Autobiographical Memory Specificity in Major Depression Treated With Electroconvulsive Therapy.

OBJECTIVE: Autobiographical memory in major depression is characterized by reduced specificity, which reflects the tendency to summarize categories of events rather than recall specific instances of events situated in a time and place. This widely studied cognitive marker for depression has not been extensively examined in patients treated with electroconvulsive therapy (ECT). METHODS: We conducted a retrospective chart review of a naturalistic cohort of patients receiving a course of brief-pulse predominantly bitemporal ECT for a major depressive episode. Patients completed the recent life s…

Rocuronium-sugammadex decreases the severity of post-electroconvulsive therapy agitation.

A 51-year-old male patient with a diagnosis of refractory major depressive disorder and unresponsive to antidepressants underwent 8 sessions of electroconvulsive therapy (ECT) every 48 hours. Succinylcholine was used for muscle relaxation until the sixth ECT session, and midazolam was administered for severe emergence agitation that recurred after each session. In the sixth ECT session, rocuronium, 0.4 mg/kg, was used for muscle relaxation and sugammadex, 2 mg/kg, for reversal of muscle paralysis. Subsequently, a clear decrease in post-ECT agitation was observed. We suggest that this combinati…

Treatment of post-electroconvulsive therapy headache with topical methyl salicylate.

Headache after administration of electroconvulsive therapy (ECT) is common, affecting approximately half of patients treated. Post-ECT headache is typically treated with acetaminophen or nonsteroidal anti-inflammatory drugs but occasionally requires agents such as sumatriptan, opioids, or β-blockers. We report on a patient whose severe post-ECT headaches responded completely to methyl salicylate ointment, applied to the area of his temporalis and masseter muscles. Topical methyl salicylate is generally well tolerated and may be a viable option for some patients with post-ECT headache.

Takotsubo cardiomyopathy after electroconvulsive therapy: a case report and review.

Takotsubo cardiomyopathy (TCM) is a syndrome of reversible stress-induced cardiomyopathy associated with profound emotional stress and a variety of medical illnesses and procedures, including electroconvulsive therapy (ECT). We describe 1 case of ECT-induced TCM followed by a successful retrial of ECT. We further discuss the management of ECT-induced TCM and the decision to perform a second trial of ECT in patients with this complication. Given the current understanding of the pathogenesis of TCM, it is appropriate to discontinue ECT during the acute setting of TCM. After the resolution of the…

Memory performance in severely depressed patients treated by electroconvulsive therapy.

OBJECTIVES: Depression is accompanied by disturbed implicit (unconscious) and explicit (conscious) memory functions. The aim was the assessment of immediate and delayed verbal and visual memory functions, concentration/attention during the course of electroconvulsive therapy (ECT) treatment. METHODS: Twenty severely depressed, drug-treatment resistant, elderly patients were assessed with the Wechsler Memory Scale-Revised (WMS-R) before and at the end of the ECT series. RESULTS: Patients revealed deficits in acquisition (immediate verbal and visual memory), attention/concentration, and retrie…

Predictors of remission after electroconvulsive therapy in unipolar major depression.

CONTEXT: Electroconvulsive therapy (ECT) is the most effective biological treatment for major depression. However, there is little agreement about clinically useful predictors of acute ECT outcomes. OBJECTIVE: To assess whether age, sex, burden of comorbid physical illness, age at onset, history of recurrence, episode duration, chronic depression or comorbid dysthymia, melancholic features, episode severity, and medication resistance are predictors of remission after an acute course of ECT. DESIGN: We performed an analysis using data gathered prospectively in 328 patients with unipolar major…

Successful treatment of status migrainosus after electroconvulsive therapy with dihydroergotamine.

We describe a case of status migrainosus precipitated by electroconvulsive therapy (ECT) that was refractory to treatment with triptan medications but which resolved with dihydroergotamine (DHE). We briefly review some of the receptor pharmacology possibly related to this observation.

Retrograde amnesia and bilateral electroconvulsive therapy. Long-term follow-up.

