Medial Elbow Injuries in the Throwing Athlete.

Medial elbow injuries in the throwing athlete are common and increasing in frequency. They occur due to repetitive supraphysiologic forces acting on the elbow during the overhead throw. Overuse and inadequate rest are salient risk factors for injury. Most athletes improve substantially with rest and nonoperative treatment, although some athletes may require surgical intervention to return to play. Because of advances in conservative and surgical treatments, outcomes after medial elbow injury have improved over time. Currently, most athletes are able to return to a high level of play after ulna…

Doppler sonography for ulnar neuropathy at the elbow.

INTRODUCTION: The aim of this study was to determine the value of Doppler sonography for evaluation of ulnar neuropathy at the elbow (UNE). METHODS: A total of 102 patients with a clinical suspicion of UNE and 50 healthy controls were examined by Doppler and gray-scale sonography. RESULTS: Intraneural vascularization was found in 31 (46.3%) of 67 patients with confirmed UNE and in 3 (6.0%) of 50 healthy controls. Combining gray-scale sonography measurement of nerve size and the presence of intraneural vascularization increased sensitivity by 3%, but decreased the specificity. Patients with i…

Elbow Injuries in the Young Throwing Athlete.

Baseball pitchers and other throwing athletes place their elbows under tremendous stresses, making them vulnerable to a number of unique injuries. Skeletally immature throwers in particular are at a greater risk for growth plate and other bony injuries, due to the relative strengths of these tissues and the kinematics involved in throwing. Care should be taken to fully evaluate these injuries based on the particular history and presentation to properly direct treatment and rehabilitation. Patients, as well as other athletes, coaches, and families, also should be made aware of the significant r…

Recovery features in ulnar neuropathy at the elbow.

[Purpose] This study evaluated the effect of age, sex, and entrapment localization on recovery time in patients treated conservatively for ulnar neuropathy at the elbow. [Subjects] Thirty-five patients (16 women and 15 men) who were diagnosed with ulnar neuropathy at the elbow using short segment conduction studies were evaluated retrospectively. [Methods] Definition of recovey was made based on patient satisfaction. The absence of symptoms was considered as the marker of recovery. Patients who recovered within 0-4 weeks were in Group 1, and patients who recovered within 4 weeks to 6 months we…

What causes ulnar neuropathy at the elbow?

OBJECTIVE: To determine what causes ulnar neuropathy at the elbow (UNE) by analyzing patients' clinical, electrodiagnostic (EDx) and ultrasonographic (US) findings. METHODS: In prospectively recruited patients with definite UNE, four blinded examiners took a history and performed neurologic, EDx and US examinations. A multivariate logistic regression model was used to investigate the association between UNE location and patient variables. RESULTS: We included 117 patients; 73% with lesions in the retroepicondylar (RTC) groove and 27% under the humeroulnar aponeurotic arcade (HUA). In our mul…

Management of Elbow Trauma: Anatomy and Exposures.

The elbow is a complex joint from the anatomic and biomechanical point of view. A thorough understanding of elbow anatomy greatly aids the surgeon in dealing with elbow trauma. The close vicinity of neurovascular structures should always be kept in mind when selecting a surgical approach. Any approach to the elbow needs to be safe and versatile. Knowledge of the attachment and orientation of elbow ligaments reduces the risk of inadvertent intraoperative injury and/or postoperative instability. Surgeons dealing with elbow trauma should have a good working understanding of elbow anatomy and the …

Magnetic Resonance Arthrography of the Wrist and Elbow.

Magnetic resonance (MR) arthrography of the wrist and elbow is useful for detecting a variety of intra-articular pathologies. MR dictations should address whether intrinsic ligament tears of the wrist are partial-thickness or full-thickness, and involve the dorsal, membranous, and/or volar components of the ligaments. With regard to elbow soft tissue pathology, partial-thickness tears of the anterior band of the ulnar collateral ligament in overhead-throwing athletes are well evaluated with MR arthrography. MR arthrography also is helpful in staging osteochondritis dissecans of the capitellum,…

Giant benign Chondroid Syringoma in the elbow.

Chondroid syringoma (CS) is a rarely seen benign tumor rooted in the epithelial and mesenchymal cells of the skin. It is generally localized in the head and neck region and rarely in the extremities. A case is presented here of a giant CS in the rarely seen location of the elbow. A 55-year old male presented at our hospital with the complaint of a slow-growing painless mass in the left elbow. On the left elbow radiograph, mass opacity was observed with a regular border surrounded by a fine radiolucent line within the subcutaneous tissue adjacent to the humerus in the posterior of the elbow. On…

Nerve injuries about the elbow in the athlete.

The athlete's elbow is a remarkable example of motion, strength, and durability. The stress placed on the elbow during sport, including the throwing motion, may lead to soft-tissue ligamentous and nerve injury. The thrower's elbow illustrates one example of possible nerve injury about the elbow in sport, related to chronic repetitive tensile and compressive stresses to the ulnar nerve associated with elbow flexion and valgus position. Besides the throwing athlete, nerve injury from high-energy direct-impact forces may also damage nerves around the elbow in contact sports. Detailed history and …

The management of acute and chronic elbow instability.

Elbow dislocations are common injuries in both the adult and pediatric populations. These injuries include simple dislocation with no associated fracture and more complex injuries with bony and ligamentous involvement. Simple dislocations are generally stable after reduction and managed with early mobilization. Complex dislocations are very unstable and operative intervention is usually necessary. Complication risks are greater and outcomes are less optimal with complex dislocations. A thorough knowledge of anatomy and understanding of the osseous and soft tissue mechanics is essential for pro…

[Arthroscopy of the elbow: diagnostic and therapeutic approaches].

The elbow is one of the most complex joints of the human body. Bony, ligamentous and muscular constraints ensure elbow stability. During recent years elbow arthroscopy has become more and more popular resulting from technical and surgical innovations. The diagnostic and therapeutic elbow arthroscopy following traumatic elbow dislocation is the best example. Functional outcomes after elbow dislocation significantly depend on sufficient evaluation of elbow stability, possible accompanying soft tissue injuries and on the initiation of adequate therapy. Elbow arthroscopy after traumatic elbow disl…

Posterior elbow problems in the overhead athlete.

The overhead athlete is defined as an athlete who uses his/her hand in an overhead position. The motion in the overhead athlete is a highly skilled movement performed at extremely high velocity, which requires flexibility, muscular strength, coordination, synchronicity, and neuromuscular control of the whole body. In this overview, we will discuss the most common pathologies seen at the posterior side of the elbow such as posteromedial impingement, proximal ulnar stress fractures, and triceps pathology. Specific nonsurgical and operative treatment guidelines for the overhead athlete's elbow ar…

Reconstruction of traumatic losses of substance at the elbow.

Traumatic lesions at the elbow involving great loss of substance are uncommon, but represent a significant problem when such cases are referred to a trauma department. Most of these injuries may cause severe final functional impairment, thereby jeopardising future activities, particularly in cases where treatment was delayed or inappropriate. The timing and method of treatment are critical. The trauma may involve soft tissues only, or bone and joint, or several structures at the same time, which results in combined complex tissue defects. Each type of tissue loss should be managed by choosing …
