ABSTRACTRATIONALE: Immune-associated hemolytic anemia is an uncommon and potentially life-threatening complication in patients with choledocholithiasis and acute cholangitis. Its occurrence during the perioperative period remains rarely reported.
PATIENT CONCERNS: A 69-year-old woman presented with right upper abdominal pain and was diagnosed with choledocholithiasis and acute cholangitis. During hospitalization, she developed sudden confusion and shock, with her hemoglobin rapidly declining to 26 g/L.
DIAGNOSES: The patient was diagnosed with acute cholangitis and immune-related hemolytic a

The patient was a 38-year-old man who was admitted to our hospital with fever, vomiting and unclear speech. Blood tests showed hemolytic anemia, thrombocytopenia, and elevated creatinine, and a Coombs test was negative. ADAMTS13 activity was <1% and ADAMTS13 inhibitor was 0.7 BU/ml, so we diagnosed acquired thrombotic thrombocytopenic purpura (aTTP). We started plasma exchange and prednisolone 1 mg/kg on the day of admission (day 1). The platelet count recovered to normal on day 5, and hemolytic findings improved. However, symptoms worsened again, and thus we started rituximab for refractory a…

An 18-year-old woman complained of fever and edema and was admitted to Showa University Hospital for treatment of thrombocytopenia and deteriorating renal function. Laboratory studies demonstrated the presence of lupus anticoagulant (LA), prolongation of prothrombin time, hemolytic anemia, a negative Coombs' test, the absence of antinuclear antibodies, and a normal fibrinogen level. Renal biopsy revealed mesangial hypercellularity, severe endocapillary cell damage, and double contour of the basement membrane walls. Immunofluorescence studies demonstrated focal, peripheral, and finely granular

INTRODUCTION: Brucellosis is one of the most common zoonotic infections in the world. Cardiac complications of the disease are usually in the form of endocarditis, and, to a lesser extent, in the form of myopericarditis.
CASE: We report the case of a 34-year-old female admitted with signs of fever, nausea, and headache. The patient came from Libya, where she drank unpasteurized milk. Whright-Coombs test and blood confirmed Brucella species infection, while transoesophagal echocardiogram revealed a thrombotic mass on the tricuspid valve that required cardiac surgery and antibiotic treatment, l…

A 58-year-old man was admitted with generalized lymphadenopathy. On admission, the patient showed polyclonal hyper-gammopathy in a blood examination, positive results in the direct/indirect Coombs test, and an elevated cold agglutinin titer. Autoimmune thrombocytopenia with a high level of platelet-associated immunoglobulin G complicated the patient's condition. An enzyme immunoassay kit for human immunodeficiency virus (HIV) recombinant proteins p24, gp41, and gp36 showed positive results. Western blot analysis showed the presence of antibodies cross-reacting with HIV p24 gag protein. HIV RNA…

We identified 3 patients with autoimmune myelofibrosis (AM) lacking American Rheumatism Association criteria for systemic lupus erythematosus (SLE). They had 1 or 2 cytopenias and lacked serologic evidence for SLE. Autoimmune features included psoriatic arthritis and positive direct Coombs test (DCT) result, DCT-positive autoimmune hemolytic anemia, and synovitis with polyclonal hypergammaglobulinemia. Bone marrow biopsy specimens from each patient were evaluated by routine morphologic and immunohistochemical examination. They demonstrated marked hypercellularity (2 cases) or hypocellularity (…

Felty's syndrome is diagnosed when a patient shows both splenomegaly and leukocytopenia of various degree during the course of rheumatoid arthritis (RA). The accompanying immunologic abnormalities (e.g., antinuclear antibody, antiplatelet antibody, and hypocomplementemia) also characterize Felty's syndrome, but some authors may regard these abnormalities as a transitional form into overlap syndrome [RA + systemic lupus erythematosus (SLE)]. Here we reported a female case of Felty's syndrome who showed marked thrombocytopenia and severe hypocomplementemia. Thrombocytopenia had been refractory a

A 4 month old girl developed a severe hemolytic uremic syndrome (HUS) following pneumococcal sepsis and meningitis. As a result of the hemolytic anemia and thrombocytopenia with associated gastrointestinal bleeding several red blood cell and thrombocyte transfusions became necessary. No plasma was transfused to the patient and all cellular blood components to be transfused were washed thoroughly in order to avoid the administration of eventually dangerous donor anti-T antibodies. Continuous peritoneal dialysis was performed for 23 days until sufficient spontaneous urine production was resumed.

