BACKGROUND: The morbidity and mortality after living donor liver transplantation (LDLT) in children with Alagille syndrome (AGS) are complex because of the multisystem involvement of the disease. Evidence on long-term outcomes and donor selection remains limited.
METHODS: This multicenter retrospective study included 49 pediatric patients with AGS who underwent LDLT between 2001 and 2020 at eight institutions in Japan, Korea, and Turkey. Clinical characteristics, transplant indications, donor selection, posttransplant growth, renal function, and complications were analyzed.
RESULTS: The medi…

The progressive increase in the number of liver transplantation candidates has brought with it a consequent increase in waiting list mortality, making it necessary to revise donor selection criteria and to analyze the factors that optimize outcomes. This retrospective observational study of 1802 liver transplantations performed in Andalusia between 2000 and 2010 analyzes the outcomes from donors aged 70 years or older (n = 211) in terms of survival rates of the graft and the recipient, the type of transplant, donor age, and DMELD (Donor-Model for End-Stage Liver Disease) score. The most freque…

Lyme disease is caused by a spirochete, Borrelia burgdorferi, and represents a potential transfusion hazard. Some Borrelia burgdorferi-infected blood donors may not be disqualified by standard donor selection procedures, thus possibly transmitting the disease. In a follow-up of 14 recipients of blood products donated by such a donor, no clinical signs or serologic evidence of a transfusion-transmitted borreliosis could be demonstrated.

BACKGROUND: Allogeneic hematopoietic cell transplantation (HCT) and solid organ transplantation (SOT) have evolved into successful, curative treatments for many severe congenital and acquired diseases. Both use medical products of human origin and should therefore have overarching regulatory frameworks. Both require critical decisions about donor selection, donor/recipient matching, immunosuppression, and long-term care, all tasks best performed by a trained, highly specialized multidisciplinary team. Both need committed institutions and governmental support for their success. Whereas the main…

The increasing integration of genomic medicine into routine medical care brings to light issues of complexity and uncertainty in the interpr … tial recipient. We discuss the factors that need to be considered, such as how this might influence donor selection, the potential for donor-derived leukaemia and the issues of offering testing to other family members.Popular discourse suggests genomics can offer clarity and transformation in healthcare. This case highlights the ethically complex nature of decisi …

Over the recent decades, requests for kidney transplants have increased in numbers. To cope with scarcity of organs' availability compared to the growing demand, the scientific community has started considering donors who were previously excluded. Recent literature has shown that transplants from these nonstandard donors have outcomes that are better compared to those of patients who remain on dialysis while on the waiting list in terms of both quality of life and life expectancy, with results comparable to transplants from standard donors. Yet, past studies have focused on various classificat

BACKGROUND: Confidential Unit Exclusion (CUE) was introduced when earlier-generation assays had lower sensitivity and longer infectious window periods for transfusion-transmitted infections. Improvements in serological and molecular testing have raised questions about its contemporary utility. This study evaluated the use and predictive performance of CUE at São João Local Health Unit Blood Bank, Porto - Portugal.
METHODS: A retrospective descriptive study assessed all donations with CUE between January 1, 1999 and August 5, 2025. Donor demographics, CUE outcomes, and laboratory results were

From the infectious diseases perspective, the safety of the US blood supply, already high-level, continues to improve via multiple mechanisms, making transfusion-transmitted infections (TTIs) now rare. Blood supply safety in 2026 rests on multiple, interdigitating cornerstones including: 1) updated donor questionnaires and guidances for blood collection centers from the Food and Drug Administration, 2) the long-term commitment and expertise of national organizations (e.g., the American Red Cross and the Association for the Advancement of Blood and Biotherapies), 3) screening of donor blood pro

Cancer development in the remnant kidney in living kidney donors represents a challenging process in terms of patient management. Total nephrectomy is the preferred method for tumors exceeding 7 cm in size. In the case presented here, partial nephrectomy was preferred because the patient was a prior living kidney donor. On the other hand, being an organ donor always creates concerns for long-term safety and survival. The guidelines on the evaluation and care of living kidney donors have generally focused on assessment of the risk for chronic kidney disease in donors and donor-to-recipient infe

