Perforated diverticulum: rare complication of interposed substernal colon.

Substernal colon interposition is a good reconstruction option after salvage surgery for caustic ingestion injury. Although redundancy and reflux sometimes complicate these cases, there have been no reports in the English literature of perforated diverticula in interposed colon segments. We report a 23-year-old man who underwent substernal colon flap reconstruction after caustic ingestion injury. He had perforated colon diverticulitis develop, which led to a lung abscess 7 years later, followed by resection of the necrotic lung parenchyma and colon repair. This rare case illuminates the import

Robotic pelvic dissection as surgical treatment of complicated diverticulitis in elective settings: a comparative study with fully laparoscopic procedure.

BACKGROUND: Recently, minimally invasive treatment of complicated sigmoid diverticulitis is becoming a valid alternative to standard procedures. Robotic approach may be useful to allow more precise dissection in arduous pelvic dissection as in complicated diverticulitis. The aim of this study is to investigate effectiveness, potential benefits and short-term outcomes of robotic-assisted laparoscopic surgical resection, compared with fully laparoscopic resection in complicated diverticulitis. METHODS: Between January 2009 and December 2017, 156 consecutive patients with history of complicated

Diverticular disease: update on pathophysiology, classification and management.

Colonic diverticulosis is prevalent, affecting approximately 70% of the western population by 80 years of age. Incidence is rapidly increasing in younger age groups. Between 10% and 25% of those with diverticular disease (DD) will experience acute diverticulitis. A further 15% will develop complications including abscess, bleeding and perforation. Such complications are associated with significant morbidity and mortality and constitute a worldwide health burden. Furthermore, chronic symptoms associated with DD are difficult to manage and present a further significant healthcare burden. The pat

Pneumaturia and faecaluria: Symptoms leading to a life-saving diagnosis.

BACKGROUND: Colonic diverticulosis is often asymptomatic, but it can complicate bleeding, abscess and stricture. Pneumaturia and faecaluria are unexpected manifestations of colonic diverticulitis complicated by colovesical fistula formation. CASE PRESENTATION: This case report highlighted a 57-year-old man who presented with lower abdominal pain which was associated with pneumaturia and fecaluria. He was diagnosed with colovesical fistula after being misdiagnosed with other diseases from various clinic visits. Direct fluoroscopy was performed and a diverticular fistula of the sigmoid colon wa

Know the name: acute epiploic appendagitis-CT findings and review of literature.

Epiploic appendages are normal pedunculated peritoneal fat containing outpouchings bordering tenia coli on the anti-mesenteric surface of the colon, extending from caecum to the rectosigmoid. Functions are currently unknown, though some postulate them a blood reservoir. The epiploic appendages can become inflamed, with clinical presentations mimicking that of diverticulitis or acute appendicitis. However, unlike acute diverticulitis or appendicitis, epiploic appendagitis are treated conservatively with antibiotics. Currently, the estimated rate of correct preoperative diagnosis of epiploic app

The Assessment of Knowledge, Awareness and Practice Regarding Diverticulitis and Its Risk Factors Among the Population of Saudi Arabia.

Background The term "diverticula" refers to the existence of diverticula in the gastrointestinal tract but is mainly located in the sigmoid colon and is used to describe colonic diverticulosis. Diverticula, which are sac-like protrusions in the wall of the large bowel, are becoming more prevalent globally, in both developed and developing nations. This increase in occurrence is primarily attributed to changes in dietary and lifestyle patterns. Raising public awareness can potentially contribute to a decrease in the incidence of the disease and its associated complications. Aim This study aims

[The computed tomography of acute sigmoid diverticulitis].

PURPOSE: To study the ranking of CT in the diagnosis of sigma diverticulitis. Comparison of results of the colon monocontrast enema with those obtained via CT and histopathology as gold standard. MATERIAL AND METHODS: 32 patients were included who were operated on for sigma diverticulitis. In 30 patients both CT and monocontrast enema were performed and in two patients CT only. RESULTS: Acute inflammatory wall alterations are identified by CT with a sensitivity of 89.7% vs 96.3% via contrast enema. Pericolic reactions of the environment were seen via CT with 89.7% sensitivity. In case of cov

Distant free air is not a contraindication for definitive laparoscopic treatment of acute perforated diverticulitis: a multi-center experience.

The paradigm of emergency laparotomy with sigmoid resection and protective stoma has been challenged for perforated diverticular disease (PDD) with free extraluminal air. Early clinical stabilization could lead to interval laparoscopic resection without stoma within 2 weeks from perforation. Patients admitted for acute diverticulitis underwent abdominal computed tomography (CT) scan. When free air was seen, endoluminal enema was administred. All patients underwent assessement of clinical stability. In unstable patients, upfront emergency surgery was performed. Stable patients underwent a conse

[Management of simple acute diverticulitis : Towards a "less is more" approach].

