Guardians of the canopy: two new turtle ant species (Hymenoptera: Formicidae: Cephalotes), from Ecuador's Chocó and Amazonia, revealed by morphology and DNA barcoding.

We describe two new species of arboreal ants of the genus Cephalotes Latreille, 1802, from Ecuador: Cephalotes esthelae Ilguan & Troya, sp. nov. and Cephalotes sacha Yumbay & Troya, sp. nov. The former, a member of the pinelii + grandinosus-group, was collected from lowland Chocó and Amazonian forests, while the latter, belonging to the atratus-group, is an inhabitant of the Amazonian region. Diagnoses and detailed morphological descriptions of workers and soldiers are provided, illustrated by high-resolution images. Species identification was further corroborated with mitochondrial cytochrome…

Uncovering the hidden burden of congenital anomalies in Ecuador: a national integrated analysis of hospital discharges, mortality records, and live birth statistics (2012-2024).

BACKGROUND: Congenital anomalies (CA), ICD-10 Q00-Q99 codes, are major causes of infant morbidity and mortality, but national assessments remain limited in countries without integrated birth defect surveillance. This study characterized hospital burden, mortality, and epidemiological patterns of CA among infants in Ecuador. METHODS: A retrospective descriptive analysis of national administrative records was conducted using data from the INEC. Hospital discharge records covered 2012-2024, while mortality records and live birth statistics were incorporated according to availability. Analyses we…

Political stability, health system resilience, and macroeconomic shocks as factors associated with malaria control outcomes: a comparative analysis of Ecuador and Venezuela (2000-2023).

BACKGROUND: The post-2000 period in South America presents a striking divergence in malaria epidemiology: Ecuador achieved a >99% reduction in malaria incidence approaching elimination, while Venezuela experienced one of the most severe resurgences in modern public health history. We examined the association between governance factors, health policies, intersectoral coordination, and malaria control outcomes by comparing the national experiences of Ecuador and Venezuela. METHODS: We conducted a comparative ecological study using epidemiological data from WHO World Malaria Reports and PAHO upd…

Leveraging the COVID-19 pandemic as a natural experiment to assess changes in antibiotic use and antibiotic-resistant E. coli carriage in semi-rural Ecuador.

The coronavirus 2019 (COVID-19) pandemic has had significant impacts on health systems, population dynamics, public health awareness, and antibiotic stewardship, which could affect antibiotic resistant bacteria (ARB) emergence and transmission. In this study, we aimed to compare knowledge, attitudes, and practices (KAP) of antibiotic use and ARB carriage in Ecuadorian communities before versus after the COVID-19 pandemic began. We leveraged data collected for a repeated measures observational study of third-generation cephalosporin-resistant E. coli (3GCR-EC) carriage among children in semi-ru…

Case Report: Fatal Scorpion Envenomation in a Shuar Child by Tityus cisandinus from Amazonian Ecuador: A Call for Specific Antivenom Availability in the Amazon Basin.

Envenomation by scorpions belonging to the genus Tityus can be life threatening in the Americas, particularly in the Amazon Basin. We report a 4-month-old Ecuadorean boy of Shuar origin stung by a scorpion identified as Tityus cisandinus in the Amazonian province of Morona Santiago, presenting with pulmonary edema and systemic inflammation. We administered immunotherapy using the scorpion antivenom available in Ecuador, of Mexican origin (anti-Centruroides). Catecholamine discharge-related events such as hyperglycemia and thrombocytosis were resolved after treatment but leukocytosis did not, s…

Evaluation of the contextualized sexual and reproductive health educational strategy "Rurankapak": a mixed-methods quasi-experimental study among adolescents and young people in Ecuador.

BACKGROUND: Sexual and reproductive health (SRH) educational interventions in Ecuador show uneven effectiveness, particularly among adolescents and young people in socioculturally diverse settings. Although the "Rurankapak" methodology has been implemented as a participatory SRH education strategy, its standardized application has limited cultural relevance, acceptability, and sustainability. OBJECTIVES: To collaboratively adapt the Rurankapak methodology for SRH education to diverse sociocultural contexts and to preliminarily assess its applicability and acceptability among adolescents and y…

Soil-Transmitted Helminths and the Intricacies of Immunoregulation: Evidence From Amazonian Ecuador for the Importance of Considering Species-Specific Effects Within the Old Friends Hypothesis.

OBJECTIVES: The old friends hypothesis (OFH) examines connections between the global increase in immunoregulatory diseases (e.g., allergy and autoimmunity) and reduced exposure to immune-priming symbionts like soil-transmitted helminths. Helminth species, however, vary in their effects on hosts and should be considered separately. We examined relationships between species-specific helminth infection and circulating biomarkers of adaptive immune antibodies (total immunoglobulin E [IgE]), systemic inflammation (C-reactive protein [CRP]), and immune regulation (interleukin-6 [IL-6]), among Indige

High prevalence and diversity of Leptospira pathogenic serogroups in pigs, cows and free roaming dogs from undeserved rural communities in the coastal region of Ecuador.

Leptospirosis is a zoonotic disease with global distribution but endemic to tropical regions that affects animal production systems. In Ecuador, leptospirosis is endemic with records of human outbreaks dated from more than one century ago, and where the role of animal reservoirs in the transmission dynamics is not well understood. In this study, we carried out a serological surveillance by Microagglutination Test with a panel of 24 serovars for 545 animals represented by pigs and cows, and free roaming dogs from undeserved rural communities in the province of Guayas (Coastal Region of Ecuador)…

Environmental Exposures Associated with Enteropathogen Infection in Six-Month-Old Children Enrolled in the ECoMiD Cohort along a Rural-Urban Gradient in Northern Ecuador†.