OBJECTIVE: The objective of this study was to examine the Borrelia seroprevalence among blood donors in Southeast Brazil.
BACKGROUND: There is evidence that Borrelia spirochetes are circulating in Brazil; however, there are no studies that characterise these bacteria and investigate their seroprevalence in the Brazilian population. Such a situation, combined with a recent outbreak of tick-borne Rocky Mountain spotted fever in the São Paulo state demonstrates the increasing role of ticks as arthropod vectors in Brazil.
METHODS: For the purpose of the study, 452 blood donors from Ribeirão Pret

OBJECTIVES: This study aimed to identify the emerging/reemerging pathogens in blood donation samples.
BACKGROUND: A metagenomic analysis has previously been used to look for pathogens but in this study, the relationship with aminotransferase (ALT) is described.
METHODS/MATERIALS: Excluding samples reactive to hepatitis B virus, hepatitis C virus, human immunodeficiency syndrome virus or syphilis and plasma samples were stratified into three groups of ALT levels (IU/L): A ≤ 50, B 51 to 69 and C ≥ 70, respectively. Each group was mixed in a pool of 100 samples, from which DNA and cDNA librarie

BACKGROUND: The accuracy of computed tomography (CT) for detecting donor hepatic steatosis (HS) before liver transplantation is not well established.
METHODS: A meta-analysis was performed to determine the accuracy of CT for HS detection in liver donor candidates. Pooled sensitivity, specificity, positive and negative likelihood ratios, hierarchical summary receiver operating characteristic (HSROC) curves, and the area under the curve (AUC) were estimated using HSROC and bivariate random-effects models.
RESULTS: Twelve studies involving 1782 subjects were eligible for this meta-analysis. For

HLA haplotype frequencies in a volunteer bone marrow donor registry should reflect the frequencies of potential transplant recipients served by that registry, a challenge in a country with diverse subethnicities of immigrants from Eastern and Western cultures, such as Israel. We evaluated the likelihood of finding suitable donors for hypothetical patients drawn from defined subethnicities in the Ezer Mizion Bone Marrow Donor Registry (EM BMDR) from donors both within and outside the registry now and during the coming decade. On average, bioinformatics modeling predicts that, given current dono

BACKGROUND: In Italy, the circulation of human immunodeficiency virus (HIV) has expanded to include population groups that do not perceive themselves to be "at risk" of HIV infection and who do not even consider undergoing HIV testing. The aim of this study was to describe the socio-demographic and behavioural characteristics, and perceived risk of HIV infection in a sample of blood donors who reported never having been tested for HIV.
MATERIALS AND METHODS: A questionnaire was administered to a sample of donors who called the Italian National AIDS/STI Help Line and reported never having been

BACKGROUND AND OBJECTIVES: During the 1918, pandemic blood components were successfully used to treat severe influenza pneumonia. A Proof of Principle trial investigating the clinical benefit of convalescent plasma was proposed in the 2009 H1N1v epidemic with the aim of screening donors for high titre antibody in order to stockpile plasma packs to be used for treatment for severe pneumonia.
MATERIALS AND METHODS: Serum samples were collected from donors. IgG antibody capture format enzyme-linked immunoassays using recombinant proteins (GACELISAs) were compared with microneutralization (MN) an

Bone graft substitutes have become progressively more widely used, and are currently heavily marketed. To make intelligent decisions, a complete knowledge of autograaft and allograft bone healing is essential, including the definition of "sterile". Differences in donor selection and tissue processing may confound the user not familiar with the implications of these different approaches. Specific products include demineralized bone matrix (DBM), specific growth factors (recombinant BMP's), ceramic grafts, and platelet-rich plasma (PRP). There are a number of useful applications of bone graft su…

The long-term shortage of livers available for transplantation has spurred the development of many strategies to bolster the donor organ supply. One particularly innovative strategy is domino liver transplantation in which a select group of liver transplant recipients can donate their explanted native livers for use as liver grafts in other patients. Several hereditary metabolic diseases (such as familial amyloid polyneuropathy, maple syrup urine disease, and familial hypercholesterolemia) are caused by aberrant or deficient protein production in the liver, and these conditions can be cured wi

Real-time RT-PCR has become the method of choice for automated detection of viral RNA target sequences in the clinical laboratory. Besides commercially available certified test systems, a variety of so-called in-house methods have been described in the literature. Generally, appropriate validation and continuous quality control are mandatory if these in-house-developed assays are used in clinical diagnostics. In this chapter, an in-house HIV-1 real-time RT-PCR assay for blood donor screening is described. The procedure includes the pooling of plasma samples, viral RNA isolation, and subsequent
