Capsule invasion exhibits tumor size-dependent prognostic impact in differentiated thyroid carcinoma.

BACKGROUND: The prognostic significance of capsular invasion and its interaction with tumor size in differentiated thyroid carcinoma remains unclear. METHODS: We analyzed 86,902 surgically treated differentiated thyroid carcinoma patients from the Surveillance, Epidemiology, and End Results (SEER) database (2000-2015). Disease-specific survival was assessed using Kaplan-Meier analysis and multivariate Cox models. Restricted cubic spline analysis examined capsular invasion effects across the continuous tumor size spectrum. Interaction effects were evaluated on multiplicative and additive scale

Vascular factors predict rate of progression in Alzheimer disease.

BACKGROUND: While there is considerable epidemiologic evidence that cardiovascular risk factors increase risk of incident Alzheimer disease (AD), few studies have examined their effect on progression after an established AD diagnosis. OBJECTIVE: To examine the effect of vascular factors, and potential age modification, on rate of progression in a longitudinal study of incident dementia. METHODS: A total of 135 individuals with incident AD, identified in a population-based sample of elderly persons in Cache County, UT, were followed with in-home visits for a mean of 3.0 years (range: 0.8 to 9

Genetic predictors of acute toxicities related to radiation therapy following lumpectomy for breast cancer: a case-series study.

INTRODUCTION: The cytotoxic effects of radiation therapy are mediated primarily through increased formation of hydroxyl radicals and reactive oxygen species, which can damage cells, proteins and DNA; the glutathione S-transferases (GSTs) function to protect against oxidative stress. We hypothesized that polymorphisms encoding reduced or absent activity in the GSTs might result in greater risk for radiation-associated toxicity. METHODS: Women receiving therapy in radiation units in Germany following lumpectomy for breast cancer (1998-2001) provided a blood sample and completed an epidemiologic

Two novel mutations, Q1053H and C1060R, located in the D3 domain of von Willebrand factor, are responsible for decreased FVIII-binding capacity.

INTRODUCTION: Congenital anomalies (CAs) represent one of the main cause of foetal death, infant mortality and morbidity, and long-term disability. CAs have been object of systematic registration activity for a long-time in many geographical areas in Europe and worldwide. CAs are often associated with disabilities of different types and severity, including the developed Countries worldwide. According to the World Health Organization (WHO), each year approximately 3,2 million of children worldwide are born with a CA and approximately 300,000 newborns with a diagnosis of birth defect die within

The neurobiology of brain and cognitive reserve: mental and physical activity as modulators of brain disorders.

The concept of 'cognitive reserve', and a broader theory of 'brain reserve', were originally proposed to help explain epidemiological data indicating that individuals who engaged in higher levels of mental and physical activity via education, occupation and recreation, were at lower risk of developing Alzheimer's disease and other forms of dementia. Subsequently, behavioral, cellular and molecular studies in animals (predominantly mice and rats) have revealed dramatic effects of environmental enrichment, which involves enhanced levels of sensory, cognitive and motor stimulation via housing in

Difficulty of mode of action determination for trichloroethylene: An example of complex interactions of metabolites and other chemical exposures.

The mode(s) of action (MOA) of a pollutant for adverse health effects may be dependent on the mixture of metabolites resulting from exposure to a single agent and may also be affected by coexposure to pollutants that have similar targets or affected pathways. Trichloroethylene (TCE) can be an useful example for illustration of the complexity coexposure can present to elucidation of the MOA of an agent. TCE exposure has been associated with increased risk of liver and kidney cancer in both laboratory animal and epidemiologic studies. There are a number of TCE metabolites that could play a role

Changes in insulin sensitivity, secretion and glucose effectiveness during menstrual cycle.

BACKGROUND: Several clinical conditions suggest an effect of sex steroids on glucose homeostasis in women. Studies examining this phenomenon have yielded controversial results. METHODS: To investigate the effect of the menstrual cycle on insulin sensitivity, glucose effectiveness and acute insulin response to glucose using the tolbutamide-modified intravenous glucose tolerance test (IVGTT) during the follicular (day 8 +/- 1) and luteal (day 23 +/- 1) phases of the menstrual cycle, the authors recruited 12 healthy regularly menstruating women. All had fasting glucose concentration of < 100 mg/

Viral infection states, immune control, and adult cognition: evidence across latency, persistence, and post-acute responses.

