The diagnosis of a symptomatic Meckel's diverticulum in an adult is uncommon. Still more infrequent is the presentation of a bleeding Meckel's diverticulum after childhood. We present a case report of a 24-year-old male with gastrointestinal hemorrhage secondary to a Meckel's diverticulum containing ectopic gastric mucosa. With the exception of a Meckel's Tc 99m pertechnetate scan, all other diagnostic procedures including Tc 99m-labeled red cell scan and angiography were negative.

A case of congenital vesical diverticulum in childhood is reported and 28 other cases in children from the Japanese literature are reviewed. A 9-year-old boy presented with hematuria and two-phase micturition. Excretory urogram and voiding cystourethrogram showed a diverticulum of the bladder on the left side and left vesico-ureteral reflex. In a Meckel's diverticulum, heterotopic tissue, inflammation and a vascular band co-existed. The band originating from the superior mesenteric artery, was separated from the mesentery and represented a remnant of the vitelline artery.

A rare case is described in which a diverticulum of the middle portion of the right Fallopian tube was demonstrated by hysterosalpingography. The patient aged 26 years, had a history of a ruptured left mid-tubal pregnancy 8 years previously and was suffering from infertility. The radiographic appearance of the lesion is characteristic, the aetiology is obscure and treatment should be conservative.

Solitary diverticula of the large bowel are comparatively rare respect the much more frequent multiple diverticula. Such diverticula are found mainly in the caecum and ascending colon and are true diverticula, i.e. their wall is made up of all the layers of the bowel wall. Meckel's diverticulum usually presents as a diagnostic problem. Its most common clinical manifestations--lower GI bleeding, small bowel obstruction, and features suggesting acute appendicitis--are fairly well known. Other clinical presentations include a Meckel's diverticulum in an omphalocele, lower GI bleeding followed by perforation, perforation secondary to blunt trauma, and presentation as iron deficiency anemia with and without episodes of overt hemorrhage. Illustrative cases emphasize indications for and usefulness of abdominal scanning as a diagnostic aid.

There are no well-substantiated reports, to our knowledge of a Meckel's diverticulum arising outside the limits of the terminal ileum. We saw a diverticulum located in the proximal jejunum that fulfilled the anatomic and histologic criteria for the diagnosis of a Meckel's diverticulum. This was an incidental finding during the treatment of a sliding esophageal hiatus hernia and caused no apparent difficulty to the patient.
