[Acoustic neurinoma with preserved inner ear function. Interesting audiologic constellation of findings].

Neurofibromatosis (NF) is a syndrome with multiple neuromas. With the central form or NF2 very often bilateral acoustic neuromas are found. Tumours derive from Schwann cells of the eighth cranial nerve; symptoms are tinnitus, hearing loss and vertigo. Therapy is surgical, if possible with the protection of remaining hearing function. We report the case of a 28 year old female patient with neurofibromatosis type 2. After complete resection of the left AN and partial resection of the right AN the patient suffered from bilateral complete deafness, although almost normal DPOAE can be recorded on o

Cogan's syndrome: clinical significance of antibodies against the inner ear and cornea.

The aim of this study was to evaluate the pathological significance of antibodies against cornea and inner ear tissue in the development of audiovestibular and ocular symptoms in patients with Cogan's syndrome (CS). We analysed the serum of 5 CS patients for binding of IgM and IgG to fresh cryosections of rat labyrinth (semicircular canals, ampulla, utricle, saccule) and cornea by indirect immunofluorescence (IF). The predominant pattern of anti-corneal IgM was staining of the superficial cell layer of the non-keratinizing squamous epithelium. IgM against cornea was found in 3 patients, all of

Three-dimensional MR of the inner ear with steady-state free precession.

PURPOSE: To describe the steady-state free-precession MR sequence and its application to the study of the inner ear. METHOD: The inner ear was imaged with CT and a 0.5-T MR unit in three dimension, to evaluate the various signals from the lumen of the labyrinth. RESULTS: Normally, the signal from the perilymphatic and endolymphatic spaces is homogeneous. However, among our cases of neurosensory deafness, differences of signal and morphology were seen in patients with otosclerosis, ossifying labyrinthitis, and inner ear malformations. CONCLUSION: Three-dimensional MR, used together with rout

[Drug therapy of acute inner ear hearing loss in childhood and adolescence].

Sensorineural hearing loss in early childhood may severely interfere with normal speech and language development. This by itself requires pursuit of an effective therapeutic procedure. Here we report the results of therapy with a rheological agent and procaine-HCl with or without the addition of steroids. Immunological testing was also studied for possible circulating antibodies in sera of affected children. Retrospective analysis of therapeutic results showed that the therapeutic procedure employed may be beneficial, since hearing improved considerably in some cases. Since an autoimmune-relat

[Human type II collagen induced autoimmune inner ear lesions in guinea pigs].

Guinea pigs were immunized with human type I, type II or type III collagen. Inner ear lesions were induced in some guinea pigs by immunizing them with human type II collagen. Histopathologic changes included endolymphatic hydrops, organ of Corti lesions and endolymphatic duct contained a large number of inflammatory cells. These histologic changes resembled those observed in patient with Meniere's disease. We didn't discover any inner ear lesions in guinea pigs by immunizing them with human type I and type III collagen, although their serum antibody titers were high and antibody presented in p

Quantitative assessment of inner ear pathology following ototoxic drugs or acoustic trauma.

Techniques currently used in the assessment of structural and/or functional damage to the peripheral auditory system are summarized. Two histological approaches are described: one which allows light microscopic evaluation of all structures of the auditory periphery, and a second which concentrates on the sensory cells and their innervation. The latter technique allows electron microscopic analysis of selected regions after a thorough light microscopic survey. Two electrophysiological methods are described as well: a single-fiber approach which provides detailed information about cochlear condi

Frequency selectivity and temporal resolution in patients with various inner ear disorders.

Both frequency selectivity and temporal resolution were examined in patients with various inner ear disorders. These included noise-induced hearing loss (n = 24), Menière's disease (n = 16), sudden deafness (n = 25), toxic inner ear damage (n = 14), presbyacusis (n = 38) and degenerative progressive inner ear hearing loss (n = 8). To facilitate quantitative comparison, various factors were introduced, namely, frequency resolution factor (FRF), temporal resolution factor (TRF) and a combined resolution factor (FTRF). The FRF of normal-hearing subjects in background noise conditions was found to

Congenital malformations of the inner ear: histologic findings in five temporal bones.

The histopathologic findings in five temporal bones from three patients with congenital malformation of the inner ear are described. The external contour of the cochlea was deformed in two temporal bones, while the internal architecture was abnormal in all five temporal bones. Intracochlear abnormalities included defects in the interscalar septum, deficiencies in the modiolus, and a paucity of neural elements. Spiral ganglion cell populations varied, but were substantially diminished in all five temporal bones. Enlargement of the vestibule and semicircular canals (lateral and posterior) was se

[Evaluation of risk factors in various forms of inner ear hearing loss].

