Electrical Stimulation of the Nucleus Accumbens for Severe, Refractory Self-Injurious Behaviour in Children (EASE-SIB): protocol for a randomised double-blinded crossover trial.

INTRODUCTION: Self-injurious behaviour (SIB) consists of persistent, repetitive movements that can result in serious injury without suicidal intent. These behaviours are prevalent among children with neurodevelopmental disorders, including profound autism. Although many individuals benefit from currently available therapies, some exhibit treatment-refractory SIB that necessitates ongoing use of personal protective equipment and restraint, presumably due to stronger neurobiological drivers. We recently completed a phase I, open-label clinical trial demonstrating the safety, feasibility and prel

Transcutaneous electrical nerve stimulation (TENS) for ischemia-reperfusion injury and secondary prevention post-PCI in STEMI: study protocol for a randomized controlled trial.

BACKGROUND: Effective strategies for managing ischemia-reperfusion injury in acute myocardial infarction remain limited, despite advances in reperfusion therapy. Transcutaneous electrical nerve stimulation (TENS), a safe and non-invasive intervention with good patient compliance, modulates pain perception and autonomic activity via specific neural pathways. These properties make it a promising candidate for integration into secondary prevention following percutaneous coronary intervention (PCI). Therefore, this randomized controlled trial investigates the potential of TENS as an innovative the

Trojan Horse Strategy for Wireless Electrical Stimulation-Induced Zn2+ Release to Regulate Neural Stem Cell Differentiation for Spinal Cord Injury Repair.

Due to the uncertain differentiation of neural stem cells (NSCs), replenishing lost neurons by endogenous neural differentiation to repair spinal cord injury (SCI) remains challenging. The electrical stimulation-induced drug release is a promising approach for the localized and controlled release of drugs to regulate the differentiation of NSCs into neurons. Here, we developed Zn-PDA@BT nanoparticles acted as Trojan Horse to enter cells through endocytosis for Zn2+-controlled release therapy by the potentials generated by the piezoelectric effect. Due to the presence of polydopamine (PDA), und

Toward rebalancing blood pressure instability after spinal cord injury with spinal cord electrical stimulation: A mini review and critique of the evolving literature.

High-level spinal cord injury commonly leads to blood pressure instability. This manifests clinically as orthostatic hypotension (OH), where blood pressure can drop to the point of loss of consciousness, and autonomic dysreflexia (AD), where systolic blood pressure can climb to over 300 mmHg in response to an unperceived noxious stimulus. These blood pressure fluctuations can occur multiple times a day, contributing to increased vessel shear stress and heightened risk of cardiovascular disease. The pathophysiology of both of these conditions is rooted in impairments in regulation of spinal cor

Clinical treatment of orthostatic hypotension after spinal cord injury with training based on electric uprise bed coupled with remote ECG and BP monitor.

BACKGROUND: The treatment for orthostatic hypotension (OH) after spinal cord injury (SCI) is an important part of rehabilitation in late-stage SCI. Electric uprise bed training is a relatively commonly used method in treating OH, and how to carry out uprise bed training safely and effectively is an urgent problem. In the early stage of SCI, we used a remote monitoring system to monitor the whole process of uprise bed training, and we explored a safe and efficient method of electric uprise bed training. MATERIAL AND METHODS: The experimental group consisted of 36 patients diagnosed with orthos

Acute effects of neuromuscular electrical stimulation on lactate, IGF-1 and cognition in chronic spinal cord injury: A pilot randomized cross-over study.

INTRODUCTION: Individuals with spinal cord injuries (SCI) exhibit an accelerated age-related cognitive decline compared to healthy individuals, even after adjusting for mood factors and concomitant traumatic brain injury. We hypothesized that neuromuscular electrical stimulation (NMES) on hamstring and gluteal muscles may induce a dose-dependent increase in lactate and insulin-like growth factor-1 (IGF-1) which is hypothesized to be associated with a temporary enhancement of cognitive performance. METHODS: Twenty-two individuals with chronic SCI participated in a randomized cross-over study,
