Young Barley (Hordeum vulgare L.) Preparations: From Phytochemical Complexity to Clinical Relevance.

Young barley, derived from the early vegetative stage of Hordeum vulgare L., constitutes a plant-based functional ingredient whose phytochemical profile differs markedly from that of mature grain. Two principal commercial forms exist-dried grass powder and juice-derived products-differing in matrix composition and bioactive compound concentration. This narrative review critically evaluates the current knowledge on the phytochemical composition, biological activity, and translational relevance of young barley preparations considered as a functional plant food. The phytochemical spectrum is domi

Atrial septal pouch: a comprehensive morphologic and morphometric evaluation with clinical relevance.

PURPOSE: The septal pouch is an anatomical variant resulting from incomplete fusion between the septum primum and the septum secundum at the interatrial septum. This partial fusion creates a conical pouch, which is potentially predisposed to thrombus formation, increasing the degree of thromboembolic risk. This cadaveric study aimed to evaluate the prevalence, morphology, and morphometry of septal pouches and their associations with other septal defects. METHODOLOGY: The interatrial septa of 60 human hearts were examined for septal pouches. Characteristics such as pouch type, apex and base or

Prevalence and clinical relevance of systematically tested antimitochondrial antibodies in systemic sclerosis.

INTRODUCTION: Antimitochondrial antibodies (AMA), the hallmark of primary biliary cholangitis (PBC), have been inconsistently reported and poorly characterized in systemic sclerosis (SSc). Although recently linked to subclinical dysmotility in a cohort enriched for gastrointestinal (GI) involvement, data on the prevalence and clinical relevance of AMA in real-life SSc cohorts remain limited. METHODS: A retrospective cross-sectional study was conducted on consecutive SSc patients followed at a referral center for the disease. Patients were tested for serum AMA according to reference standards, …

Clinical relevance of hyperamylasemia and pancreatitis-like imaging linked with accidental hypothermia.

BACKGROUND: Elevated pancreatic enzymes and pancreatitis-like imaging findings have been reported in patients with accidental hypothermia; however, their clinical significance remains unclear. This study aimed to explore a serum amylase threshold that may support consideration of computed tomography (CT) for evaluating pancreatitis-like findings in accidental hypothermia and describe the clinical course of affected patients. METHODS: We conducted a retrospective single-center observational study of adult patients with accidental hypothermia admitted to a tertiary emergency and critical care c

Persistent Median Arteries: A Cadaveric Study of Prevalence, Variants, and Clinical Relevance.

The median artery, an embryonic vessel, may persist into adulthood as the persistent median artery (PMA), a variant of clinical and surgical relevance. In a cadaveric study of 23 upper limbs from 12 donated adult cadavers, PMA was identified in eight limbs (34.8%). Half of these (4/8 limbs, 17.4%) pierced the median nerve in the proximal forearm, an uncommon finding. Two palmar-type PMAs were identified, both of which exhibited atypical terminal branching in the hand (2/23 limbs, 8.7%). These anatomical variations may contribute to median nerve compression syndromes, such as carpal tunnel synd

Berberine-Drug Interactions: Mechanisms, Clinical Relevance and Risk Stratification-A Narrative Review.

Background: Berberine, an isoquinoline alkaloid present in Berberis spp., Coptis chinensis and Hydrastis canadensis, is among the most widely consumed metabolic-health supplements, popularized as "nature's Ozempic". Concurrent, often undisclosed use with prescription drugs is common in older adults, yet berberine is far from inert. Objective: To synthesize the evidence on berberine as a perpetrator of supplement-drug interactions, propose a four-axis mechanistic taxonomy, with product quality treated separately as a modifier of exposure rather than as a mechanism, and derive a clinically actio

A Novel 3D Bioprinting Strategy for Bioengineering of Urethra with Clinical Relevance.

Urethral strictures can cause significant discomfort and progressive urinary tract damage if left untreated. Current reconstructive options, including urethral resection and buccal mucosa grafting, are associated with several limitations such as donor-site morbidity, limited tissue availability, and variable long-term outcomes. To address these challenges, we developed a novel multilayered 3D-bioprinted urethral construct designed to closely mimic the native urethral architecture. Using an Integrated Tissue and Organ Printing System (ITOP) equipped with a rotating mandrel, tubular urethral con

Doming Volume in Mitral Valve Prolapse: Pathophysiology, Imaging Implications and Clinical Relevance.

