The retrievable type of inferior vena cava filter has been widely used to prevent pulmonary thromboembolism in patients with deep vein thrombosis and contraindication of anticoagulation. Physicians make considerable efforts to remove the filter according to the manufacturer and US Food and Drug Administration safety advisory recommendation. However, forced filter retrieval might cause vascular injury within 3 weeks. Herein, we report pathologic and angiographic findings to suggest filter associated vascular injury during forced retrieval just within recommended period in a patient with deep ve

Surgical therapy is an established and effective therapy for Graves' disease. Application of this therapy, however, became less often recently, because of its negative features such as need for admission, scar formation and possible complications. For patients with Graves' disease surgical therapy is the only available therapy if they are associated with thyroid cancer, or if they are pregnant and antithyroid drugs are contraindicated. Recently, indication of the surgical therapy for this disease tends to be limited to special or difficult cases. Here, we analyze trend in our practice in Kuma

Forty-two cases of pemphigus vulgaris (PV) and bullous pemphigoid (BP) were treated with plasmapheresis by one of 3 techniques: centrifugation, double filtration plasmapheresis (DFPP), or a combination of the two. Each plasmapheresis resulted in a rapid reduction in the autoantibody titer and an improvement in clinical symptoms, thereby allowing a lower dose of corticosteroid to be administered and remission to be achieved. These findings suggest that plasmapheresis is an effective treatment for PV and BP patients who have been unresponsive to conventional therapy, for those for whom conventio

Antibiotics are the most frequently prescribed medications in pregnancy, their application and choice should be made according to the highest criteria. Penicillins and cephalosporins can be safely used in pregnancy, however, their side effects such as allergic reactions present a greater therapeutic problem. Tetracyclines, chloramphenicol and quinolones are strictly contraindicated in pregnancy. 5-nitroimidazole derivatives, aminoglycosides and sulfonamides are indicated only in severe infections. Not only the risk of malformation is increased in the embryonic stage, i.e. in the first 8 weeks

Conventional treatment of two patients with chronic idiopathic thrombocytopenic purpura (CITP) and petechial intracranial hemorrhage was ineffectual. However, after a treatment with vincristine, the neurological manifestations disappeared and platelet counts improved; clinical recovery was complete. This success contrast with results informed in similar patients treated with corticosteroids or splenectomy, among whom the mortality is as high as 100%. The favorable reaction may be related to the various mechanisms of the action of vincristine, which usually lead to control of CITP. The use of t

Emergency ward physicians are presented daily with patients in pain. Provisions of safe, quick pain control remains one of their major duties. Hypnosis can be used as an effective adjunct or substitute for analgesic medications when these drugs prove to be ineffective or contraindicated. Four such illustrative cases of attempted pain control are presented. The psychological foundations of pain and its assessment are discussed. The emergency ward physician can obtain facility in hypnotic techniques with only modest training. Hypnosis may then become a valuable tool in helping him provide safe a

The clinical problems posed by a series of partial or total monolateral carotid occlusions complicated by cerebral infarction are discussed. Emphasis is laid on the frequent aspecificity of the symptomatology, the inconsistency of palpatory and auscultatory findings in the cervical region, and the value of certain medical techniques that may precede angiographic diagnosis. Considering the lack of reports in the literature, a case in which carotid stenosis was considered to be of luetic nature is noteworthy. Prophylactic treatment designed to prevent another cerebral attack hinges essentially o

BACKGROUND/AIM: Managing central nervous system (CNS) metastases of epidermal growth factor receptor (EGFR)-mutated non-small-cell lung cancer can be challenging if dose-limiting toxicities prevent adequate drug exposure.
CASE REPORT: We report the case of a 70-year-old woman with a suprasellar metastasis abutting the optic chiasm for whom radiotherapy was contraindicated. Although treatment with 80 mg osimertinib once daily was begun, recurrent grade 3 neutropenia required a dose reduction to 40 mg every other day. During alternate-day dosing, the suprasellar lesion progressed despite contin

BACKGROUND: We aimed to determine whether sodium valproate (VPA) should be contraindicated in all mitochondrial diseases, due to known VPA-induced severe hepatotoxicity in some mitochondrial diseases.
METHODS: We systematically reviewed the published literature for mitochondrial DNA (mtDNA) and common nuclear genotypes of mitochondrial diseases using PubMed, Ovid Embase, Ovid Medline and MitoPhen databases. We extracted patient-level data from peer-reviewed articles, published until July 2022, using the Human Phenotype Ontology to manually code clinical presentations for 156 patients with gen