Viral infections can leave latent genomes, persistent reservoirs, or prolonged host responses after the presenting illness has resolved. This Review organizes their relevance to adult cognition around three overlapping infection-state prototypes: latency with episodic reactivation, chronic persistence, and acute infection with post-acute host responses. We examine how local antiviral surveillance, antigen-driven T-cell dysfunction, innate immune memory, and inflammatory resolution shape neurovascular and glial responses. Complement-mediated synaptic remodeling, proteostatic disruption, and imm

Mumps Vaccination Policies and Epidemiological Consequences: A Comparative Review of the United States and Japan.

Mumps remains a public health challenge worldwide despite being a vaccine-preventable disease. This review outlines the historical development and contrasting mumps vaccination policies in the United States and Japan and their epidemiological consequences. In the United States, where the two-dose measles, mumps, and rubella (MMR) vaccine is routinely administered, the incidence of mumps has markedly decreased. Recently, however, sporadic outbreaks have emerged, driven by breakthrough infections among vaccinated individuals. These resurgences are driven largely by waning immunity, coupled with

Human Myopia: Past, Present and Future.

The study of human myopia spans over two millennia, yet many of the principles shaping current research and management trace their origins to the scientific advances of the late nineteenth century. This review takes a broad historical perspective on the evolution of myopia research-from early philosophical and optical observations, through the emergence of modern ophthalmology, to the molecular and biomechanical models of the present era. Many foundational ideas, including the links between education, illumination and myopia; the concept of emmetropisation and the therapeutic use of atropine,

The Impact of Aging on Organ Systems in Sickle Cell Disease: a Comparative Review of Physiological Adaptation and Dysfunction.

Sickle cell anemia (SCA) is a progressive, systemic disorder that can lead to multi-organ dysfunction. While it has traditionally been most prevalent in regions where malaria is endemic, recent epidemiological studies have shown an increasing disease prevalence in non-endemic areas, primarily attributed to global human migration patterns. The severity of SCA typically worsens with age. In early childhood, affected individuals may present with renal hyperfiltration, neurocognitive delays, cardiac remodeling, and skeletal fragility. The presence of these early manifestations often predicts the d

Environmental Risk Factors for Parkinson's Disease: A Critical Review and Policy Implications.

The age-standardized prevalence of Parkinson's disease (PD) has increased substantially over the years and is expected to increase further. This emphasizes the need to identify modifiable risk factors of PD, which could form a logical entry point for the prevention of PD. The World Health Organization (WHO) has recommended reducing exposure to specific environmental factors that have been reported to be associated with PD, in particular pesticides, trichloroethylene (TCE), and air pollution. In this review we critically evaluate the epidemiological and biological evidence on the associations o

Integrated genomic, epidemiologic investigation of Candida auris skin colonization in a skilled nursing facility.

Candida auris is a fungal pathogen of high concern due to its ability to cause healthcare-associated infections and outbreaks, its resistance to antimicrobials and disinfectants and its persistence on human skin and in the inanimate environment. To inform surveillance and future mitigation strategies, we defined the extent of skin colonization and explored the microbiome associated with C. auris colonization. We collected swab specimens and clinical data at three times points between January and April 2019 from 57 residents (up to ten body sites each) of a ventilator-capable skilled nursing fa

Apple Supplementation Improves Hemodynamic Parameter and Attenuates Atherosclerosis in High-Fat Diet-Fed Apolipoprotein E-Knockout Mice.

Epidemiological studies describe the association between apple consumption and improved cardiovascular and metabolic dysfunction. Our recent multiparametric screening on cellular model studies has shown that apples exhibit vascular tropism including Granny Smith (GS) variety independently of the storage condition. The present study aimed to evaluate the cardiovascular and metabolic protection of supplementation of GS variety after storage in classic cold (GSCC) and extreme ultra-low oxygen conditions (GSXO) in the apolipoprotein E-deficient 8-week-old mice fed with high fat diet for 14 weeks.

Comparative Proteomic Analysis of Carbonylated Proteins from the Striatum and Cortex of Pesticide-Treated Mice.

Epidemiological studies indicate exposures to the herbicide paraquat (PQ) and fungicide maneb (MB) are associated with increased risk of Parkinson's disease (PD). Oxidative stress appears to be a premier mechanism that underlies damage to the nigrostriatal dopamine system in PD and pesticide exposure. Enhanced oxidative stress leads to lipid peroxidation and production of reactive aldehydes; therefore, we conducted proteomic analyses to identify carbonylated proteins in the striatum and cortex of pesticide-treated mice in order to elucidate possible mechanisms of toxicity. Male C57BL/6J mice w