Mitral valve prolapse represents the most common cause of primary mitral regurgitation in Western countries and has traditionally been viewed as a disorder driven by valvular incompetence and chronic volume overload. Within this paradigm, left ventricular enlargement was expected to correlate with regurgitant severity. However, patients with myxomatous bileaflet prolapse often exhibit left ventricular dilatation disproportionate to the degree of regurgitation, leading to the hypothesis of an intrinsic myocardial disease process. Cardiovascular magnetic resonance imaging has challenged this con

Finding hybrid schistosomes in genital schistosomiasis: are we missing their clinical relevance?

Genital schistosomiasis is a parasitic disease that affects both men and women as male and female genital schistosomiasis (MGS and FGS), respectively. Symptoms mimic sexually transmitted infections in men and women, owing to inflammation in the genital organs caused by migrating and trapped parasite ova. Schistosoma haematobium is the main causative organism, with an estimated 75% of infected women experiencing FGS, equating to approximately 56 million women mainly in sub-Saharan Africa. Estimates for MGS are mostly lacking, but numbers may be equivalent. S. haematobium has been found in hybri

ECM remodeling in hypothyroidism-associated MAFLD: mechanisms, clinical relevance, and therapeutic targets.

Metabolic dysfunction-associated fatty liver disease (MAFLD) is a major chronic liver disease increasingly linked to endocrine and immunometabolic dysregulation. Hypothyroidism, both overt and subclinical, has emerged as a significant endocrine risk factor for MAFLD. Extracellular matrix (ECM) remodeling, a hallmark of fibrosis, represents a crucial but underexplored mediator in this process. This review highlights how altered thyroid hormone (TH) and thyroid-stimulating hormone (TSH) signaling promote ECM remodeling through metabolic, inflammatory, and fibrogenic pathways, with emphasis on he

Animal Models of Traumatic Brain Injury and Their Relevance in Clinical Settings.

BACKGROUND: Traumatic brain injury (TBI) is a significant concern that often goes overlooked, resulting from various factors such as traffic accidents, violence, military services, and medical conditions. It is a major health issue affecting people of all age groups across the world, causing significant morbidity and mortality. TBI is a highly intricate disease process that causes both structural damage and functional deficits. These effects result from a combination of primary and secondary injury mechanisms. It is responsible for causing a range of negative effects, such as impairments in co

Sinoatrial Node Dynamics: Clinical Relevance and microRNA-Based Modulation of Pacemaker Activity.

The sinoatrial node (SAN) functions as the primary pacemaker of the heart, initiating rhythmic electrical impulses that govern cardiac contraction. It consists of specialised pacemaker cells located between the superior vena cava and the right atrium, supported by a paranodal region that contributes to rhythm stabilisation and may serve as a secondary pacemaker during dysfunction. This review describes the structural organisation and molecular regulation of the SAN, with particular focus on the role of HCN4 ion channels and the influence of the microRNA miR-486-3p in modulating pacemaker activ

Melatonin Membrane Receptors in the Vascular System: Function, Modulation and Clinical Relevance.

Melatonin (MEL) is a hormone whose secretion is regulated by daily and seasonal environmental light cycles. MEL production is strictly limited to the night and is inhibited by light. Besides exhibiting chronobiotic properties, MEL possesses vasoactive properties mediated by the plasma membrane melatonin receptors (MT1/MT2) and ion channels expressed in the vascular system. Therefore, it is important to understand the distribution and functions of these receptors in the vascular system. In this review, we analyzed 50 studies to map the expression of MT1/MT2 receptors in the vascular system and

The Potential Clinical Relevance of Necrosis-Necroptosis Pathways for Hypoxic-Ischaemic Encephalopathy.

Hypoxic-ischaemic encephalopathy (HIE) is a major cause of neonatal brain injury and is associated with a high rate of death and lifelong disability. Its pathogenesis is still poorly understood, and there is no proven treatment for preterm infants. Therapeutic hypothermia for term and near-term infants partially improves outcomes, highlighting the need to target additional mechanisms. This review evaluates evidence that necrosis and necroptosis contribute materially to evolving brain injury in both term and preterm brains. Serial imaging studies suggest that lesions typically develop over many
