The development of a new, ultra-fine, and flexible neuroendoscope for intracranial observation.

BACKGROUND: A neuroendoscope is a technical advance that allows surgeons to visualize certain regions of the brain that was previously inacc … rebellopontine angle, the lower cranial nerves and trigeminal nerve could be observed, and from the cisterna magna, the floor of the fourth ventricle and the aqueduct could be seen. CONCLUSION: The angioscope is a useful instrument to observe intracranial locations safely and effectively even within a limited surgical field. Further modifications will be requir …

Evaluation of the effects of quercetin on brain lesions secondary to experimental hydrocephalus in rats.

INTRODUCTION: Hydrocephaly is a disease that affects not only the dynamics of the cerebrospinal fluid, but also other structures of the central nervous system. Although shunt is effective in reducing ventriculomegaly, many neurological damages are not reversed with surgery. Several studies demonstrate that oxidative stress is involved in the genesis of hydrocephalus lesions. OBJECTIVE: Evaluate the neuroprotective response of quercetin in hydrocephalus. MATERIALS AND METHODS: Male newborns rats were used, which received the 15% kaolin injection in the cisterna magna for induction of hydrocep…

Alterations in the expression of protease-activated receptor 1 and tumor necrosis factor-α in the basilar artery of rats following a subarachnoid hemorrhage.

The present study aimed to investigate the expression of protease-activated receptor 1 (PAR1) and tumor necrosis factor (TNF)-α in a rat model of subarachnoid hemorrhage (SAH)-induced cerebral vasospasm (CVS). The rat models were established by twice injecting blood into the cisterna magna, after which the following experimental groups were established: The normal group, the SAH3d group, the SAH5d group and the SAH7d group. The rats were perfused and the basilar artery was removed for histological examination. The cross-sectional area of the basilar artery lumen was measured using computer sof…

An excitatory action of adrenaline, noradrenaline, and 5-hydroxytryptamine on the spinal cord.

1. Fasciculation was produced in the trapezius and sternomastoid muscles of anaesthetized cats by adrenaline, noradrenaline and 5-hydroxytryptamine (5-HT) put into the cisterna magna. Isoprenaline, similarly applied, was ineffective.2. Each of the three active amines was without effect when applied for a second time after the fasciculation in response to the first application had passed off, but 5-HT was effective after adrenaline and, similarly, adrenaline after 5-HT.3. Fasciculation produced by adrenaline, but not by 5-HT, was inhibited by ergotamine or phenoxybenzamine, given intravenously.…

Initial experience with combined endoscopic third ventriculostomy and choroid plexus cauterization for post-hemorrhagic hydrocephalus of prematurity: the importance of prepontine cistern status and the predictive value of FIESTA MRI imaging.

PURPOSE: Post-hemorrhagic hydrocephalus of prematurity (PHHP) is among the most common causes of infant hydrocephalus in developed nations. This population has a high incidence of shunt failure, infection, and slit ventricle syndrome. Although effective for other etiologies of infant hydrocephalus, the efficacy of combined endoscopic third ventriculostomy and choroid plexus cauterization (ETV/CPC) in PHHP has not been investigated. This pilot study reports the initial experience. METHODS: Ten patients (four grade III and six grade IV intraventricular hemorrhage) requiring definitive treatment

Central nervous system action of melatonin on gastric acid and pepsin secretion in pylorus-ligated rats.

We recently demonstrated that centrally administered melatonin at low doses inhibits the induction of gastric lesions by water-immersion restraint stress. To investigate the mechanism of the potent anti-ulcer action of melatonin, the central nervous system (CNS) effects of melatonin on gastric acid and pepsin secretion were studied in conscious pylorus-ligated rats. Intracisternal (i.c.) melatonin (1-100 ng) dose-dependently decreased acid and pepsin output, while a higher i.p. dose (1 microg) had no inhibitory effect. The i.c. melatonin did not change serum gastrin concentrations. Serum melat

Continuous intracisternal and high cervical intrathecal bupivacaine analgesia in refractory head and neck pain.