OBJECTIVE: Graves disease is the most common cause of thyrotoxicosis. Medical management is the first-line treatment but may be contraindicated or ineffective. In patients with severe, refractory thyrotoxicosis therapeutic plasma exchange (TPE) may be indicated as a bridge to thyroidectomy.
METHODS: We present 3 cases of thyrotoxicosis refractory to medical management that were successfully treated with TPE and subsequent total thyroidectomy, and provide an analysis of the response to therapy via a change in free thyroxine (fT4) levels throughout their treatment course.
RESULTS: The average

Background Although indomethacin (IND) is the standard treatment for hemodynamically significant patent ductus arteriosus (hsPDA) in Japan, it may be associated with renal impairment and gastrointestinal complications. The use of paracetamol for hsPDA closure has recently increased. Unlike IND, paracetamol does not have a peripheral vasoconstrictive effect and can be given to infants with contraindications to IND. Based on limited data available from randomized trials, paracetamol and IND seem to have similar effects. However, there have been no reports of the use of paracetamol for hsPDA in

BACKGROUND: Tachyarrhythmia has been thought to be an absolute contraindication for the Fontan operation.
PATIENTS AND METHODS: Three patients, 22, 9, and 11 years of age, diagnosed as having atrioventricular atresia combined with the Wolff-Parkinson-White syndrome underwent surgical treatment. Each had drug-resistant atrioventricular tachycardia that required direct cardioversion. Two patients with tricuspid atresia had an intermittent right-sided accessory pathway (ACP), and one with mitral atresia had a concealed left-sided ACP. The ACP was divided using an epicardial approach in two patie

INTRODUCTION: Migraine is one of the leading causes of disability worldwide. Neuromodulation is a non-pharmacologic class of treatment for migraine. Cefaly is a neuromodulatory device that modulates pain through peripheral nerve stimulation of the supraorbital and supratrochlear nerves, leading to central pain modulation through the trigeminal nucleus caudalis (TNC).
AREAS COVERED: This review article discusses the physiology of migraine with reference to neuromodulation, summarizes the available evidence for the use of Cefaly in acute and preventive treatment of migraine, including studies t

American journal of transplantation : official journal of the American Society of Transplantation and the American Society of Transplant Surgeons2020eng
The approach to transplantation in human immunodeficiency virus (HIV)-positive patients has been conservative due to fear of exacerbating an immunocompromised condition. As a result, HIV-positive patients with diabetes were initially excluded from beta cell replacement therapy. Early reports of pancreas transplant in patients with HIV described high rates of early graft loss with limited follow-up. We report long-term follow-up of islet or pancreas transplantation in HIV-positive type 1 diabetic patients who received a kidney transplant concurrently or had previously undergone kidney transplan

When providing care for patients who are discharged from the hospital after experiencing acute coronary syndrome (ACS), several issues should be addressed. Drug regimens should be reviewed to ensure that patients are taking appropriate drugs, including antiplatelet agents, angiotensin-converting enzyme inhibitors/angiotensin II receptor blockers, aldosterone antagonists, beta blockers/calcium channel blockers, cholesterol-lowering drugs, and nitroglycerin. The review also should confirm that patients understand when and how to take their drugs, and that there are no obstacles (eg, cost) that m

BACKGROUND: Psychiatrists treating patients with drug and alcohol addiction currently consider these afflicions to be mental disorders. If patients are so mentally disturbed that they are a danger to themselves or others, then compulsory hospitalisation seems to be an acceptable treatment option. However, it would seem that at present this solution is not normal practice in addiction care.
AIM: To describe the indications for compulsory hospitalisation when mental disorders associated with addiction and withdrawal cause risks and dangers.
METHOD: Discussion of the indications for compulsory

Strontium ranelate is one of the first-line agents with proven anti-fracture activity used in the therapy of post-menopausal osteoporosis. Its mechanism of action makes it, however, different from other drugs, since it simultaneously stimulates two reverse processes: bone formation and bone resorption. The action of the agent depends on various mechanisms, including the activation of calcium receptors, localised on osteoblasts and osteoclasts, and on the influence on the OPG/RANKL system. The drug effectively prevents spinal, hip and extravertebral fractures. The agent's anti-fracture efficacy
