Hemorrhages in the root of the tongue in fire fatalities: the incidence and diagnostic value.

Hemorrhages in the root of the tongue have been considered to be a finding associated with asphyxiation. The aim of the present study was to examine the incidence and diagnostic value of the lingual hemorrhages in fire fatalities with reference to the related pathological and toxicological findings, in comparison with asphyxiation and drowning cases. In fire fatalities (n=90), small to marked hemorrhages were observed in 26 cases (28.9%). In the reference groups (asphyxiation and drowning), the hemorrhages were frequently observed in ligature strangulation (n=10/15), manual strangulation (n=5/…

A quantitative RT-PCR assay of surfactant-associated protein A1 and A2 mRNA transcripts as a diagnostic tool for acute asphyxial death.

Although an immunohistochemical investigation of pulmonary surfactant-associated protein A (SP-A) suggested a characteristic increase in fatal asphyxiation, no particular change was observed in the total amount of SP-A mRNA. SP-A is encoded by two highly similar genes, SP-A1 and SP-A2, which are differentially regulated in the expression. In the present study, to investigate the molecular pathology of SP-A, we established a method for quantitative RT-PCR assay of SP-A1 and SP-A2 mRNA transcripts. Using this method, fatalities from acute mechanical asphyxia (n=12) and drowning (n=9) were examin…

Forensic issues in cases of water birth fatalities.

Birth under water has become a widely disseminated technique that is promoted to improve the quality of labor. The case of a 42-week gestation male infant is reported who died of respiratory and multiorgan failure secondary to florid pneumonia and sepsis due Pseudomonas aeruginosa following a water birth. Other infants who have been delivered underwater have drowned or have had near-drowning episodes with significant hyponatremia and water intoxication. Local and disseminated sepsis has been reported, with respiratory distress, fevers, hypoxic brain damage, and seizures. There have also been e…

Case-specific characteristics of pink teeth in dental autopsies - A systematic review.

The cadaveric alterations that derive from the decomposition of the human body are often investigated and examined in medical autopsies together with any other evidence of thanatological interest. This study aimed to systematically review case-specific characteristics of dental autopsies that reported the pink tooth phenomenon (PTP). The review was performed in October/2018 and followed PRISMA and Cochrane guidelines. Seven databases were searched as primary study sources (PubMed, Scopus, LILACS, SciELO, Web of Science, Science Direct and Embase) and three (OATD, Open Grey and Open Thesis) wer

Accidental Hypothermia in a Swiss Alpine Trauma Centre-Not an Alpine Problem.

BACKGROUND: Research in accidental hypothermia focuses on trauma patients, patients exposed to cold environments or patients after drowning but rarely on hypothermia in combination with intoxications or on medical or neurological issues. The aim of this retrospective single-centre cohort study was to define the aetiologies, severity and relative incidences of accidental hypothermia, methods of measuring temperature and in-hospital mortality. METHODS: The study included patients ≥18 years with a documented body temperature ≤35 °C who were admitted to the emergency department (ED) of the Univer…

A single case of PTP was detected (4.76%) - a 26-year-old male victim of hanging. CONCLUSION: This study illustrates and highlights the unspecific aspect of the PTP. In practice, …

Postmortem serum levels of pulmonary surfactant-associated proteins A and D with regard to the cause of death in medicolegal autopsy.

Pulmonary surfactant-associated proteins A and D (SP-A and -D) are tissue-specific components. Previous studies showed an increase in the po … mortem interval. The left-to-right difference was significantly larger for mechanical asphyxiation, drowning, intoxication and spontaneous cerebral hemorrhage. Both SP-A and -D levels in bilateral cardiac blood were significantly higher for drowning and secondary pulmonary damage involving ARDS after traumas, but were lower for hypothermia (cold exposure). …

An autopsy case of adrenal insufficiency 20 years after hypophysectomy: relation between stress and cause of death.

A 63-years-old man was found dead with the body soaking in water lying face up on a riverbank. Autopsy and diatom examination demonstrated that the cause of death was drowning. He had undergone hypophysectomy 20 years earlier. Autopsy, pathological and endocrinological findings demonstrated secondary and chronic hypothyroidism, hypogonadism, and adrenal insufficiency. The cadaver had fallen into the river, and received numerous wounds such as abrasions and subcutaneous hemorrhage. Moreover, it was suspected that he had developed hypothermia before death. Cortisol in the blood and 17- OHCS in u…

The impact of trends in psychotropic prescribing on the method of suicide in the elderly.

Suicide rates in the elderly have declined in many countries in recent years. Monitoring and control of intracranial pressure in non-traumatic encephalopathies.

Fifteen patients with non-traumatic encephalopathies underwent continuous monitoring of intracranial pressure. These included 11 children with Reye's syndrome, two with viral encephalitis, one with mushroom poisoning, and one with hypoxia from drowning. Elevated intracranial pressure was treated with decadron, hyperventilation, hyperosmolar agents and in some cases hypothermia and barbiturates. The results are discussed.

Habituation of the cold shock response: A systematic review and meta-analysis.

Cold water immersion (CWI) evokes the life-threatening reflex cold shock response (CSR), inducing hyperventilation, increasing cardiac arrhythmias, and increasing drowning risk by impairing safety behaviour. Repeated CWI induces CSR habituation (i.e., diminishing response with same stimulus magnitude) after ∼4 immersions, with variation between studies. We quantified the magnitude and coefficient of variation (CoV) in the CSR in a systematic review and meta-analysis with search terms entered to Medline, SportDiscus, PsychINFO, Pubmed, and Cochrane Central Register. Random effects meta-analyses…

Seizure-related injuries in northeast Thailand.

OBJECTIVE: The authors aimed to assess the frequency, characteristics, and risk of seizure-related injuries from traffic accidents, near-drowning, burns, fractures, head and soft tissue injuries. MATERIAL AND METHOD: Attending physicians assisted 300 consecutive seizure patients to complete a questionnaire. The types and frequency of injuries were then expressed in terms of the type of seizure and related activity. RESULTS: The male (154) and female (146) patients were between 13 and 91 years of age (mean, 36.27 +/- 14.55). The seizure types comprised of Generalized Tonic-Clonic (GTC) (26%),…

Survival of immersions during recreational boating events in Alaska, 1999-2004.

OBJECTIVE: This retrospective study analyzed records of boating or other watercraft-related events in Alaska from 1999-2004, where at least one drowning occurred, to identify factors associated with survivors of these same events. DATA: Records of recreational boating events involving at least one fatality were obtained through the U.S. Coast Guard. Group rescue responses and rescue assistance responses by others outside the parties were categorized and analyzed. We conducted t tests and odds ratios to analyze inter-group differences. RESULTS: There were 32 immersion events involving 36 fata…

AIM: To review the outcome of childhood submersion injury (SI). METHODS: We reviewed discharge data of all children with SI who were hospitalized in a university teaching hospital between January 2002 and January 2008. RESULTS: There were 15 admissions (8 males and 7 females). Outdoor SI (n = 10) were more common than indoor SI (n = 5) and 7 cases occurred in public swimming pools with life guard service. There were significant differences between the two types of SI. Indoor SI more likely occurred in the Chinese mainland. The victims were generally younger, more likely to have low Glasgow C
