Combined molecular and clinical approach for decision making for surgery in HNPCC patients: a report on three cases in two families.

Hereditary nonpolyposis colorectal cancer (HNPCC) is associated with highly penetrant germline mutations in mismatch repair genes. Due to a high lifetime risk in gene carriers for synchronous and for metachronous colorectal cancer and endometrial cancer in women, prophylactic and extended surgery are considered as options for gene carriers. A 54-year-old patient with a history of metachronous rectal cancer and a family history fulfilling the Amsterdam criteria presented with carcinoma of the cecum and highly dysplastic adenomas of the splenic flexure and descending colon. As a result of these

Clinical application of a light microscopic scoring method to make decisions regarding the pharmacotherapy of an Eimeria maxima abatement programme.

The purpose of the present study was to use an Eimeria maxima light microscopic lesion scoring (MLS) system to measure magnitudes of E. maxima infection in sections of broiler chick intestine, and to help make decisions regarding the efficacy of an E. maxima abatement programme. Chicks with whole-body pallor, elevated feed conversion ratios (10 points higher than normal) and reduced body sizes (0.1 kg smaller than normal) had light microscopic lesions caused by E. maxima infections. These intestine segments had high Eimeria maxima MLSs. High scores, coupled with the fact that there was no micr

Clinical decision making in Barrett's oesophagus can be supported by computerized immunoquantitation and morphometry of features associated with proliferation and differentiation.

Grading of dysplasia in Barrett's oesophagus has a therapeutic impact, but subjective grading is associated with substantial observer variation. Quantitative pathological methods could help to achieve a more accurate and reproducible diagnosis. In the present study, the immunoquantitation of p53 and Ki67 and the morphometric analysis of features associated with proliferation and differentiation were evaluated for this purpose. In slides of 35 oesophagectomy specimens, 73 areas that displayed either no dysplasia (ND), low-grade dysplasia (LGD), high-grade dysplasia (HGD), or intramucosal carcin

Fine-Tuning, Retrieval-Augmented Generation, and Hybrid Adaptation of Language Models for Clinical Decision-Making in Health Care: Systematic Review.

BACKGROUND: Large language models (LLMs) demonstrate strong performance on medical knowledge benchmarks, but their safe and effective use in clinical practice depends on posttraining adaptation rather than raw model capability. Fine-tuning, retrieval-augmented generation (RAG), and hybrid approaches are principal strategies for grounding language models in clinical evidence, yet their comparative effectiveness remains unclear. OBJECTIVE: This systematic review aims to synthesize evidence on fine-tuning, RAG, and hybrid posttraining strategies for clinical diagnosis and decision-support tasks

Long-Term Outcomes of Patients With Suspected Angina Pectoris Following Clinical Decision-Making Guided by ATP-FFR Versus Nicorandil-FFR.

BACKGROUND: Fractional flow reserve (FFR)-guided revascularization improves long-term outcomes in patients with coronary artery disease. Although adenosine triphosphate (ATP) is the standard hyperemic agent, nicorandil (NIC) has emerged as a practical alternative due to its combined nitrate-like and ATP-sensitive potassium channel-activating properties. However, data on long-term vessel-related outcomes guided by NIC-induced FFR are limited. AIMS: To compare long-term clinical and vessel-based outcomes in patients with suspected angina pectoris whose treatment strategies were guided by ATP-in

Impact of Continuing Medical Education on Family Physicians' Knowledge and Clinical Decision-Making Regarding Intermittent Fasting in Type 2 Diabetes.

AIMS/BACKGROUND: Intermittent fasting (IF) is increasingly adopted by patients with type 2 diabetes mellitus (T2DM), often without adequate medical supervision. Despite growing patient demand, family physicians frequently report limited formal training in fasting-related diabetes management. This study aimed to evaluate the efficacy of a structured continuing medical education (CME) intervention in improving family physicians' knowledge, confidence, and clinical decision-making regarding IF in T2DM. METHODS: A single-group pre-test and post-test interventional design was employed. Four weekly…

Flexible Bronchoscopy and Non-Small-Cell Lung Cancer Staging: A Narrative Review of Modern Techniques for Optimized Clinical Decision-Making.

Non-small-cell lung cancer (NSCLC) is a widespread and aggressive form of cancer, and in cases of its occurrence, accurate diagnosis and precise staging play a crucial role in determining treatment and estimating prognosis. Flexible bronchoscopy (FB) is a minimally invasive method used to assess the local and regional extent of the disease. FB facilitates the identification of endobronchial lesions and the collection of biopsy samples for histopathological diagnosis. It also enables the evaluation of regional lymph node involvement via advanced techniques such as endobronchial ultrasound with

Reviewing long-term opioid use in patients experiencing chronic pain in general practice: Activating patients and supporting clinical decision making.

BACKGROUND: Australia has a high rate of opioid prescribing, which has been associated with multiple harms. Regular review of patients' opioid-related risk factors and realigning treatment improves clinical outcomes; however, general practitioners (GPs) can find this consultation difficult. OBJECTIVE: The aim of this article is to support GPs and their chronic pain patients who have been prescribed long-term opioids with a structured and validated opioid review process through the Routine Opioid Outcome Monitoring (ROOM) tool and the patient-led Opioid Safety Toolkit. DISCUSSION: The ROOM to

A novel clinical classification of spinal tuberculosis for decision-making guidelines from Chinese spine surgeons: a multicenter retrospective cohort study.

BACKGROUND: Spinal tuberculosis (TB) presents substantial therapeutic challenges as it can lead to severe structural and neurological complications. However, evidence-based therapeutic decision-making is hindered by the limitations of existing classification systems, which lack anatomical specificity, quantifiable parameters, and reliability. METHODS: The classification system proposed herein categorizes spinal TB into four types based on clinical manifestations and radiologic features: type I (mild): single-segment involvement, with <50% vertebral collapse and preserved stability; type II (m

Clinical decision making for abdominal stab wounds in high resourced but low volume centres require structured guidelines to be effective.

AIM: The management of abdominal stab wounds (SW) has continued to evolve. The use of CT and laparoscopy has been advocated to reduce the rate of laparotomy. This study reviews our experience with SW in a high income, low volume setting. METHODS: A retrospective study was undertaken from 2006 to 2020 at Waikato Hospital, New Zealand. All adult patients age > 16 years that were admitted following trauma were included. RESULTS: Seventy three cases of SW were included. Thirty two cases had indications for immediate laparotomy (peritonitis in 15, hemodynamic instability in 13, evisceration in 4)

Real-time drug testing of paediatric diffuse midline glioma to support clinical decision making: The Zurich DIPG/DMG centre experience.

BACKGROUND: Children diagnosed with diffuse midline gliomas (DMG) have an extremely poor overall survival: 9-12 months from diagnosis with currently no curative treatment options. Given DMG molecular heterogeneity, surgical biopsies are needed for molecular profiling and as part of enrolment into molecular-based and precision medicine type clinical interventions. In this study, we describe the results of real time profiling and drug testing at the diffuse intrinsic pontine glioma/DMG Research Centre at University Children's Hospital Zurich. METHOD: Biopsies were taken using a frame based ster