Acute diverticulitis is the most common complication of diverticular disease, increasing in industrialized countries and in young people under 45 years of age. The modified Hinchey classification remains the most widely used and includes simple diverticulitis, i.e. localized inflammation without sepsis, and complicated diverticulitis from pericolic abscess to stercoral peritonitis. Recent studies recommend conservative management of uncomplicated forms. This article summarizes the management of simple acute left-sided diverticulitis based on the new recommendations and focusing on antibiotic t

[Right-sided diverticulitis mimicking acute appendicitis].

The probability diagnosis in two patients, women aged 43 and 41 years, who for the last few days had had pain in the right lower abdomen, without nausea or vomiting, was acute appendicitis; a third patient, a woman aged 49 with the same symptoms, had undergone appendectomy in the past. Peroperative findings and, in two patients, microscopy of the resected specimen showed diverticulitis in the caecum or ascending colon. This is an uncommon disease, which mimicks acute appendicitis. Treatment depends on the severity of the inflammation. In the absence of perforation or abscess, conservative trea

Toxicity of non-steroidal anti-inflammatory drugs in the large bowel.

Non-steroidal anti-inflammatory drugs (NSAIDs) may cause damage to the colon, either inducing a non-specific colitis or exacerbating colonic diverticulitis or inflammatory bowel disease. Symptoms of NSAID-induced colitis include abdominal pain, bloody diarrhoea and weight loss. Colonoscopy may show non-specific inflammation, ulceration or diaphragm-like stricture. As the mechanism of toxicity is still controversial, management should be to discontinue the drug, treat medically with metronidazole or sulphasalazine, and, in cases of life-threatening complications, treat surgically.

Intestinal endometriosis and its complications: case report and review.

Endometriosis affects 8%-15% of all women, and of these 3%-37% have intestinal involvement. Affected women are typically of childbearing age and of low parity, with a history of cyclic abdominal pain and progressive dysmenorrhea. Endometriosis may involve the rectum and sigmoid, ileum, cecum, and appendix, generating symptoms suggestive of acute obstruction, appendicitis, ileitis, diverticulitis, or colonic carcinoma. Thus, this entity should be included in the differential diagnosis of recurrent abdominal pain and other episodic bowel symptoms in women of childbearing age.

Colorenal Fistula in a Patient With Recurrent Urinary Tract Infections and Urinary Urgency: A Case Report.

Colorenal fistula is a rare condition resulting from chronic inflammation, malignancies, diverticulitis, or infection. This case examines a 68-year-old female with a history of urinary urgency, left back pain, and recurrent urinary tract infections. Anteroposterior non-contrast computed tomography scan showed a large stone in the renal pelvis and gas in the collecting system. Left ureteroscopy revealed multiple ureteral polyps. Retrograde pyelogram revealed contrast filling the left colon. Ureteral polyp biopsies were benign, and colonoscopy was negative for malignancy. This case describes the

[Diagnosis and treatment of vesico-intestinal fistula].

In a retrospective study of 28 patients with vesico-intestinal fistula, the maximal diagnostic latency was eight years. The most common causes were diverticulitis (16 patients) and carcinoma of the colon (4 patients). Recurrent urinary tract infections with pneumaturia and/or faecaluria were found in 23 patients. The diagnosis was most accurately confirmed by cystoscopy and barium enema which showed evidence of a fistula in 20 patients. Patients suffering from diverticulitis were managed with excellent results while surgical intervention in malignancy was associated with brief survival. Divert

Risk of colectomy after conservative treatment of diverticulitis of the left hemicolon complicated by abdominal or pelvic abscess: protocol of a systematic review and meta-analysis.

INTRODUCTION: Acute diverticulitis of the sigmoid colon is increasingly treated by a non-operative approach. The need for colectomy after recovery from a flare of acute diverticulitis of the left colon, complicated diverticular abscess is still controversial. The primary aim of this study is to assess the risk of interval emergency surgery by systematic review and meta-analysis. METHODS AND ANALYSIS: The systematic review and meta-analysis will be conducted in accordance to the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols statement. Role of probiotics in management of diverticular disease.

Patients with diverticular disease may experience a variety of chronic symptoms, including abdominal discomfort, bloating, and altered bowel habit. They are also at risk of complications, including hemorrhage, diverticulitis, abscess, and fistula formation. The potential role of abnormal colonic microflora in the pathogenesis of diverticular inflammation has led to investigation of novel therapies such as probiotics. Probiotics are microorganisms that may be of net benefit to humans when consumed. The rationale and safety of their use in diverticular disease is discussed and current literature
