Hemofiltration (HF) is an established method for treating patients with impaired renal function and excessive fluid retention. In cardiac surgery fluid accumulation during extracorporeal circulation (ECC) may contribute to postoperative organ dysfunction, leading to severe pulmonary insufficiency. In this study our experiences with hemofiltration installed in the cardiopulmonary bypass are demonstrated in patients with preoperative pulmonary edema, impaired renal function, and long-term ECC, proving its efficacy by measurement of extravascular lung water (EVLW) in the early post-bypass period.

Ethchlorvynol injection in humans leads to a clinical picture consistent with increased permeability pulmonary edema, ie, the adult respiratory distress syndrome. There has been only one such case reported in which the pulmonary wedge pressure was measured. Within the past decade, cardiovascular magnetic resonance (CMR) imaging has led to unprecedented growth in our understanding of myocarditis. From what began as a diagnostic tool for assessing ventricular function, CMR has transitioned into visualizing changes that occur in myocardial tissue during inflammation, including edema, hyperemia/inflammation, and fibrosis. Pulmonary microemboli may play a role in the adult respiratory distress syndrome, creating both pulmonary oedema and hypoxaemia. We measured the time course of pulmonary oedema and hypoxaemia after pulmonary microemboli of 63-74 mu starch were infused into dogs. Dogs were divided into two groups: six dogs that did not develop pulmonary oedema, and seven that did. Immediately after emboli there was no difference between groups in the large fall in PaO2 or the rise in Qs/Qt. Therefore the hypoxaemia of pulmonary embolism is not created by pulmonary oedema. We describe a case of a 79-year-old woman who developed pain and pitting edema of the upper right extremity due to total venous obstruction following the implantation of an atrioventricular sequential pacemaker. She was treated by arm elevation and anticoagulation after which the pain and edema subsided. To our knowledge, this is the first reported case of this complication occurring following implantation of a dual-chamber pacing system. We recommend that venographic studies be done after implantation if apparent arm swelling occurs, and that anticoagulant therapy be instituted if thrombosis

The presence of human coronavirus HKU1 infection associated with pericardial inflammation is not reported. We are reporting a young woman with systemic lupus erythematosus, who was positive for HKU1 during her pericarditis flare. Diagnostic imaging demonstrated pericardial effusion, edema, and late gadolinium enhancement on cardiac magnetic resonance imaging and echocardiography. She was on multiple anti-inflammatory medications and achieved remission with anakinra. Her management and a brief literature review is also presented.

A 33-year-old obese gypsy woman was admitted in labor with signs of preeclampsia followed by sudden pulmonary edema and cardiac arrest. Cesarean delivery was performed during resuscitation maneuvers, but both mother and fetus died. The diagnosis of peripartum cardiomyopathy was confirmed by pathology. The association of latent peripartum cardiomyopathy and episodes of hypertension greatly increases the risk of maternal and fetal death. Careful management of anesthesia and rigorous measures are necessary with a view to possible litigation.

Noncardiogenic pulmonary edema induced by salicylate toxicity is becoming a well-recognized entity. However, the diagnosis can be easily missed early in the disease course. We report here adult respiratory distress syndrome (ARDS) in a 54-year-old man found to have salicylate toxicity one day after admission to the alcohol detoxification service at a community hospital.
