Comparison of the use of the humerus intramedullary nail and dynamic compression plate for the management of diaphyseal fractures of the humerus. A randomised controlled study.

The aim of this study was to compare the results of the humerus intramedullary nail (IMN) and dynamic compression plate (DCP) for the management of diaphyseal fractures of the humerus. Forty-seven patients with diaphyseal fracture of the shaft of the humerus were randomised prospectively and treated by open reduction and internal fixation with IMN or DCP. The criteria for inclusion were grade 1 or 2a compound fractures, polytrauma, early failure of conservative treatment and unstable fractures. The patients with pathological fractures, grade 3 open fractures, refractures and old neglected frac

Identification and classification of tibioperoneal diaphyseal toxopachyosteosis (Weismann-Netter-Stuhl syndrome): based on two new cases and a review of the literature.

Using two new cases and 70 case reports in the literature as a starting point, the authors focus on the Weismann-Netter-Stuhl syndrome. Weismann-Netter and Stuhl reported the first cases of tibioperoneal diaphyseal toxopachyosteosis in 1954. This syndrome is defined as an anomaly of the diaphyseal part of both tibiae and fibulae with posterior cortical thickening and anterior-posterior bowing. This anomaly is usually bilateral and symmetrical and patients are short. The thickening of the fibula is true tibialisation and is the main feature and the only feature confirming diagnosis. Routine lab

Proximal forearm antegrade elastic stable intramedullary nailing for pediatric distal radius metaphyseal-diaphyseal junction fractures: A modified technique.

BACKGROUND: Distal radius metaphyseal-diaphyseal junction (MDJ) fractures in children are technically demanding, and the optimal fixation strategy remains controversial. The clinical performance of proximal-forearm antegrade elastic stable intramedullary nailing (A-ESIN) was compared with conventional retrograde crossed Kirschner-wire fixation (R-KW). METHODS: This retrospective study included 79 children treated for distal radius MDJ fractures at [blinded for review] between January 2018 and April 2024, with a minimum follow-up of 12 months. Thirty-eight underwent A-ESIN and 41 received R-KW

Rotational problems and clinical reflections after locked intramedullary nailing in diaphyseal femur fractures: A minimum follow-up of 5 years.

BACKGROUND: Rotational malalignment following intramedullary nailing (IMN) of femoral shaft fractures remains a clinically significant concern, with previous studies reporting variable incidence rates and inconsistent risk factors. This study aimed to determine the incidence of rotational malalignment after closed static-locked intramedullary nailing for adult diaphyseal femoral fractures, identify contributing etiological factors, and evaluate its clinical impact on functional capacity and quality-of-life metrics. METHODS: A retrospective cohort study was conducted involving 54 adults treate

Microbiology, risk factors, and clinical outcomes of low-grade infection in long-bone diaphyseal non-union after open fracture.

AIM: Our primary aim was to describe the microbiology of long-bone non-unions after open fracture. Then, we sought to determine the clinical and biological factors associated with the risk of positive intraoperative tissue cultures and the outcome after non-union surgery. METHODS: We retrospectively selected patients > 18 years, who had a primary planned surgical treatment of long-bone non-union after diaphyseal open fracture, with no previous history of infection. Infection was confirmed when growth of the same micro-organism was observed from at least two bone/tissue samples obtained at the

Corrective osteotomy of proximal diaphyseal deformity of the tibia and tibiotalocalcaneal arthrodesis fixed with a single long retrograde intramedullary nail - a case report.

The treatment of diaphyseal fractures aims to reestablish the length and angular and rotational alignment of the fractured bone so that the biological process of consolidation can occur. When these objectives are not achieved, deformities or pseudarthrosis may occur. The goal of the arthrodesis described in this paper is to treat pain, stabilize the ankle, and align the limb for plantigrade support, in a patient who suffered with a diaphyseal fracture of the tibia and evolved with mal union and posttraumatic arthrosis of the ankle. Level of evidence: IV - Case report.

