Liquid Biopsies in the Clinical Management of Germ Cell Tumor Patients: State-of-the-Art and Future Directions.

Liquid biopsies constitute a minimally invasive means of managing cancer patients, entailing early diagnosis, follow-up and prediction of response to therapy. Their use in the germ cell tumor field is invaluable since diagnostic tissue biopsies (which are invasive) are often not performed, and therefore only a presumptive diagnosis can be made, confirmed upon examination of the surgical specimen. Herein, we provide an overall review of the current liquid biopsy-based biomarkers of this disease, including the classical, routinely used serum tumor markers-the promising microRNAs rapidly approach

Minimal residual disease in cervical cancer: biological persistence, ctHPV DNA-guided surveillance, and recurrence-directed intervention.

Cervical cancer remains a leading cause of cancer-related mortality among women worldwide, with approximately 30-40% of patients experiencing recurrence after definitive chemoradiotherapy for locally advanced disease. The concept of minimal residual disease (MRD)-the persistence of viable tumor clones after curative-intent treatment that escape conventional radiographic and clinical detection-has transformed surveillance and treatment paradigms in hematological malignancies and, increasingly, in solid tumors. In cervical cancer, the near-universal association with high-risk human papillomaviru

Thermosensitive hydrogel-enhanced RPA-CRISPR/Cas12a biosensor for ultrasensitive detection of methylated loci in breast cancer ctDNA.

BACKGROUND: Methylation differences exist between breast cancer tissues and normal tissues. The release of methylated circulating tumor DNA (ctDNA) by tumor cells provides a foundation for breast cancer liquid biopsy using methylated ctDNA. However, detection of low-abundance methylated loci in ctDNA remains a significant challenge to date. Existing one-tube detection systems cannot avoid target depletion caused by CRISPR/Cas12a cleavage, leading to reduced sensitivity. RESULTS: This study developed a thermosensitive hydrogel-based one-tube RPA-CRISPR/Cas12a detection system, combined with me…

Molecular residual disease: Charting a new path for treating resectable solid tumors.

Molecular residual disease (MRD), detected via circulating tumor DNA (ctDNA) after curative-intent surgery, identifies resectable solid tumor patients at high recurrence risk, thereby enabling enriched trials with larger effect sizes and smaller samples. Yet enrichment narrows the eligible population and thus challenges conventional randomized designs. We propose an MRD Registry as an external control cohort that integrates quantitative ctDNA data with outcomes to validate ctDNA dynamics as response indicators-facilitating efficient MRD-based trial design and accelerating novel therapy develop…

Advances in In Vitro Diagnostics for Cholangiocarcinoma: From Biomarker Discovery to Artificial Intelligence.

Cholangiocarcinoma (CCA) is a highly aggressive malignancy that is difficult to diagnose early and carries a poor prognosis. Conventional serological diagnostics are limited by inadequate sensitivity and the risks of invasive procedures, resulting in most patients being diagnosed at an advanced stage. In recent years, liquid biopsy has emerged as a novel approach for non-invasive and dynamic molecular tumor monitoring by detecting biomarkers such as circulating tumor cells (CTCs), extracellular vesicles (EVs), circulating tumor DNA (ctDNA) and clusterin (CLU). Furthermore, artificial intellige…

Classification of Brain Tumors by Nanopore Sequencing of Cell-Free DNA from Cerebrospinal Fluid.

BACKGROUND: Molecular brain tumor diagnosis is usually dependent on tissue biopsies or resections. This can pose several risks associated wi … ilable. RESULTS: 110/129 samples were technically successful, and 50 of these contained detectable circulating tumor DNA (ctDNA) by CNV or methylation profiling. ctDNA was detected in samples from patients with progressive disease but also from patients without known residual disease. CNV plots showed diagnostic and prognostic alterations, such as C19MC amplifications …

Liquid biopsy in pancreatic cancer: current advances, limitations, and future perspectives.

Pancreatic ductal adenocarcinoma (PDAC) is the most common pancreatic malignancy and is usually diagnosed at advanced stages. The diagnosis of PDAC is established through pancreatic cytology/tissue biopsy, most commonly obtained under endoscopic ultrasound-guidance. Although this approach is generally safe and accurate for the histological diagnosis, maintaining/obtaining material for additional molecular analysis may be challenging. Thus, there is growing interest in minimally invasive approaches capable of providing molecular information without necessarily requiring tissue sampling. Liquid

The Predictive Role of ctDNA and CTCs in Patients with Advanced Non-Small Cell Lung Cancer Receiving Immunotherapy: A Systematic Review and Meta-Analysis.

Circulating tumour DNA (ctDNA) and circulating tumour cells (CTCs) have emerged as promising non-invasive biomarkers for the immunotherapy response in advanced non-small cell lung cancer (NSCLC). However, their clinical utility remains uncertain due to variability in findings across studies. We conducted a systematic review and meta-analysis of studies from 2014 to 2024, assessing the predictive value of ctDNA and CTCs in advanced NSCLC patients receiving immunotherapy. Primary outcomes were progression-free survival (PFS) and overall survival (OS). Forty-four studies were included (28 ctDNA,

Liquid biopsy in lung cancer.

Precision medicine based on biomarkers, such as genetic abnormalities and PD-L1 expression, has been established for the treatment of nonsmall cell lung cancer. Recently, liquid biopsy has emerged as a valuable and minimally invasive alternative. This method analyzes blood and other bodily fluids to detect cancer-related genetic abnormalities and molecular residual disease (MRD). Liquid biopsy, which includes testing for circulating tumor cells, circulating tumor DNA (ctDNA), and microRNA (miRNA), offers several advantages over conventional methods. It is minimally invasive, can be performed r…

Improving the diagnosis of leptomeningeal metastases by molecular profiling of cell-free DNA from cerebrospinal fluid.

