A metabonomic approach identifies human urinary phenylacetylglutamine as a novel marker of interstitial cystitis.

An ultra-performance liquid chromatography-mass spectrometry (UPLC-MS) based metabonomic approach was applied to identify a candidate metabolite with not known to be associated with interstitial cystitis (IC). IC is a chronic clinical syndrome associated with urinary frequency and urgency and/or pelvic pain. The ability to non-invasively diagnose the early stage of IC would be important for improving the patient's quality of life. The current standard IC diagnosis is cystoscopy, which is invasive and painful. Urine samples from the following were taken and analyzed: 10 IC patients, 10 bacteria

Multiple intravesical instillations of low-dose resiniferatoxin in the treatment of refractory interstitial cystitis.

BACKGROUND: Current medications used in the treatment of interstitial cystitis (IC) have limited efficacy. This prospective study investigated the efficacy of multiple intravesical instillations of resiniferatoxin (RTX) at the concentration of 10 nM. METHODS: Patients with proven IC previously treated with traditional medications for more than 6 months without clinical benefit were enrolled. They were excluded if bladder outlet obstruction or urinary tract infection was present. Intravesical instillation of low-dose RTX (10 nM) once weekly for 4 weeks was performed at the outpatient departmen

Interstitial cystitis: a critique of current concepts with a new proposal for pathologic diagnosis and pathogenesis.

Interstitial cystitis (IC) has continued to be an unresolved problem in clinical urology despite intense investigation over the past 16 or more years. Its etiology and pathogenesis are still undetermined, and its pathologic diagnosis is essentially one of exclusion, with no specific or clear criteria. In this review, current concepts of the etiology/pathogenesis and pathology are critically analyzed, new pathologic observations summarized, and a proposal of neurogenic inflammation as the primary pathogenetic factor is presented in the context of all currently available information. The popular

MiR-9-enriched mesenchymal stem cells derived exosomes prevent cystitis-induced bladder pain via suppressing TLR4/NLRP3 pathway in interstitial cystitis mice.

BACKGROUND: Inflammatory response of central nervous system is an important component mechanism in the bladder pain of interstitial cystitis/bladder pain syndrome (IC/BPS). Exosomes transfer with microRNAs (miRNA) from mesenchymal stem cell (MSCs) might inhibit inflammatory injury of the central nervous system. Herein, the purpose of our study was to explore the therapeutic effects by which extracellular vesicles (EVs) derived from miR-9-edreched MSCs in IC/BPS and further investigate the potential mechanism to attenuate neuroinflammation. METHODS: On the basis of IC/BPS model, we used variou

Metabotropic glutamate receptor 5 expression associates with pain and inflammatory pathways in interstitial cystitis.

Interstitial cystitis (IC)/bladder pain syndrome (BPS) is a chronic condition with severe pelvic pain and urinary symptoms significantly impairing quality of life. This study investigated the clinical relevance of the metabotropic glutamate receptor (mGluR) family in IC/BPS. Bladder biopsy samples were taken from 61 patients, including 42 Hunner-type IC, 11 non-Hunner type IC, and 8 controls without IC. Gene expression analysis revealed that mGluR2, mGluR3, and mGluR5 were significantly elevated in patients with IC/BPS compared to controls. Among these, mGluR5 showed the strongest association

Low-Energy Shockwave Therapy for Interstitial Cystitis/Bladder Pain Syndrome From Bench to Clinic.

There is no effective and durable treatment for patients with interstitial cystitis/bladder pain syndrome (IC/BPS). There is thus an urgent medical need to develop a novel treatment for it. Low-energy shockwave (LESW) therapy, a nondrug treatment, is known to exert anti-inflammatory, pain-reducing, and tissue-regenerating effects. LESW therapy has been widely used in orthopedics, sports medicine, and urology, with a high safety profile. Previous animal studies on a rat model of interstitial cystitis revealed that LESW therapy can reduce bladder inflammation and pain, while also facilitating ti

Research Methodology in Acupuncture for Managing Interstitial Cystitis/Bladder Pain Syndrome: A Scoping Review.

BACKGROUND: Acupuncture and moxibustion are being explored as complementary therapies with promising potential in managing interstitial cystitis/bladder pain syndrome (IC/BPS). While some studies indicate possible benefits, their scientific basis and clinical efficacy remain subjects of debate, partly due to methodological flaws in study designs involving acupuncture and related modalities, which warrant further attention and discussion. The main objective is to analyze and evaluate the research methodologies used in existing literature on acupuncture for managing IC/BPS, providing insight int

Integrating biomarker clustering for improved diagnosis of interstitial cystitis/bladder pain syndrome: a review.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is a debilitating chronic condition characterized by pelvic pain and urinary disturbances, making its diagnosis challenging due to overlapping symptoms with various lower urinary tract disorders. Current diagnostic methods primarily rely on subjective evaluations, leading to significant difficulties in accurately identifying and managing IC/BPS. This review explores the potential of clustering different disease biomarkers to enhance diagnostic precision for IC/BPS. We examine current research identifying non-invasive biomarkers, including in

Comparative Risk of Developing Interstitial Cystitis With Childhood Gastrointestinal, Urological, Autoimmune, or Psychiatric Disorders.

AIMS: Interstitial cystitis (IC) is a chronic urological condition associated with significant discomfort, posing diagnostic and therapeutic challenges. Although its etiology remains unclear, early-life conditions such as gastrointestinal (GI) disorders, urological anomalies (UA), psychiatric disorders (PD), and autoimmune diseases (AD) have been hypothesized as potential risk factors for developing IC in adulthood. This study aims to investigate these associations by conducting a retrospective cohort analysis utilizing data from the TriNetX US Collaborative Network, encompassing over 118 mill

Baicalin ameliorates interstitial cystitis/bladder pain syndrome by inhibiting the TLR4/NF-κB pathway.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is a chronic inflammatory bladder disorder characterized by pelvic pain and urinary symptoms, yet effective therapeutic options remain limited. Baicalin (BA), a bioactive flavonoid derived from Scutellaria baicalensis, exhibits anti-inflammatory and antioxidant properties, though its role in IC/BPS has not been fully elucidated. This study demonstrates that BA significantly ameliorates cyclophosphamide-induced urination frequency and bladder tissue damage and reduces serum levels of pro-inflammatory cytokines, including TNF-α, IL-6, and IL-1

Evaluation of outcomes of clinical phenotyping-based treatment for bladder pain syndrome/interstitial cystitis.

INTRODUCTION: Bladder pain syndrome/Interstitial cystitis (BPS/IC) is clinically of diverse types because different causes contribute to the development of their symptoms. It is important to classify patients into various groups based on the possible etiopathogenesis of their condition. Treatment may be tailored to each specific group according to the possible cause. METHODOLOGY: Twenty-five patients diagnosed with BPS/IC were categorized into four different clinical phenotypes (CP) based on their history of symptoms, allergy, dysfunctional voiding, neuropathic pain, and the presence of Hunne