BACKGROUND: The upper cervical component of the spinomesencephalic tract and cranial nerves V, VII (nervus intermedius), IX, and X are involved in mechanisms of acute and chronic pain from head and neck structures. To date there is no reliable method for relief of refractory pain (i.e., pain that cannot be relieved by conventional pharmacologic therapies) from these structures. Therefore, we explored continuous intracisternal infusion of bupivacaine for the treatment of refractory pain of the head and neck. METHODS: Intracisternal catheters were inserted in 13 adults with refractory nonmalign

MR imaging of experimental meningeal melanomatosis in nude rats.

MR imaging of the rat brain has become an increasingly frequently used method in experimental neuroradiology. On a generally available 1.5 T whole body tomograph, supplemented with an individually made small coil and a special SE sequence we obtained fairly fine images of the structures of the rat brain. With gadolinium-DTPA, we were able to visualize posterior fossa and cervical leptomeningeal growth of intrathecally injected B16 melanoma in nude rats. Using MRI to follow experimental leptomeningeal metastasis, may provide a new means for diagnostic evaluation and preclinical testing of treat

Unusual cisternal coil migration in the follow-up period after aneurysm perforation during endovascular coiling.

A 61-year-old woman suffered cisternal coil migration in the follow-up period after endovascular coil embolization for a ruptured cerebral aneurysm. She presented with sudden onset of headache. Computed tomography demonstrated diffuse subarachnoid hemorrhage, and cerebral angiography disclosed a left anterior choroidal artery aneurysm. The aneurysm was treated by endovascular embolization with Guglielmi detachable coils. During the embolization procedure, the microcatheter perforated the aneurysm. For direct closure of the perforation site with coils, the microcatheter was withdrawn and coils

Visualization of the intracisternal angioarchitecture at the posterior fossa by use of image fusion.

OBJECTIVE: Magnetic resonance (MR) images most clearly visualize intracranial tissues but have some limitations in terms of detailed analysis of the intracisternal vasculature. To compensate for these shortcomings, an image fusion of three-dimensional digital subtraction angiography (DSA) and MR images, DSA-MR fusion, has been developed. The goal of this study was to evaluate the usefulness of DSA-MR fusion for the visualization of the intracisternal arteries and veins at the posterior fossa. METHODS: Ten consecutive patients (five with neurovascular compression syndrome and five with brain t

Intracisternal injection of apolipoprotein A-IV inhibits gastric secretion in pylorus-ligated conscious rats.

BACKGROUND/AIMS: Fat feeding increases not only serum but also cerebrospinal fluid concentration of apolipoprotein (apo) A-IV, a protein produced mainly by the small intestine in the rat. We hypothesized that apo A-IV may have a central effect on gastric secretion. METHODS: Gastric juice was collected by the pylorus ligation method. Rats underwent pylorus ligation and received intracisternal injection of apo A-IV under brief isoflurane anesthesia. Two hours after the injection, gastric juice was collected and gastric acid output determined. RESULTS: Intracisternal injection of 0.5 microgram

Definition of the role of contemporary surgical management in cisternal and parenchymatous cysticercosis cerebri.

With increasing immigration from endemic regions, the incidence of neurocysticercosis in North America is rising. This retrospective study was undertaken to examine the role of surgery in those cases presenting with large cystic parenchymal and cisternal lesions in the current era of anthelminthic agents administered orally. A total of 237 patients presented with newly diagnosed neurocysticercosis to our institution over a recent 5-year period (mean age, 31.2 years). Among those who presented with cystic mass lesions predominantly affecting the brain parenchyma and cisternal spaces, 20 (8.4%;

Functional and autoradiographic studies to locate the sites at which clonidine acts to cause hyperglycaemia and inhibition of opiate-induced sympathetic outflow.

The effect of intracisternal administration of clonidine (1 microgram/kg) on the response to intravenous injection of morphine (4 mg/kg) was examined in conscious rabbits. Morphine acts on central opiate receptors to increase sympathetic outflow and cause hypertension. Clonidine, given intracisternally, prevented the morphine-induced rise in mean arterial pressure, fall in heart rate and increase in catecholamines in plasma. Using in vitro autoradiography, alpha 2-adrenoceptors were localised in the nucleus of the tractus solitarius and the dorsal motor nucleus of the vagus and these may be tw