Treatment of unstable forearm fractures at the metaphyseal-diaphyseal junction in children: antegrade ESIN vs. transepiphyseal intramedullary K-wire fixation.

BACKGROUND: Treatment of unstable forearm fractures in the metaphyseal-diaphyseal junction (MDJ) zone is still a matter of debate. Major drawbacks of all types of fixations include either invasiveness, technical impracticality, or lack of acceptance by patients. This study reports results after antegrade ESIN (a-ESIN) compared to transepiphyseal intramedullary K-wire (TIK) for unstable MDJ forearm fractures. METHODS: The MDJ of the forearm was defined as the square over the joints of both forearm bones subtracted with the square over the metaphysis of the radius alone. The data of 40 consecut

Shoulder problems after percutaneous antegrade intramedullary nailing in humeral diaphyseal fractures using contemporary straight third-generation nail.

BACKGROUND: Antegrade intramedullary nailing in humeral shaft fracture has been abandoned by certain orthopedic surgeons because of rotator cuff injury caused by first- and second-generation intramedullary nails (IMNs). However, only a few studies have specifically addressed the results of antegrade nailing for the treatment of humeral shaft fractures with a straight third-generation IMN; thus, complications need to be re-evaluated. We hypothesized that fixation of displaced humeral shaft fractures with a straight third-generation antegrade IMN with the percutaneous technique avoid shoulder pr

Bone Morphogenetic Protein-2 Rapidly Heals Two Distinct Critical Sized Segmental Diaphyseal Bone Defects in a Porcine Model.

INTRODUCTION: Segmental bone defects (SBDs) are devastating injuries sustained by warfighters and are difficult to heal. Preclinical models that accurately simulate human conditions are necessary to investigate therapies to treat SBDs. We have developed two novel porcine SBD models that take advantage of similarities in bone healing and immunologic response to injury between pigs and humans. The purpose of this study was to investigate the efficacy of Bone Morphogenetic Protein-2 (BMP-2) to heal a critical sized defect (CSD) in two novel porcine SBD models. MATERIALS AND METHODS: Two CSDs wer

Alignment and Fixation in Revision Total Knee Arthroplasty With Press-Fit, Offset, Diaphyseal-Engaging Stem Extensions, and Eccentric Reaming Technique.

BACKGROUND: A revision total knee arthroplasty must control limb alignment, often to address the cause of failure. Press-fit stems that engage the diaphysis with cement restricted to the metaphysis constitute one fixation technique. These long stems restrict coronal alignment of the prosthesis and as a result reduce the likelihood of extreme malposition. For the same reasons, long stems can make it difficult to manipulate alignment and achieve a specific coronal alignment angle. Nevertheless, femoral stems with a tight diaphyseal fit may still occupy a small range of varus-valgus positions due

Straight nail insertion through a laterally shifted entry for diaphyseal atypical femoral fractures with bowing: good indications and limitations of this technique.

PURPOSE: Inserting a straight (piriformis fossa entry) nail through the tip of the greater trochanter has been used for treating atypical femoral fractures (AFFs) with bowing. This study aimed to determine what degree of bowing can be successfully treated using a laterally shifted entry technique. METHODS: Twenty-three complete and six incomplete diaphyseal AFFs treated using the shifted entry technique were retrospectively analysed. Radiologic parameters and complications were evaluated. The complete AFFs were divided into two groups based on the severity of preoperative bowing: grade 0-II b

Laterally applied single bone plate option for fixation of complete diaphyseal fracture of a third metatarsal bone in a circus work pony.

BACKGROUND: Complete fractures of the third metacarpal/metatarsal bones (Mc/t3) are not common, but can occur in various situations and all types of horses, contributing for approximately one third of all long bone fractures in the horse, mostly related to external trauma or to high energy injuries. To stabilize Mc/t3 fractures in the horse, conservative management with walking casts and/or open reduction and internal fixation techniques are referred in the scientific literature, these last one generally by double platting application to bone surface in 90 degrees. In the present case will be
