Clinical correlations with dermatomyositis-specific autoantibodies in adult Japanese patients with dermatomyositis: a multicenter cross-sectional study.

OBJECTIVE: To clarify the association of clinical and prognostic features with dermatomyositis (DM)-specific autoantibodies (Abs) in adult Japanese patients with DM. DESIGN: Retrospective study. SETTING: Kanazawa University Graduate School of Medical Science Department of Dermatology and collaborating medical centers. Patients A total of 376 consecutive adult Japanese patients with DM who visited our hospital or collaborating medical centers between 2003 and 2008. MAIN OUTCOME MEASURES: Clinical and laboratory characteristics of adult Japanese patients with DM and DM-specific Abs that inclu…

HLA class II haplotype and autoantibody associations in children with juvenile dermatomyositis and juvenile dermatomyositis-scleroderma overlap.

OBJECTIVES: To investigate a large cohort of children with juvenile dermatomyositis (JDM), and those with JDM-scleroderma (JDM-SSc) overlap, using detailed serological analysis, HLA class II genotyping and clinical characterization. METHODS: Children (114) with JDM were recruited, and clinical data collected, through the JDM National Registry and Repository (UK and Ireland). Sera were assayed for ANA using standard immunofluorescence techniques and specific antibodies characterized using ELISA, immunodiffusion and radioimmunoprecipitation. Patients and controls (n = 537) were genotyped at the…

[Isolated muscular tuberculosis in dermatomyositis treated with corticosteroids. Developing outbreak of dermatomyositis under rifampicin treatment. 2 cases].

The authors report two cases of muscular tuberculosis in patients with dermatomyositis treated with steroids. Antituberculous therapy with Rifampicin led to a severe relapse of the dermatomyositis in both cases, suggesting enzymatic induction inhibiting the action of the steroids.

Anifrolumab in Refractory Dermatomyositis and Antisynthetase Syndrome.

Dermatomyositis (DM) and antisynthetase syndrome are rare autoimmune disorders within the spectrum of inflammatory myopathies, typically characterized by cutaneous and muscular inflammation along with systemic manifestations. This case report highlights the evolving treatment strategies for inflammatory myopathies, with a focus on the use of anifrolumab, a type-1 interferon receptor blocker, in a 29 year-old female with refractory DM/antisynthetase syndrome. The patient presented with classic DM features, including heliotrope rash, Gottron's papules, and malar erythema, but lacked significant …

Juvenile dermatomyositis: a severe and atypical presentation.

INTRODUCTION: Juvenile dermatomyositis (JDM) is a rare immune-mediated disease, characterised by proximal muscle weakness and typical skin rashes1. We present a patient with severe JDM, to highlight the importance of a timely diagnosis and early initiation of treatment. CASE DESCRIPTION: A 9-year-old girl presented to the hospital due to asthenia, rash, generalized oedema, and inability to walk. At observation, the patient had dysphonia, dysphagia, proximal muscle weakness, petechial rash, skin ulcers, and anasarca. The levels of creatine kinase, aldolase, transaminases, and ferritin were ele…

Cardiac evaluation of patients with juvenile dermatomyositis.

BACKGROUND: The present study aims to evaluate possible cardiac involvement in juvenile dermatomyositis (JDM) patients by conventional methods and cardiac magnetic resonance imaging (MRI) along with a systematic review of the literature on cardiac features in JDM. METHODS: The study group consisted of JDM patients who underwent cardiac MRI. We conducted a systematic review of the published literature involving JDM patients with cardiac involvement. RESULTS: In the present study, although baseline cardiologic evaluations including electrocardiography and echocardiography were within normal li…

An Unusual Presentation of Dermatomyositis With Muscle Hypertrophy.

Peripheral nerve hyperexcitability is a rare disorder characterized by spontaneous motor unit activity. Although peripheral nerve hyperexcitability is seen in multiple immune-mediated neurological conditions, an association with dermatomyositis has rarely been reported. We present a 65-year-old woman with serological and muscle biopsy features of dermatomyositis who also developed marked muscle hypertrophy, stiffness, and delayed relaxation along with electrodiagnostic features of peripheral nerve hyperexcitability such as that seen in Isaacs syndrome.

[Juvenile dermatomyositis, the dermatologist's point of view].

Juvenile dermatomyositis is a rare multi-system auto-immune disease, particularly causing inflammation of skin and muscles of children. The diagnosis is based on the clinical picture with typical cutaneous lesions, which frequently are the first signs of the disease in contrast to muscle involvement. Muscular MRI is nowadays the first line investigation to diagnose myositis. Recently specific auto-antibodies have been detected allowing a better understanding of the disease and being important prognostic factors. An early diagnosis and aggressive treatment is crucial to induce remission of the …

[Advances in the diagnostics and treatment of juvenile dermatomyositis].

Juvenile dermatomyositis (JDM) is by far the most frequent inflammatory myopathy in childhood and adolescence. It is clinically characterized by inflammatory changes of the skin and muscles but as a multisystemic disease can also affect the skeletal system, the gastrointestinal tract, lungs and heart. Intrinsic (multigenetic risk) and extrinsic factors (triggers) are involved in the pathogenesis resulting in endothelial damage, involvement of fascies, activation of the interferon system and autoimmune reactions including formation of myositis-specific autoantibodies (MSA). In contrast to derma…
