Effect of electrical stimulation of the LES on LES pressure in a canine model.

Gastric electrical stimulation modulates lower esophageal sphincter pressure (LESP). High-frequency neural stimulation (NES) can induce gut smooth muscle contractions. To determine whether lower esophageal sphincter (LES) electrical stimulation (ES) can affect LESP, bipolar electrodes were implanted in the LES of four dogs. Esophageal manometry during sham or ES was performed randomly on separate days. Four stimuli were used: 1) low-frequency: 350-ms pulses at 6 cycles/min; 2) high-frequency-1: 1-ms pulses at 50 Hz; 3) high-frequency-2: 1-ms pulses at 20 Hz; and 4) NES: 20-ms bipolar pulses at

Feasibility of gastric electrical stimulation by use of endoscopically placed electrodes.

BACKGROUND: Gastric electrical stimulation (GES), which has been reported to have therapeutic potentials for gastroparesis and obesity, involves the surgical placement of electrodes with the patient under general anesthesia. New methods are needed for implanting GES electrodes in a safer and more feasible way. OBJECTIVE: Our purpose was to investigate the safety and feasibility of placing electrodes endoscopically for GES. DESIGN AND SETTING: A pilot study. SUBJECTS: Six female hound dogs that weighed 13 to 22 kg. INTERVENTIONS: Endoscopically placed electrodes passed through the abdomen a

Electrical stimulation of the epidural space using a catheter with a removable stylet.

BACKGROUND AND OBJECTIVES: Electrical stimulation can be used to verify the location of epidural catheters. With the traditional technique, the latter must be primed with saline to allow for electrical conduction: any air lock will, thus, hinder the flow of current. Therefore, we set out to explore an alternative mode of stimulation by use of a catheter containing a removable stylet. This study examines the reliability of this new technique. METHODS: In 71 patients undergoing surgery, a thoracic epidural catheter was inserted preoperatively. Loss of resistance was used to identify the epidura

On the activation threshold of nerve fibers using sinusoidal electrical stimulation.

Carpal tunnel syndrome (CTS) diagnosis could be improved by selectively activating different types of nerve fibers traversing the carpal tunnel based on their diameter. The objective of this study was to establish the types of fibers activated by different sinusoidal electrical stimuli. The frequencies selected correspond to those used in an available application known as current perception threshold (CPT). This method has been proposed in the literature to assess the severity of CTS in a patient. CPT operates by varying the amplitude and frequency of stimulating sine wave currents. Subthresho

Mechanisms of high-frequency electrical stimulation of the stomach in gastroparetic patients.

Previous studies have demonstrated that high frequency and low energy gastric electrical stimulation (GES) reduced nausea and vomiting in gastroparetic patients without improving gastric emptying. The mechanisms of action for this have not been clarified. The aim of our study was to investigate the effects of GES on autonomic function, gastric distention and tone, and central control mechanism in gastroparetics patients. 10 gastroparetic patients refractory to standard medical therapy participated in this study and data were collected at baseline session, within two weeks before surgery for im

Reanimation of paralyzed laryngeal muscles by electrical stimulation synchronized with inspiration.

OBJECTIVE: To determine the effects of electrical stimulation on denervated cat posterior cricoarytenoid (PCA) muscle. STUDY DESIGN AND SETTING: This study was conducted on six cats with PCA muscle denervation. All animals were sacrificed 12 weeks after surgery and the glottal area in the live animals and the fiber diameters of PCA muscle were obtained. RESULTS: Signals synchronized with inspiration were recorded and transmitted to stimulate PCA muscle. The abduction of the paralyzed vocal cord during inspiration was observed; this allows enough flow of air through the larynx to maintain the

Unloading reaction to electrical stimulation at neutral and supinated ankle positions.

An unloading reaction has been characterized as a modified flexor reflex (FR), in which the standing subjects decrease the load on the stimulated foot and increase the load on the contralateral side, but, without withdrawal of the stimulated foot. Different behavioral circumstances have been shown to modulate this reflex. It is not known whether unloading reactions can be modulated with a loaded supinated ankle position, which, in excess, may result in an ankle sprain injury. Since ankle sprain depends on the load applied to a supinated foot, our premise is that unloading reactions may protect

Effects of electrical stimulation of the raphe area on the micturition reflex in cats.

