The authors present a case of fracture dislocation of the ankle joint with the proximal part of the fibula entrapped behind the tibia ; Bosworth's fracture. This fracture was described for the first time in 1848 by Hugier, and classified by Bosworth in 1947. In our case, diagnosis was not done in a first time, and closed reduction failed. This fracture was operated on and open reduction was performed. At the present time, the functional result is good.

We reviewed 24 displaced fractures through the physis of the medial epicondyle of the distal humerus. One was a Salter-Harris type-II fracture-separation of the whole distal humeral epiphysis; the others involved only the medial epicondylar centre of ossification. Two cases had presented as pseudarthroses. One fracture had been treated closed in a plaster slab and 21 had had open reduction and internal fixation with sutures, Kirschner wires or Palmer nails. At 2 to 13 years later we found five types of deformity of the epicondyle: pseudarthrosis, an ulnar sulcus, a double-contoured epicondyle,

OBJECTIVES: To investigate which treatment of maxillofacial fractures is more effective and what type of complications is the most common after observed treatment. The second aim is to explore relationship between treated facial bone fractures and temporomandibular joint (TMJ) pathology.
MATERIAL AND METHODS: Cases with TMJ pathology in Lithuanian University of Health Sciences (LUHS) in the Department of Maxillofacial Surgery (MS) during 2012-2014 were analysed to research the occurrence of TMJ disorders after facial bone fracture treatment. Moreover, the clinical data of patients that were t

BACKGROUND: Pediatric supracondylar fracture and forearm bone fracture is common in isolation but combined supracondylar fracture with ipsilateral forearm bone fracture, known as floating elbow is not common injury. The incidence of this association varies between 3% and 13%. Since the injury is rare and only limited literatures are available, choosing best management options for floating elbow is challenging.
METHOD: In retrospective review of 759 consecutive supracondylar fracture managed in between July 2005 to June 2011, children with combined supracondylar fracture with forearm bone inju

In order to prevent permanent disability, a proper diagnosis of the fixed posterior fracture dislocation of the fibula with fracture dislocation of the ankle joint must be made. Full-length (knee to ankle) radiographs are necessary to make this diagnosis. Although a closed reduction has been described as successful, we feel that an open reduction is usually necessary. Freeing of the trapped fibula from behind the tibia results in a satisfactory reduction of this fracture dislocation. The reduction and fixation of the malleolar fragment is then simple, and an excellent result can be expected.

The authors studied, with clinical and radiological criteria the results of closed treatment (reduction by manipulation and immobilization in a plaster cast for 7 or 8 weeks) of severe displaced malleolar fractures. Of 145 patients who had suffered an ankle fracture and were treated with closed methods between 1979 and 1989, 78 were reviewed, with an average follow-up of 6.2 years. The main difficulties of closed treatment concerned reduction of the fractures; anatomical reduction was achieved only in 32.4% of the cases, and deterioration of the position achieved often occurred (26.2%). INTRODUCTION: Metatarsophalangeal (MTP) joint dislocations are uncommon due to the joint's inherent stability. Simultaneous dislocations of multiple lesser MTP joints with an associated fracture are extremely rare and typically result from high-energy trauma.
CASE REPORT: A 36-year-old female sustained dorsolateral dislocation of the 2nd, 3rd, and 4th MTP joints and an intra-articular fracture of the 5th metatarsal base following a tractor run-over injury. Closed reduction under regional anesthesia was successfully performed. The joints were found to be stable post-reduction and immobilized w

Salter-Harris type I (SH-I) fractures of the distal fibula are commonly encountered in pediatric orthopedics. We describe 2 unique cases of adolescents with completely displaced SH-I distal fibula fractures that were treated operatively. In the first case, a closed reduction attempt failed and the patient required open reduction and internal fixation of the distal fibula and syndesmosis. The syndesmotic ligaments were avulsed from the distal fibular metaphysis. In the second case, closed reduction of the distal fibula fracture was partially successful, but anatomic reduction could not be achie

