Informing estimates of probability of Clostridioides difficile infection for testing and treatment: expert consensus from a modified-Delphi procedure.

BACKGROUND: Clostridioides difficile infection (CDI) may be misdiagnosed if testing is performed in the absence of signs or symptoms of disease. This study sought to support appropriate testing by estimating the impact of signs, symptoms, and healthcare exposures on pre-test likelihood of CDI. METHODS: A panel of fifteen experts in infectious diseases participated in a modified UCLA/RAND Delphi study to estimate likelihood of CDI. Consensus, defined as agreement by >70% of panelists, was assessed via a REDCap survey. Items without consensus were discussed in a virtual meeting followed by a se…

Molecular characterization and antibiotic resistance of Clostridioides difficile in patients with inflammatory bowel disease from two hospitals in China.

OBJECTIVES: Patients with inflammatory bowel disease (IBD) are susceptible to Clostridioides difficile infection (CDI), resulting in poor outcomes and recurrence; therefore, the molecular characterization of C. difficile in IBD patients in China needs further investigation. METHODS: C. difficile strains were isolated and identified from faecal samples of adult and paediatric IBD patients. Toxigenic strains were typed using multilocus sequence typing and whole genomic sequencing (WGS) to construct the phylogenetic tree. Susceptibility to 10 antimicrobials was evaluated using the Etest. RESULT…

The Efficacy of Faecal Microbiota Transplant and Rectal Bacteriotherapy in Patients with Recurrent Clostridioides difficile Infection: A Retrospective Cohort Study.

The most effective treatment for recurrent Clostridioides difficile infection (CDI) is faecal microbiota transplantation (FMT); however, the optimal route of administration is thus far unknown. This retrospective cohort study of 343 patients sought to evaluate the efficacy of treatment with FMT capsules, FMT enema, and rectal bacteriotherapy (RBT) during a five-year period. The primary endpoint was clinical resolution from CDI after eight weeks, and secondary endpoints were time to recurrence and death during the follow-up period. The proportion of patients with clinical resolution was 79.9% i…

Prognostic role of perioperative acid-base disturbances on the risk of Clostridioides difficile infection in patients undergoing cardiac surgery.

BACKGROUND: It is unclear whether acid-base balance disturbances during the perioperative period may impact Clostridium difficile infection (CDI), which is the third most common major infection following cardiac surgery. We hypothesized that perioperative acid-base abnormalities including lactate disturbances may predict the probability of incidence of CDI in patients after cardiac procedures. METHODS: Of the 12,235 analyzed patients following cardiac surgery, 143 (1.2%) developed CDI. The control group included 200 consecutive patients without diarrhea, who underwent cardiac procedure within

Presence of Clostridioides difficile and multidrug-resistant healthcare-associated pathogens in stool specimens from hospitalized patients in the USA.

BACKGROUND: Healthcare-associated infections (HCAIs) continue to be a major cause of morbidity and mortality. Many HCAI pathogens, including multidrug-resistant organisms (MDROs), colonize the gastrointestinal tract. AIM: To determine the frequency of MDRO carriage in patients who do and do not harbour toxigenic Clostridioides difficile in their stools. METHODS: Stool specimens received from nine US laboratories were cultured using media selective for C. difficile, Staphylococcus aureus, vancomycin-resistant enterococci (VRE), and carbapenem-resistant Gram-negative organisms (CROs). Specimen…

Time trends and predictors of laboratory-confirmed recurrent and severe Clostridioides difficile infections in Manitoba: a population-based study.

BACKGROUND: Many previous studies of Clostridioides difficile infection (CDI) epidemiology have used hospital discharge data codes, which can have limited accuracy. We used a data set of laboratory-confirmed cases of CDI in the province of Manitoba, Canada, to describe the epidemiology of CDI over a decade. METHODS: We conducted a population-based historical cohort study using Manitoba Health's population-wide laboratory-based CDI data set linked to administrative health databases. All individuals living in Manitoba and experiencing a CDI episode between 2005 and 2015 were included (n = 8471)…

Clostridioides difficile ribotype 106: A systematic review of the antimicrobial susceptibility, genetics, and clinical outcomes of this common worldwide strain.

Clostridioides difficile typing is invaluable for the investigation of both institution-specific outbreaks as well as national surveillance. While the epidemic ribotype 027 (RT027) has received a significant amount of resources and attention, ribotype 106 (RT106) has become more prevalent throughout the past decade. The purpose of this systematic review was to comprehensively summarize the genetic determinants, antimicrobial susceptibility, epidemiology, and clinical outcomes of infection caused by RT106. A total of 68 articles published between 1999 and 2019 were identified as relevant to thi…

Predictors of Clostridioides difficile recurrence across a national cohort of veterans in outpatient, acute, and long-term care settings.

