Occupational therapists' perspectives on binocular diplopia in neurorehabilitation: A national survey.

BACKGROUND: Oculomotor dysfunction affects a significant number of adults with neurological conditions and binocular diplopia is a common symptom which impacts an individual's ability to participate in meaningful daily activities. Occupational therapists use partial and complete occlusion to minimize binocular diplopia, however a review of the literature reflected a lack of standardized protocol for each intervention technique. The purpose of this study was to examine occupational therapists' perspectives on the use of partial and complete occlusion and the clinical reasoning process used. ME…

Decoding and reasoning mental states in major depression and social anxiety disorder.

BACKGROUND: Major depression (MDD) and social anxiety (SAD) disorders are debilitating psychiatric conditions characterized by disturbed interpersonal relationships. Despite these impairments in social relationships, research has been limited in simultaneously evaluating the dysfunction in MDD or SAD within two aspects of theory of mind (ToM): decoding mental states (i.e., Affective ToM) and reasoning mental states (i.e., cognitive ToM). Taking this into consideration, the current study assesses both decoding and reasoning mental states abilities in MDD, SAD, and healthy controls (HC). METHOD

BACKGROUND: Large language models (LLMs) are emerging as tools to support clinical decision making. HIV management is a compelling use case … curated benchmark of HIV-related open-ended medical question-answer pairs spanning basic knowledge, clinical reasoning, complex patient vignettes, and bias-modified scenarios. We evaluated seven general-purpose and three medical LLMs. Performance was assessed using lexical similarity and an extended medical LLM-as-a-judge framework capturing key clinical dimensions, …

Teaching AI-Informed Diagnostic Reasoning in Dental Education: A Conceptual Framework.

Artificial intelligence (AI) is increasingly present in dental education and clinical practice through systems that support image interpreta … stifying an accountable management decision. It is conceived as a metacognitive overlay on existing clinical reasoning competencies rather than as a standalone clinical discipline. The framework can be operationalized in Objective Structured Clinical Examinations (OSCEs) through staged disclosure of AI output, independent learner interpretation before disclosure, st …

Clinician reasoning in the use of cultural formulation to resolve uncertainty in the diagnosis of psychosis.

We examined how the process of cultural formulation contributes to diagnostic assessment of patients with psychotic disorders at a specialized Cultural Consultation Service (CCS). Specifically, we investigated the reasoning process used to resolve uncertainty of psychotic disorder diagnosis in African immigrant patients referred to the CCS for assessment of possible psychotic disorder. Qualitative thematic analysis of 23 clinical case conference transcripts was used to identify clinicians' reasoning styles. Use of the CF appears to facilitate the emergence of a rule-governed reasoning process

Potential mechanism of impaired perceptual reasoning in children with obstructive sleep apnea syndrome: topological analysis of brain white matter network employing graph theory.

Childhood obstructive sleep apnea syndrome (OSAS) disrupts normal ventilation and sleep structure and affects cognitive functions. However, the neurophysiological mechanisms underlying cognitive impairment are unclear. This study investigates the topological connectivity of white matter networks in children with moderate to severe OSAS and explores the underlying mechanisms of cognitive impairment. We collected clinical data of patients with moderate to severe OSAS (n = 43) and non-OSAS (n = 30). Intelligence testing was conducted using the China Wechsler Intelligence Scale for Children-Fourth

Physical examination before diagnostic ultrasonography: Alive and kicking or a relic of the past?

PURPOSE: To investigate how often physicians perform physical examination prior to referring patients for diagnostic ultrasonography (US), and whether its omission affects clinical reasoning quality or diagnostic yield. METHODS: In this prospective study, patients undergoing diagnostic US at a tertiary care center (December 2024-April 2025) were asked whether their referring physician had performed a physical examination. Patient demographics, referring specialty, healthcare professional, US indication, and anatomical region were recorded. Clinical reasoning quality (0-100%) was defined as th…

Mechanisms in clinical practice: use and justification.

While the importance of mechanisms in determining causality in medicine is currently the subject of active debate, the role of mechanistic reasoning in clinical practice has received far less attention. In this paper we look at this question in the context of the treatment of a particular individual, and argue that evidence of mechanisms is indeed key to various aspects of clinical practice, including assessing population-level research reports, diagnostic as well as therapeutic decision making, and the assessment of treatment effects. We use the pulmonary condition bronchiectasis as a source

Depression as inferential rigidity: a meta-abductive account.

INTRODUCTION: Depression is a highly prevalent mental disorder worldwide, and its cognitive rigidity-characterized by persistent negative beliefs resistant to countervailing evidence-remains a critical puzzle in philosophical psychiatry and clinical psychology. Existing theories of abductive reasoning have struggled to explain why similar adversities lead to rigid negative cognition in some individuals but adaptive coping in others. METHODS: Drawing on a hierarchical reconstruction of Peircean abduction, this study develops a three-layer model of depressive inferential pathology. The theoreti

Donor Dilemma: A National Simulation-Based Case Curriculum for Teaching Donor Call Decision-Making in Infectious Diseases Trainees.

INTRODUCTION: Donor-call decision-making during organ allocation requires clinical reasoning in the setting of uncertainty, time pressure, and evolving information. In transplant infectious diseases (TID), this skill is essential, yet exposure during training is often inconsistent and informal. Educational strategies that allow trainees to practice this type of decision-making remain limited. We aimed to evaluate the feasibility, acceptability, and perceived impact of an asynchronous electronic, case-based progressive disclosure curriculum designed to support clinical reasoning during donor-ca…

Assessing the Alignment Between the Humpty Dumpty Fall Scale and Fall Risk Nursing Diagnosis in Pediatric Patients: A Retrospective ROC Curve Analysis.

Background/Objectives: Falls in hospitalized pediatric patients are frequent and can lead to serious complications and increased healthcare costs. Nurses typically assess fall risk using structured tools such as the Humpty Dumpty Fall Scale (HDFS), alongside nursing diagnoses such as Fall risk ND, which are based on clinical reasoning. However, the degree of alignment between the HDFS and the nursing reasoning-based diagnostic approach in assessing fall risk remains unclear. This study aims to assess the alignment between the HDFS and Fall risk ND in identifying fall risk among hospitalized pe…

The relevance of dysfunctional reasoning to OCD and its treatment: Further evidence for inferential confusion utilizing a new task-based measure.

BACKGROUND AND OBJECTIVES: Previous research has highlighted the role of dysfunctional reasoning processes (i.e. "inferential confusion") in the development and maintenance of Obsessive-Compulsive Disorder (OCD). Inferential confusion has previously been found to be a unique predictor of OC symptoms and has shown specificity for OCD. However, these findings have primarily relied on a single self-report questionnaire, and only a limited number of experimentations have been conducted to establish the specificity of inferential confusion to OCD with alternate measures. The current paper demonstra
