entity. Management includes medical and surgical treatment. We describe here a newborn who developed a severe chylothorax after repair of an oesophageal atresia. The chylothorax was treated successfully by a combination of argon beam coagulation of the mediastinum and fibrin glue application. However, the patient developed complete thrombosis of the left femoral vein with clot extension to the inferior vena cava which resolved after infusion of recombinant tissue plasminogen activator (rt-PA). The use of argon plasma coagulation and mediastinal fib…

Traumatic chylothorax after blunt thoracic injury: A case series from a level 1 trauma centre.

Traumatic chylothorax is an uncommon complication of thoracic injury resulting from disruption of the thoracic duct or its tributaries. Non-iatrogenic blunt traumatic chylothorax is particularly rare and may present with delayed clinical features, making diagnosis challenging. We present a case series of four patients with blunt trauma-associated chylothorax managed at a level 1 trauma centre over a 22-year period. We aim to highlight patterns of delayed diagnosis, explore the relationship between injury level and chylothorax laterality, and describe the multidisciplinary management strategies…

Chylopericardial Tamponade Accompanied by Chylothorax after Right Upper Lobectomy: A Rare but Fatal Postoperative Complication.

INTRODUCTION: Postoperative chylothorax after thoracic surgery is relatively rare, and cardiac tamponade accompanied by the former is a scarce phenomenon. Although there have been scarce reports of such cases in cardiac surgery, reports on general thoracic surgery are exceedingly rare. CASE PRESENTATION: A 57-year-old male with suspected lung cancer underwent right upper lobectomy and selective mediastinal lymph node dissection. Postoperatively, he developed chylothorax that was unresponsive to conservative management, and thoracic duct ligation was planned for postoperative day 13. However, …

Chylothorax after patent ductus arteriosus ligation in a dog with persistent left cranial vena cava.

This report is the first to document chylothorax after patent ductus arteriosus (PDA) ligation in a dog with a persistent left cranial vena cava (PLCVC), highlighting potential complications arising from concurrent cardiovascular anomalies. A nine-month-old female desexed Pomeranian was referred for PDA. Echocardiography confirmed a left-to-right shunt with mild left-sided cardiac changes from volume overload. At left fourth intercostal thoracotomy, an aberrant vein (5 mm) coursing over the PDA within the mediastinum was retracted proximally to facilitate PDA ligation. Retraction was released …

Laparoscopic transhiatal en-mass thoracic duct ligation for persistent bilateral spontaneous Chylothorax: A case report.

Chylothorax is a rare entity associated with morbidity. Surgical thoracic duct ligation (TDL) by thoracoscopic approach is the recommended choice for persistent chylothorax. However, thoracoscopy is not feasible in case of previous pleurodesis. We describe a successful laparoscopic transhiatal en-mass TDL in a 61-year-old lady for persistent spontaneous chylothorax after failed optimal conservative management and three sessions of pleurodesis. The study shows that laparoscopic transhiatal thoracic duct ligation is an effective alternative in a case where thoracoscopy is not feasible due to var…

Refractory chylothorax following COVID-19: Successful surgical management of a rare complication; a case report.

INTRODUCTION AND IMPORTANCE: Chylothorax is an uncommon complication linked to COVID-19. The fundamental pathophysiology and most effective management strategy are still uncertain. CASE PRESENTATION: A 72-year-old man presented with worsening dyspnea and fatigue one month post-COVID-19. Imaging demonstrated a significant right pleural effusion, with thoracentesis confirming the presence of chylothorax. Despite the implementation of conservative interventions, the effusion remained unresolved. During the right thoracotomy procedure, a 1 cm (about 0.39 in) perforation in proximity to the Azygos…

Postoperative chylothorax after left pulmonary wedge resection without mediastinal lymph node dissection: a case report.

Postoperative chylothorax is a rare but serious complication after pulmonary resection. In previous studies, we have found no reports of postoperative chylothorax after left pulmonary wedge resection. Considering the many variations in the route of the thoracic duct, it also has the risk of postoperative chylothorax. We describe a case of refractory chylothorax after left pulmonary wedge resection without mediastinal lymph node dissection. Conservative treatment and supradiaphragmatic thoracic duct ligation did not obtain satisfactory results in this patient. Finally, under the guidance of mag…

[Postoperative Chylothorax Following Gastric Tube Cancer Surgery Cured after Pneumonia and Empyema-A Case Report].

A 74-year-old man was under follow-up after esophageal cancer surgery and CRT for hypopharyngeal cancer. Follow-up endoscopy revealed an ulcerative lesion in the lower gastric tube, and biopsy showed group 5(tub1). Endoscopic resection was difficult, and surgery was decided. Gastric tube resection and subcutaneous jejunum reconstruction were performed. Postoperatively, chylothorax was observed. Enteral nutrition was discontinued, and the patient was managed with TPN, and continuous subcutaneous octreotide and continuous intravenous etyrefrine were started. Even after conservative treatment was…

Persistent chylothorax associated with lymphatic malformation type 6 due to biallelic pathogenic variants in PIEZO1.

PIEZO1 is required for lymphatic valve formation, and several lymphatic abnormalities have been reported to be associated with autosomal recessive PIEZO1 pathogenic variants including neonatal hydrops, lymphedema involving various body regions, and chylothorax. Persistent or recurrent chylothorax has been infrequently described in association with pathogenic variants in the PIEZO1 gene. We present a 4-year-old female with bilateral pleural effusions detected prenatally, who was diagnosed with bilateral chylothoraces post-partum. She subsequently had recurrent pleural effusions involving both p…

Cystic Lung Disease Presenting as Recurrent Non-traumatic Chylothorax: Case Report and a Mini-Review.

Malignancy and infections are the most common causes of recurrent chylothorax. Cystic lung disease, especially sporadic pulmonary lymphangioleiomyomatosis (LAM), is a rare condition that may manifest as recurrent chylothorax. We present a case of a 42-year female who presented with dyspnea on exertion secondary to recurrent chylothorax, requiring three thoracenteses within a few weeks. Chest imaging showed multiple bilateral thin-walled cysts. Thoracentesis revealed milky-colored pleural fluid, which was exudative and lymphocytic predominant. Infectious, autoimmune, and malignancy workup was n…

Double trouble: Biochemically confirmed bilateral chylothorax with positive pleural fluid cytology due to breast adenocarcinoma.

Although chylothorax is a well-described complication of malignancy, especially lymphoma, breast adenocarcinoma has not been a commonly implicated primary tumor. There has been only one published report of biochemically confirmed bilateral chylothorax in solid malignancy, and this was not associated with breast adenocarcinoma. Likewise, there has been only one published report of bilateral chylothorax in solid malignancy with positive pleural fluid cytology on both sides; again, the primary tumor was not breast adenocarcinoma. Herein we present a case that combines all three of these rarely re…

[Successful treatment of refractory chylothorax associated with diffuse large B-cell lymphoma by multidisciplinary care].

Chylothorax is an intrathoracic leakage of chyle due to thoracic duct damage. Malignant lymphoma is the most common nontraumatic cause of chylothorax. In March 2019, a 74-year-old woman presented to our department with bilateral pleural effusion and mesenteric/retroperitoneal masses. She was diagnosed with diffuse large B-cell lymphoma upon performing a biopsy. In May 2019, she was hospitalized for dyspnea due to pleural effusion, and thoracentesis revealed abundant chyle. Although the tumor shrunk after chemotherapy, chylothorax improvement was poor; thus, she could not be discharged. For the…
