Two siblings with extrinsic duodenal obstruction caused by congenital peritoneal bands are reported. Attention is drawn to the unusual physical sign of halitosis as a presenting feature. It is suggested that this physical sign may be an indication for barium studies.

Thirty outpatients with erosions of the duodenal bulb unrelated to peptic ulcer were randomly allocated to an 8-week treatment regimen with either rioprostil, 200 micrograms b.d., or with sucralfate, 1 g t.i.d. The results of 250 consecutive ERCP examinations were analysed in order to assess whether or not juxtapapillary duodenal diverticula are associated with choledocholithiasis. Cholangiography showed common bile duct stones in 71 patients of whom 25 (35%) had periampullary diverticula. Clear bile ducts were shown in 99, of whom only 12 had diverticula (12%) (p less than 0.05). The work generalized an experience with the treatment of 747 patients with diverticula of the duodenum. Operations were performed on 326 of them (43.6%). Adenocarcinoma of the duodenum is an uncommon malignancy that usually presents itself with either obstructive symptoms or jaundice, depending on its location. The diagnosis should be suspected in any patient with a high small bowel obstruction, weight loss, or chronic abdominal pain. Endoscopy and barium contrast x-ray films are the preferred initial studies; enteroclysis may provide additional information. Surgical resection offers the only hope for long-term survival, which should approach 50% with resectable tumors.

The histological aspect of fundic and antral mucosa of patients with endoscopically proven duodenal ulcer was investigated and compared with that observed in asymptomatic controls of similar age. Fundic gastritis was less frequent in ulcer patients than in controls. In contrast the chronic inflammatory lesions of the antral mucosa were more frequent, more severe and appeared at an earlier age in ulcer patients than in controls.

Barium studies of the upper gastrointestinal tract and small bowel were performed in 16 adult patients with biopsy-proven nontropical sprue. Commercially available contrast media containing micropulverized barium sulfate, suspending agents, and various other additives were used. Giant duodenal ulcer is a variant of peptic ulcer that is 2 cm in diameter or greater and essentially replaces the duodenal bulb. Diagnosis by upper gastrointestinal series is often missed, due to the large size of the ulcer, which causes it to look like a scarred duodenal bulb or duodenal diverticulum. This study reviews our experience with 32 patients who presented with giant duodenal ulcer between 1963 and 1982. Duodenal ulcer is being observed with increasing frequency in the Black population of South Africa, this increase occurring pari passu with industrialization. During the past 15 years, 21 patients with duodenal injuries were treated at the University of Iowa Hospitals and Clinics. The major cause of injury (66.6%) was blunt abdominal trauma. Eighteen patients (85.7%) underwent celiotomy. Only one patient (4.7%) had an isolated duodenal injury.