PURPOSE: The greatest challenge in treating Clostridioides difficile infection (CDI) is disease recurrence, which occurs in about 20% of patients, usually within 30 days of treatment cessation. We sought to identify independent predictors of first recurrence among a national cohort of veterans with CDI. METHODS: We conducted a case-control study among acute and long-term care Veterans Affairs (VA) inpatients and outpatients with a first CDI episode (positive stool sample for C. difficile toxin[s] and receipt of at least 2 days of CDI treatment) between 2010 and 2014. Cases experienced first r…

Phenotypic profiling of neutrophils in acute Clostridioides difficile infection identifies a TNF-induced activation signature associated with epithelial damage.

Neutrophils are key first responders in the host response to Clostridioides difficile infection (CDI). Although a high number of tissue and blood neutrophils clearly correlates with adverse outcomes in CDI patients, their functional role remains poorly defined. Using murine (CMT-93) and human (Caco-2) intestinal epithelial cell (IEC) lines co-cultured with neutrophils and a pre-clinical mouse model of CDI, we show that activated neutrophils exacerbate C. difficile-induced IEC injury. To identify neutrophil subtypes and biological pathways that contribute to CDI-associated IEC damage, we utiliz…

Sepsis associated with Clostridioides difficile infection carries similar mortality to sepsis of other origin: a propensity score-matched analysis.

Severe Clostridioides difficile infection (CDI) is associated with high mortality, partly due to comorbidities. We aimed to assess whether sepsis due to CDI leads to worse outcomes compared to sepsis from other infections and whether CDI is associated with a distinct host immune profile. In this retrospective analysis, patients with CDI and sepsis, included in two prospective multi-center CDI studies, (one conducted during 2015-2021 and one during 2022-2023), were compared to comorbidity- and severity-matched patients with sepsis of other cause [community acquired pneumonia (CAP), hospital-acq…

Changing patterns and biological features of community-acquired Clostridioides difficile infection in Southwest China: 7 years of surveillance data.

The molecular epidemiological features of community-acquired Clostridioides difficile infection in Southwest China from 7 years of surveillance data were analyzed. Four representative C. difficile strains were selected for RNA-seq, biofilm formation, toxin expression, and cytotoxicity assays. Overall, 5.04% of the C. difficile strains were isolated within 7 years, 85.51% of which were toxigenic C. difficile (both tcdA+/tcdB+). Multilocus sequence typing (ST) and genomic sequencing divided all the isolates into two clusters, namely, clade 1 and clade 4, respectively. ST37 of C. difficile gradua…

Efficacy of hydrogen peroxide vaporizing against methicillin-resistant Staphylococcus aureus, extended-spectrum β-lactamase-producing bacteria, and Clostridioides difficile.

BACKGROUND: Environmental contamination with drug-resistant bacteria and spore-forming organisms such as Clostridioides difficile poses a significant risk of healthcare-associated infections (HAIs). While manual disinfection using disinfectant-impregnated cloths is standard practice, it is susceptible to human error and may leave residual contamination by pathogens. METHODS: We evaluated the bactericidal efficacy of hydrogen peroxide vaporizing using a compact device (NOCOSPRAY2) in a simulated hospital room (28.8 m3). Surfaces were inoculated with methicillin-resistant Staphylococcus aureus …

Revealing roles of S-layer protein (SlpA) in Clostridioides difficile pathogenicity by generating the first slpA gene deletion mutant.

Clostridioides difficile infection (CDI) with high morbidity and high mortality is an urgent threat to public health, and C. difficile pathogenesis studies are eagerly required for CDI therapy. The major surface layer protein, SlpA, was supposed to play a key role in C. difficile pathogenesis; however, a lack of isogenic slpA mutants has greatly hampered analysis of SlpA functions. In this study, the whole slpA gene was successfully deleted for the first time via CRISPR-Cas9 system. Deletion of slpA in C. difficile resulted in smaller, smother-edged colonies, shorter bacterial cell size, and a…

Molecular epidemiology, antimicrobial susceptibility, and toxin production of clinical Clostridioides difficile isolates from a teaching hospital in Northern China.

To grasp the epidemiological trend and drug resistance mechanisms of Clostridioides difficile (C. diff) in Beijing, 302 C. diff isolates were obtained from patients with diarrhea. The sequence types (STs) from mainstream strains were all susceptible to metronidazole, vancomycin, piperacillin/tazobactam, meropenem, and tigecycline but almost resistant to ciprofloxacin and clindamycin. The missense mutation of GyrA/GyrB and RpoB resulted in fluoroquinolone and rifamycin resistance, respectively. Toxigenic strains from clade IV were likely to be missed due to the deficiency of tcdA gene. Four tcd…