As in cardio-vascular diseases certain risk factors are held responsible for the genesis of inner ear affections. We thoroughly examined 205 patients, suffering from various kinds of internal ear disorders. Examinations included audiometric tests, medical, neurological and ophthalomological consultations and laboratory findings. Evaluation showed a significant increase of certain risk factors in our patients compared to the normal population. 50.2% of our patients were overweight, 29.3% had a high blood cholesterol, 28.8% had anaemia, and 22.4% had hypertension. These lesions of the auditory s

Predictive value of laboratory tests in "autoimmune" inner ear disease: preliminary report.

The purpose of this prospective, controlled study was to estimate the prevalence of immune-mediated (autoimmune) inner ear disease in a high-risk patient population, in order to determine the predictive value of a positive lymphocyte transformation test. The high-risk group was defined as any dizzy patient with unilateral or bilateral-asymmetric sensorineural hearing loss, who had not previously received immunotherapy. From more than 400 consecutive patients with a chief complaint of dizziness, 58 were entered into the study over an 8-month period. The control group consisted of 15 normal volu

Unidirectional inner ear valve implant for endolymphatic sac surgery in Meniere's disease.

In five patients with long-standing Meniere's disease, a unidirectional inner ear valve was implanted into the endolymphatic sac (ELS) to facilitate removal of excess endolymph in the hydropic state. These patients were all candidates for ablative surgery. They all had intractable vertigo and severe hearing losses. Four out of five patients had positive preoperative glycerol tests. In the patients with positive glycerol tests, the group averages before surgery were as follows: pure-tone average (PTA), 63 dB; speech reception threshold (SRT), 79 dB; and discrimination score, 27%. All of the pat

In vivo imaging of middle-ear and inner-ear microstructures of a mouse guided by SD-OCT combined with a surgical microscope.

We developed an augmented-reality system that combines optical coherence tomography (OCT) with a surgical microscope. By sharing the common optical path in the microscope and OCT, we could simultaneously acquire OCT and microscope views. The system was tested to identify the middle-ear and inner-ear microstructures of a mouse. Considering the probability of clinical application including otorhinolaryngology, diseases such as middle-ear effusion were visualized using in vivo mouse and OCT images simultaneously acquired through the eyepiece of the surgical microscope during surgical manipulation

Advances in biomarkers for differential diagnosis of autoimmune and autoinflammatory inner ear disorders.

Autoimmune inner ear disease (AIED) represents a rare yet potentially reversible cause of sensorineural hearing loss, characterized by immune-mediated damage to cochlear and vestibular structures. Unlike autoimmunity, which is driven by adaptive immune responses and autoantibodies, autoinflammation stems from innate immune system dysregulation, typically involving inflammasome hyperactivation. This review synthesizes recent advancements in biomarkers for the differential diagnosis of AIED, emphasizing their role in distinguishing it from conditions such as Meniere's disease (MD) and chronic ot

[Progress of tympanic administration with its application in the treatment of inner ear disease].

As an effective means of local drug delivery, intratympanic administration has shown a wide range of applications in the treatment of inner ear diseases, such as Meniere's disease, sudden deafness, and immune-related inner ear disease. These diseases have complex etiologies and limited treatment effects, leading to many difficulties in clinical diagnosis and treatment. Intratympanic administration significantly enhances the therapeutic effect through bypassing the blood-labyrinth barrier and increasing the local concentration of the drug in the inner ear. This article reviews the historical de

Timing of Delamination of Inner Ear Neurons Specifies Their Topography and Target Innervation.

The neurons of the inner ear delaminate from a sensorineurogenic epithelium in the ventral part of the otocyst. Delaminated neuroblasts then condense to form the acoustico-vestibular ganglion (AVG). As they differentiate, the neurons connect mechanosensory hair cells (HCs) of the inner ear with their targets in the hindbrain in a precise topographical manner. However, it is unclear how or when positional identities of neurons are specified within the AVG, such that topographical information from HCs is maintained into the auditory centers of the brain. Here, we find that the time of delaminati

Role of Pcdh15 in the development of intrinsic polarity of inner ear hair cells.

In vertebrates, auditory information is transduced in the cochlea by mechanosensory hair cells (HC) through an eccentrically organised structure known as the hair bundle. This consists of a true cilium, known as the kinocilium, and modified microvilli, known as stereocilia. The hair bundle has a distinct structure with stereocilia organised in graded rows, with the longest abutting the kinocilium. The hair bundles of all HC are aligned to the tissue axis and are planar polarised. Important in the development and physiology of HC are protein bridges consisting of cadherin-23 (CDH23) and protoca