To assess hemolysis in hyperbilirubinemic infants with ABO isoimmunization, we measured the carboxyhemoglobin (HbCO) concentrations in full-term infants with ABO isoimmunization during the first week of life. Two groups of infants, ABO compatible (n = 34) and ABO incompatible (n = 30), were further divided into two groups with and without hyperbilirubinemia. All the infants from ABO incompatible pregnancies showed a positive indirect Coombs' test. The groups with hyperbilirubinemia included infants with maximum serum total bilirubin levels about 15 mg/dL and infants treated by phototherapy or

A 48-year-old female patient with known autoimmune hemolytic anemia (AIHA) presented with severe anemia and pancytopenia. Despite typical hemolysis parameters and a positive direct antiglobulin test confirming the presence of warm autoantibodies, erythropoiesis was found to be hyporegenerative. This constellation suggested a combined mechanism of increased erythrocyte destruction and impaired hematopoietic capacity. Further diagnostic workup revealed active parvovirus B19 infection. This case highlights that the coincidence of AIHA and parvovirus B19 infection can lead to an atypical hematolog

BACKGROUND: The clinical and laboratory features of hemolytic disease of the newborn can be challenging to diagnose during pregnancy in the apparent absence of a blood group antibody. Low-frequency antibodies go undetected due to the lack of appropriate antigen-positive reagent red blood cells (RBCs).
CASE REPORT: A pregnant woman of Southeast Asian descent was referred to a maternal-fetal medicine outpatient clinic due to a complicated obstetric history and a negative antibody screen. This initial visit at 29 weeks and 0 days' gestational age (GA) was unremarkable. A hydropic infant, born at

Serologic testing using the indirect antiglobulin test (IAT) is known to be insufficient to determine the clinical significance or insignificance of a given antibody to red blood cells (RBC), particularly in cases of antibodies to high-prevalence antigens, such as anti-Ge or anti-Yta. An in vitro functional cellular assay, the monocyte monolayer assay (MMA), has been studied for more than 40 years for its potential use to differentiate between clinically significant and insignificant RBC antibodies. The MMA has recently been used to select donor blood for transfusion into patients having a ser

OBJECTIVE: Systemic lupus erythematosus (SLE) is an autoimmune disease that may present manifestations that resemble other diseases. Visceral leishmaniasis (VL) is a parasitic infection whose hallmarks may mimic SLE symptoms. Here, we report a case series and evaluate the published, scientific evidence of the relationship between SLE and VL infection.
METHODS: To assess original studies reporting cases of VL-infected patients presenting manifestations that are capable of leading to inappropriate suspicions of SLE or mimicking an SLE flare, we performed an extensive search in several scientifi

UNLABELLED: In patients with hemolytic anemia associated with spherocytosis, differential diagnosis has to be made whether the hemolysis is immune-mediated or of non-immune origin. We report a case of hereditary spherocytosis in a 12-yr-old male child, in whom flow-assisted diagnosis was made. In this case, diagnosis was not determined because routine laboratory workups for hereditary spherocytosis yielded discrepant

IgA deficiency (IgAD) is frequently associated with autoimmune phenomena. The aim of this study is to evaluate the frequency of 22 different autoantibodies in 60 patients with IgAD and to examine the physical and other laboratory findings of the suspected cases for autoimmune diseases. The evaluated autoantibodies were Anti-nuclear antibody (ANA) profile (autoantibodies against RNP/Sm, SS-A, Ro-52, SS-B, Scl-70, Pm-Scl, Jo-1, centromere B, PCNA, dsDNA, nucleosomes, histones, ribozomal P-protein, AMA-M2), anti-cardiolipin IgG and IgM, anti-neutrophilic cytoplasmic antibodies (ANCA), rheumatoid

Two premature infants who had episodes of hemolysis following blood transfusion are described. No incompatibility was found between donor and infant in either case. Further investigation of the donors' blood excluded other possible causes of hemolysis. The only anomaly found was that the donors were glucose-6-phosphate dehydrogenase (G6PD) deficient, while the infants had normal G6PD activity. Our report describes a new possible cause of hemolysis in preterm infants, and suggests that screening for G6PD deficiency may be necessary before blood is transfused to a preterm infant.