Enteropathogens are major contributors to mortality and morbidity, particularly in settings with limited access to water, sanitation, and hygiene infrastructure. To assess transmission pathways associated with enteropathogen infection, we measured household environmental conditions and assayed 22 enteropathogens using TaqMan Array Cards in stool samples from 276 six-month-old children living in communities along a rural-urban gradient in Northern Ecuador. We utilized multivariable models, risk factor importance, and distance-based statistical methods to test factors associated with infection. …

Prevalence and associated risk factors of intestinal parasites among schoolchildren in Ecuador, with emphasis on the molecular diversity of Giardia duodenalis, Blastocystis sp. and Enterocytozoon bieneusi.

BACKGROUND: Intestinal helminths, including Soil-Transmitted Helminth (STH), and Gastrointestinal Protist (GP) infections are major contributors to the global burden of disease, particularly in low-income countries such Ecuador. Their epidemiology in these settings is largely unknown. METHODOLOGY: This prospective cross-sectional study investigates the carriage of intestinal helminths, including STH, and GP in asymptomatic schoolchildren (3-11 years) in the Chimborazo and Guayas provinces, Ecuador. Single stool samples (n = 372) and epidemiological questionnaires on demographics and potential…

Regional variation in Ascaris lumbricoides and Trichuris trichiura infections by age cohort and sex: effects of market integration among the indigenous Shuar of Amazonian Ecuador.

BACKGROUND: Soil-transmitted helminth (STH) infection peaks during childhood and varies by sex. The impact of market integration (MI) (increasing production for and consumption from a market-based economy) on these infection patterns, however, is unclear. In this study, STH infection is examined by sex and age among indigenous Shuar inhabiting two regions of Amazonian Ecuador: (1) the modestly market-integrated Upano Valley (UV) and (2) the more traditional Cross-Cutucú (CC) region. METHODS: Kato-Katz fecal smears were examined for parasite presence and infection intensity. Factorial ANOVAs a…

Reconstructing severe acute respiratory infection dynamics from ICD-10 hospital discharge data: a 10-year analysis of 11.2 million discharges in Ecuador, 2014-2023.

BACKGROUND: In many low- and middle-income countries, surveillance of severe acute respiratory infections (SARI) relies on administrative hospital data without virological confirmation, and its ability to capture epidemic dynamics and age-specific burden remains uncertain. METHODS: We analyzed 11,232,698 hospital discharges recorded by Ecuador's National Institute of Statistics and Censuses between 2014 and 2023, identified SARI episodes using International Classification of Diseases, 10th Revision (ICD-10) codes under broad and length-of-stay-restricted case definitions, and incorporated a n…

Electrochemical detection of Toxocara canis excretory-secretory antigens in children from rural communities in Esmeraldas Province, Ecuador: association between active infection and high eosinophilia.

BACKGROUND: The diagnosis of active Toxocara canis infections in humans is challenging. Larval stages of T. canis do not replicate in human tissues and disease may result from infection with a single T. canis larva. Recently, we developed a nanobody-based electrochemical magnetosensor assay with superior sensitivity to detect T. canis excretory-secretory (TES) antigens. Here, we evaluate the performance of the assay in children from an Ecuadorian birth cohort that followed children to five years of age. METHODS: Samples were selected based on the presence of peripheral blood eosinophilia and …

Community-based social connection intervention programme to improve cardiovascular and brain health in older adults in rural Ecuador: study protocol for a quasi-experimental trial.

INTRODUCTION: Loneliness and social isolation are increasingly recognised as determinants of cardiovascular and brain health, particularly among older adults. Evidence from high-income settings links social disconnection to higher risk of coronary heart disease, stroke, cognitive decline and mortality, yet few interventions have been adapted for rural, resource-limited environments. In rural coastal Ecuador, demographic stability, low migration and strong community engagement provide a unique context to evaluate a culturally grounded intervention. This study aims to determine whether a multi-c…

First Report of Trypanosoma vivax (Duttonella), Babesia bovis and Babesia bigemina DNA in Cattle from the Galapagos Islands, Ecuador, and Its Relationship with Anaplasma marginale.

Bovine trypanosomoses, caused by Trypanosoma vivax, is a disease present in African and South American countries. This haemoflagellate protozoan parasite, as well as Anaplasma marginale and Babesia spp., are microorganisms that have a blood tropism, mainly causing fever and anaemia, which reduces the productive capacity of dairy or meat farms. This study aimed to detect T. vivax and other blood parasites in bovine herds in the Galapagos Islands. A total of 170 blood samples from bovines in 19 farms on Santa Cruz Island (the most populated) were collected and analyzed using different PCR techni

Non-tuberculous mycobacteria and other acid fast bacilli pathogens identification by qPCR and MALDI-ToF MS in tuberculosis-like lesions of slaughtered cattle from Ecuador.

INTRODUCTION: Mycobacterial infections are caused by the Mycobacterium tuberculosis complex (MTBC) but also by non-tuberculous mycobacteria (NTM). While the importance of NTM in opportunistic infections in humans has been gaining attention, surveillance and control programs for cattle health and production remain predominantly focused on bovine tuberculosis (BTB) that it is caused exclusively by MTBC. METHODS: In this study, we conducted a comprehensive inspection of 5,803 cattle carcasses destined for human consumption across 22 abattoirs in five provinces of Ecuador's coastal region, search…