BACKGROUND: The limited sensitivity of cerebrospinal fluid (CSF) cytology leads to delayed or missed diagnoses of leptomeningeal metastases (LM). This study explores the diagnostic potential of next generation sequencing (NGS) in diagnosing LM by analyzing primary tumor mutations in CSF. METHODS: CSF-cell-DNA, CSF-cell-free-DNA (cfDNA) and plasma-cfDNA from ten patients with cytological confirmed LM were used to examine the best DNA source for analysis. Next, CSF samples from 85 patients with a known primary tumor mutation and a suspicion on LM were examined to compare diagnostic sensitivitie

Standardized droplet preamplification method for downstream circulating cell-free DNA analysis.

Circulating cell-free DNA (ccfDNA) can be found in blood and other biofluids and is a minimally invasive biomarker for several pathological processes. As tumors become more invasive, an increasing amount of circulating tumor DNA (ctDNA) is also shed into the peripheral circulation. Combined analysis of ccfDNA and ctDNA has demonstrated prognostic and predictive value in metastatic disease. However, localized tumors shed significantly less ccfDNA/ctDNA and accurate detection remains a technical challenge. To overcome this barrier, droplet preamplification has been used to perform robust multipl…

Cell-free HPV DNA as a complementary biomarker to imaging in HPV-associated oropharyngeal squamous cell carcinoma: a narrative synthesis of key studies.

INTRODUCTION: Human papillomavirus (HPV)-associated oropharyngeal squamous cell carcinoma (OPSCC) represents a biologically distinct disease entity with superior prognosis compared with HPV-negative head and neck cancers. While imaging-based strategies, particularly those monitoring hypoxia, have emerged as promising tools for response-guided de-escalation, they may not fully capture tumor biology or residual disease risk. Consequently, circulating tumor-derived HPV DNA (ctHPV-DNA) has gained attention as a minimally invasive biomarker with potential to complement imaging for treatment monitor

Plasma ctDNA is a tumor tissue surrogate and enables clinical-genomic stratification of metastatic bladder cancer.

Molecular stratification can improve the management of advanced cancers, but requires relevant tumor samples. Metastatic urothelial carcinoma (mUC) is poised to benefit given a recent expansion of treatment options and its high genomic heterogeneity. We profile minimally-invasive plasma circulating tumor DNA (ctDNA) samples from 104 mUC patients, and compare to same-patient tumor tissue obtained during invasive surgery. Patient ctDNA abundance is independently prognostic for overall survival in patients initiating first-line systemic therapy. Importantly, ctDNA analysis reproduces the somatic …

DNA nanodevice-engineered macrophages as living sensors for dual-mode in vivo monitoring of tumor-associated macrophage polarization.

Exploiting macrophages as cell-autonomous reporters of their polarization states represents a promising strategy for interrogating tumor-associated macrophage (TAM) phenotypes in cancer. Here, we develop a macrophage-based sensing platform, termed eMφ. This platform is created by engineering macrophages with a modular and multiplexable DNA origami nanodevice to report TAM polarization states within the tumor microenvironment. Upon tumor infiltration, microenvironmental cues drive macrophage polarization, triggering the engineered system to convert endogenous signals (e.g., Arg1 or iNOS mRNA) i

[Progress of Liquid Biopsy in the Diagnosis and Treatment of Lung Cancer
with Malignant Pleural Effusion].

Lung cancer is the malignancy with the highest mortality rate worldwide. Malignant pleural effusion (MPE) caused by advanced lung cancer severely affects the quality of life and prognosis of patients. Tumor-associated genetic testing is the basis for making precise treatment decisions. There are some risks of tissue biopsy, and it is difficult to sample repeatedly. Due to its non-invasive and can reflect the full profile of tumor gene characteristics, liquid biopsy is increasingly used in the diagnosis and treatment of lung cancer. Because of the extremely low DNA level of circulating tumor, t

Tumor-naïve ctDNA detection with deep learning-enhanced error suppression for sensitive mutation calling.

BACKGROUND: Circulating tumor DNA (ctDNA) detection offers minimally invasive monitoring of cancer from blood samples. While tumor-informed approaches are sensitive, their clinical application is limited by cost, tissue availability, and turnaround time. In contrast, tumor-naïve assays offer faster turnaround times and broader applicability, but often sacrifice sensitivity. METHODS: Here, we developed DEEPctMUT, a computational pipeline for tumor-naïve ctDNA detection that integrates unique molecular identifiers (UMIs) with three complementary error-polishing strategies: (1) machine learning-…

Non-invasive diagnosis and surveillance of bladder cancer with driver and passenger DNA methylation in a prospective cohort study.

BACKGROUND: State-of-art non-invasive diagnosis processes for bladder cancer (BLCA) harbour shortcomings such as low sensitivity and specificity, unable to distinguish between high- (HG) and low-grade (LG) tumours, as well as inability to differentiate muscle-invasive bladder cancer (MIBC) and non-muscle-invasive bladder cancer (NMIBC). This study investigates a comprehensive characterization of the entire DNA methylation (DNAm) landscape of BLCA to determine the relevant biomarkers for the non-invasive diagnosis of BLCA. METHODS: A total of 304 samples from 224 donors were enrolled in this m
