High-fidelity tissue engineering of patient-specific auricles for reconstruction of pediatric microtia and other auricular deformities.

INTRODUCTION: Autologous techniques for the reconstruction of pediatric microtia often result in suboptimal aesthetic outcomes and morbidity at the costal cartilage donor site. We therefore sought to combine digital photogrammetry with CAD/CAM techniques to develop collagen type I hydrogel scaffolds and their respective molds that would precisely mimic the normal anatomy of the patient-specific external ear as well as recapitulate the complex biomechanical properties of native auricular elastic cartilage while avoiding the morbidity of traditional autologous reconstructions. METHODS: Three-di

Avoiding complications of microtia and otoplasty.

Congenital ear surgery constitutes one of the most challenging reconstructive procedures in plastic surgery secondary to the complex architecture of the auricle and the multiple factors involved in the reconstructive techniques. The most common complications encountered during microtia repair and otoplasty are described. Emphasis is placed on preventing these complications.

Relapsing polychondritis: an ultrastructural study.

Ear cartilage from a typical case of relapsing polychondritis was examined with the electron microscope. A large number of dense granules and vesicles, which were compatible with matrix vesicles or lysosomes, surrounded the affected chondrocytes. In less severely damaged chondrocytes, these granules and vesicles appeared to be formed by pinching off of the cytoplasmic processes or by budding from the processes. Calcification of the granules was minimal. In severely damaged chondrocytes, an admixture of these granules and cytoplasmic organelles occurred. It is speculated that many of these dens…

Perichondritis: Not All Ear Pain Is Otitis.

Acute auricular perichondritis is an infection and inflammatory disease of the external ear that can potentially cause serious complications if not diagnosed and treated promptly. Delays in treatment can lead to devastating focal cartilage necrosis and, subsequently, permanent deformities of the ear. We present the case of a two-year-old boy who was diagnosed with acute perichondritis after presenting to the emergency department (ED) with acute ear redness, swelling, and tenderness. In this article, we will discuss how the diagnosis of perichondritis is made and give a brief literature review

Finite element model of the stapes-inner ear interface.

Since 1958, stapedotomy has been the method of choice for middle ear surgeons who operate on patients suffering from otosclerosis, especially stiffening of the interface between the stapes footplate of the middle ear and the oval window, which is a part of the cochlea of the inner ear. Later, many surgeons started to use the Schuknecht prosthesis, which consists of cartilage and is inserted into the complete opened oval window during stapedectomy. Our study shows that basilar membrane (BM) displacement is increased with an increasing stapes footplate area by a numerical simulation including th

p21-/- Mice Exhibit Spontaneous Articular Cartilage Regeneration Post-Injury.

OBJECTIVE: Recent studies have implicated the cyclin dependent kinase inhibitor, p21, in enhanced tissue regeneration observed in MRL/MpJ "super-healer" mice. Specifically, p21 is downregulated in MRL cells and similar ear hole closure to MRL mice has been observed in p21-/- mice. However, the direct implications of p21 deletion in endogenous articular cartilage regeneration remain unknown. In this study, we investigated the role of p21 deletion in the ability of mice to heal full-thickness cartilage defects (FTCDs). DESIGN: C57BL/6 and p21-/- (Cdkn1atm1Tyj) mice were subjected to FTCD and as

Salvage of the severely injured ear.

We present three cases of severe external ear injuries and their management. The first patient's ear, amputated in an automobile accident, was revascularized microsurgically. Replantation, when possible, offers the ultimate result. However, disadvantages include the length of the operation and prolonged hospitalization. The second patient sustained a human bite ear amputation. Salvage consisted of immediate coverage with a vascularized temporoparietal fascial flap and split-thickness skin graft. Revision of the external auditory meatus and postauricular sulcus was required. This technique is r

The Challenges and Considerations with Autologous Rib Microtia Reconstruction.

Microtia encompasses a spectrum of congenital pinna malformation. Autologous rib reconstruction is a widely utilized surgical reconstruction technique for microtia. The surgery presents various challenges that can impact surgical outcomes and patient satisfaction. Sculpting and shaping the cartilage into a detailed auricular framework poses is a technically difficult surgery due to the complexity of the ear's anatomy. Postoperative complications such as scar formation, infection, and framework resorption may lead to poor aesthetic outcomes like asymmetry of the reconstructed ear and inadequate

Type II collagen distribution in the monkey ear.

A possible role for type II collagen autoimmunity in the pathogenesis of Meniere's disease and otosclerosis was recently suggested by studies demonstrating anti-type II collagen antibody in the serum of humans with these disorders and by the induction of similar lesions in animals immunized with type II collagen. In light of these findings, we studied the anatomic distribution of type II collagen in the nonhuman primate ear by immunohistochemical techniques using defined monoclonal antibodies to type II collagen. Type II collagen was observed in the cartilage plate of the auricle and external

[Chronic atrophic polychondritis. A case report].

Chronic atrophic polychondritis is a rare systemic disease characterized by recurring inflammation of cartilage of the ear, nose, larynx and of the tracheobronchial tree. Nowadays, it is considered to be an autoimmune disease. The case of a 25-year-old female patient hospitalized in our department for treatment of a dyspnea is herein presented. ORL laryngoscopic examination revealed a small sized larynx with a pronounced narrowing of its lumen and very "floppy" cartilage. Bronchoscopic findings were a circumferential stenosis along the entire trachea and the right major bronchi. Results from b

Negative pressure manoeuvre in microtia reconstruction with autologous rib cartilage.

In microtia reconstruction, maintaining a healthy contact between the skin and the fabricated cartilage framework is essential to attaining a smooth and accentuated contour of the reconstructed auricle. Conventional means to achieve this include bolster sutures and continuous suction drains, both of which have associated shortcomings. A new dressing method was developed and applied in 10 consecutive patients who underwent the first of a two-stage microtia reconstruction using the Nagata technique. A small catheter was introduced into the space between the skin and the cartilage framework. Nega

Immediate reconstruction of the auricle using the amputated cartilage and the temporoparietal fascia.

Successful restoration of the auricle following amputation using microsurgical techniques for reimplantation remains the best reconstructive alternative. In many cases of partial amputation, however, this option may not be technically feasible. Deepithelialization of the amputated part followed by coverage of the ear cartilage using an ipsilateral temporoparietal fascia turn-down flap may allow a single-stage salvage procedure that provides an acceptable final result.

A new method of reconstruction in microtia, with emphasis on conchal creation.

A new method for reconstruction in microtia is described. The auricle is divided into two components--conchal and helical. The conchal component includes the conchal cavity, antihelix, superior and inferior antihelical crura, and tragus, while the helical component incudes the helix and scapha. In a departure from ordinary methods, the conchal part is reconstructed first and the helical part second. Carving and fabricating costal cartilages, usage of soft tissue flaps, and creation of the tragus are important. Especially important in this technique is conchal restoration. The advantages and di