The raphe nucleus has a variety of physiological functions, including emotion, regulation of skeletal muscle motoneurons, spinal transmission of nociceptive signals, sleep, respiration, gastric motility, and cardiovascular function. Recent evidence has shown that centrally administered serotonin has modulatory effects on micturition function, and that decreased brain serotonin might underlie depression and an overactive bladder. We applied high-frequency stimulation (HFS; 0.2-ms duration, 100 Hz) in the raphe nucleus and the adjacent midline area in 20 supracollicular decerebrate cats, which m

Surface electrical stimulation of the quadriceps femoris in patients affected by haemophilia A.

Eighteen sessions of surface electrical stimulation was applied to the quadriceps femoris of the left leg of ten male subjects affected by severe haemophilia A, while ten healthy subjects constituted the control group. The isometric strength, the electromyographic activity and the diameter of the rectus femoris were measured in both legs before and after a six-week treatment period. After the treatment, the people affected by haemophilia showed a gain in strength by 13.8% in the stimulated leg and by 17.1% in the non-stimulated one. No changes were detected in the electromyographic activity. O

Is there a role for ultrasound and electrical stimulation following injury to tendon and nerve?

Ultrasound (US) and electrical stimulation have been widely used in hand therapy to promote recovery after nerve and tendon injuries. There is support for the use of low-dosage continuous wave and pulsed US for carpal tunnel syndrome and tendonitis. Iontophoresis with dexamethasone sodium phosphate can relieve pain in acute elbow tendonitis, but there is no support for phonophoresis for any tendonitis. Animal model research supports the use of low-dosage US to improve the mechanical properties of the Achilles tendon when initiated immediately after tenorrhaphy. There are no studies available w

Negative myoclonus induced by cortical electrical stimulation in epileptic patients.

Negative myoclonus (NM) is a motor disorder characterized by a sudden and abrupt interruption of muscular activity. The EMG correlate of NM is a brief (<500 ms) silent period (SP) not preceded by any enhancement of EMG activity (i.e. myoclonus). This study investigated the role of premotor cortex (PMC), primary motor cortex (MI), primary somatosensory area (SI) and supplementary motor area (SMA) in the pathophysiology of cortical NM by means of intracerebral low frequency (1 Hz) electrical stimulation. In three drug-resistant epileptic patients undergoing presurgical evaluation, we delivered s

Predicting optimal electrical stimulation for repetitive human muscle activation.

Functional electrical stimulation is the use of electrical currents to activate paralyzed muscles to produce functional movements. Muscle force output must meet or exceed the external load to maintain a posture or produce movements. A mathematical force-fatigue modeling system that predicts muscle force responses during repetitive electrical stimulation has been developed in our laboratory to help identify stimulation patterns that optimize force output for individual subjects. This study tests how well this model predicts the number of contractions that can be maintained above a required forc

Programmed electrical stimulation in Brugada syndrome: how reproducible are the results?

INTRODUCTION: Inducibility of ventricular arrhythmias at programmed electrical stimulation (PES) ranges between 50% and 80% of patients with Brugada syndrome. However, the variety of PES protocols and the lack of data relative to a control group or to ventricular arrhythmia reproducibility contribute to a still undefined interpretation of PES outcome in Brugada syndrome. METHODS AND RESULTS: Twenty-one patients with Brugada syndrome (18 men and 3 women; mean age 34 years; 9/21 symptomatic; 8/21 with SCN5A gene mutation) underwent a PES protocol from two right ventricular sites. The endpoint w

Functional electrical stimulation of bladder and bowel in spinal cord injury.

The purpose of this collective review is to examine the use of functional electrical stimulation for incontinence. The Finetech-Brindley bladder system enhances voiding through stimulation via electrodes implanted around the ventral sacral roots. Detrusor hyperreflexia is eliminated through complete dorsal rhizotomy, which results in loss of reflex defecation and reflex erection/reflex lubrication. Consequently, a new system is being devised in which functional electrical stimulation for incontinence in spinal cord injury can be achieved without dorsal rhizotomy.

Effects of electrical stimulation and seminal plasma on the motility of mouse sperm.

The effects of electric current (in vivo and in vitro) and seminal plasma on epididymal and ejaculated sperm obtained from C57BL x CBA and C57BL/6J mice were investigated by studying motility parameters, fertilization and embryo development. Electroejaculates were obtained by applying a series of computer-generated sinusoidal alternating currents (0.25-3.0 V at 50 Hz) delivered for 1, 2 and 3 s with 1-s rest periods using a four-electrode rectal probe for 4 min. Epididymal sperm obtained from the same mice were either subjected to electric current in vitro in a Plexiglass chamber or incubated