CASE: A young polytrauma victim of a road traffic accident was diagnosed with comminuted articular fracture of the left distal tibial pilon and open segmental fracture of the fibula. The comminuted distal tibial metaphysis was translocated into the interosseous space and, hence, was irreducible. He underwent single-stage closed Ilizarov device stabilization and gradual postoperative closed reduction to obtain good functional outcome at 9 months. The lung and scapular injuries were treated conservatively.
CONCLUSION: Acute complex irreducible tibial pilon fracture can be treated according to p

A 77-year-old woman reported a spiral right humeral shaft fracture, which was treated by closed reduction and fixation with elastic bundle retrograde intramedullary nail. Despite a proper closed reduction and a minimally invasive surgery obtained, nonunion arose. The patient refused the advised second surgical treatment. At 12 months, the patient was admitted to our department as a consequence of persistent pain and unnatural humeral shaft mobility. New X-ray imaging highlighted intramedullary nail rupture due to excessive implant mobility at the nonunion site. Consequently, nail removal was p

Sensory disturbance following orbitozygomatic complex fractures was studied in 65 patients in relation to type of fracture and method of treatment. The fracture-type-dependent treatments were: no surgical intervention (n = 20), closed reduction with or without wire fixation (n = 17), open reduction with miniplate fixation (n = 15) and/or reconstruction of the orbital floor (n = 13). Several methods were applied to assess sensory function, on average 6.3 months after treatment, i.e. the patient's report and tests regarding touch, two methods of two-point discrimination, and cold, all applied on

BACKGROUND: The haematoma block (HB) has been successfully used to minimise pain prior to otherwise painful closed reduction manoeuvres for distal radius fractures. However, the invasive manner of the procedure, which technically produces an open fracture, still raises the question, whether HBs increase the risk of infection compared to conventional intravenous analgesia (IA). The purpose of this study was to assess complication rates and safety of the HB procedure for the closed reduction of surgically treated distal radius fractures.
METHODS: We included 176 distal radius fractures in 170 p

UNLABELLED: Intracapsular hip fractures in young adults have a significant risk of complications. Consequently, some authors advocate urgent and/or open fracture reduction. Our aim was to analyse outcomes following such fractures with reference to influence of fracture displacement, timing of surgery and method of reduction (open/closed) on the incidence of non-union (NU) and avascular necrosis (AVN).
METHODS: Specific search terms were used to retrieve relevant published studies from 1966 to May 2003.
RESULTS: Eighteen studies involving 564 fractures were analysed. The overall incidence of

Forearm fractures in children are among the most common fractures however isolated distal ulnar physeal fractures are rare, especially Salter-Harris type IV fracture involving epiphysis, physis and metaphyseal parts. Growth disturbances are not uncommon in such fractures. Also, certain anatomical variants, including cleft ulna epiphysis, may mimic fracture lines on radiographs, unnecessary operational intervention may result from misinterpretation. We describe 2 cases the first of a 12-year-old child who had sustained a bilateral wrist trauma after falling from a bicycle. A left distal forearm

BACKGROUND: Type III supracondylar fractures represent a difficult injury to treat and there is no universal consensus regarding the best treatment. The purpose of this study is to assess the incidence of complication associated with open reduction and compare open reduction vs closed reduction, in order to determine which treatment lead to better clinical and radiological outcomes. Is open reduction really associated with a higher number of iatrogenic complication and worse clinical outcomes in comparison to closed reduction?
METHODS: A total of 55 patients, affected by type III supracondyla

Posterior dislocation of the shoulder, a rare injury, results from direct trauma, indirect trauma, or via a seizure or electrical shock. We present a case with a posterior fracture dislocation of the shoulder secondary to a seizure in which interposition of the biceps tendon precluded closed reduction. The fractured lesser tuberosity fragment included the bicipital groove, allowing the biceps tendon to sublux posteriorly preventing closed reduction, thus requiring a subsequent open reduction.

The results of conservative treatment of a perilunate fracture dislocation were retrospectively studied. In six patients, the treatment consisted of closed reduction and immobilization in a below the elbow cast for 12 to 16 weeks. Five patients had a compromised healing of the scaphoid fracture. The frail vascularization of the scaphoid, extensive ligamentous injury and possible mid-carpal instability in case of perilunate fracture dislocation as well as a non-anatomical reduction of a scaphoid fracture are causative factors in ensuing scaphoid pseudarthrosis. If anatomical reduction of a peri