Memory for past events was assessed in 43 patients who had been prescribed bilateral electroconvulsive therapy (ECT) for relief of depressive illness. Four memory tests of personal or public events were administered before ECT, shortly after the fifth treatment, one week after completion of treatment, and about seven months later. The results indicated that ECT can initially disrupt recall of events that occurred many years previously, but recovery of these memories was virtually complete by seven months after treatment. It was also clear that persisting memory loss for information acquired on…

Neurological Devices; Reclassification of Electroconvulsive Therapy Devices; Effective Date of Requirement for Premarket Approval for Electroconvulsive Therapy Devices for Certain Specified Intended Uses. Final order.

The Food and Drug Administration (FDA) is issuing a final order to reclassify the electroconvulsive therapy (ECT) device for use in treating catatonia or a severe major depressive episode (MDE) associated with major depressive disorder (MDD) or bipolar disorder (BPD) in patients age 13 years and older who are treatment-resistant or who require a rapid response due to the severity of their psychiatric or medical condition, which is a preamendments class III device, into class II (special controls). FDA is also issuing this final order to require the filing of a premarket approval application (P…

Magnetic Seizure Therapy vs Electroconvulsive Therapy in Schizophrenia: Prefrontal-Amygdala Plasticity and Cognitive Safety.

PURPOSE: Reducing electroconvulsive therapy (ECT)'s cognitive burden without compromising efficacy is critical. Although magnetic seizure therapy (MST) shows comparable symptom remission in schizophrenia, its neuroanatomical safety-particularly limbic preservation-lacks controlled trial validation. PATIENTS AND METHODS: This triple-blind RCT (NCT02746965) randomized 34 schizophrenia patients to ECT (n=16) or MST (n=18). Structural changes were quantified via 3T MRI (FreeSurfer). Outcomes included Positive and Negative Syndrome Scale (PANSS), Repeatable Battery for the Assessment of Neuropsych…

A Computational Modeling Study of Focal Electrically Administered Seizure Therapy and Frontoparietal Electroconvulsive Therapy.

OBJECTIVES: Modification of the electrode placement with electroconvulsive therapy (ECT) impacts on patient outcomes, including cognitive effects. The investigation of alternative right unilateral placements (RUL) has the potential to further improve ECT outcomes by reducing cognitive side effects. METHODS: This study investigated differences in the intracerebral distribution of current density comparing the standard d'Elia right unilateral positioning (temporoparietal [TP]) with two recently developed RUL placements, focal electrically administered seizure therapy (FEAST) and frontoparietal …

The Role of the Autonomic Nervous System in the Mediation of the Resolution of Suicidal Ideation With Electroconvulsive Therapy: A Hypothesis and Review of Heart Rate Variability Over a Course of Electroconvulsive Therapy.

Longitudinal observational studies have shown a meaningful decrease in suicidal thinking and suicidal behavior after receipt of electroconvulsive therapy (ECT). The antisuicide effect of ECT may be related to success in the global relief of the presenting syndrome such as depressive or psychotic illness. However, it is possible that the antisuicide effect is specific to ECT per se, over and above the relief of the clinical syndrome. Electroconvulsive therapy is associated with many observable neurochemical and physiologic effects, and some of these may plausibly be specifically linked to an an…

Over the Cuckoo's Nest: Does Experiencing Electroconvulsive Therapy Change Your Mind? A Mixed Methods Study of Attitudes and Impact of Electroconvulsive Therapy on Patients and Their Relatives.

OBJECTIVE: Electroconvulsive therapy (ECT) is an effective treatment for major depressive disorder, but some aspects remain controversial. Few studies have taken an in-depth mixed methods approach toward the study of attitudes, and there are no significant studies that explore the change of attitudes before and after treatment. The aim was to compare attitudes of patients and their relatives before and after ECT using quantitative and qualitative methods. METHODS: One hundred twenty-three participants were recruited. Forty-one patient/relative participants were recruited from 2 accredited ECT…

The postictal systems window hypothesis of electroconvulsive therapy in major depression.

Electroconvulsive therapy (ECT) remains the most effective acute treatment for severe major depressive disorder, particularly in melancholic, psychotic, suicidal, catatonic or retarded, and treatment-resistant presentations. Existing mechanistic accounts are biologically valid but are often presented as parallel lists: monoamine release, excitatory-inhibitory rebalancing, neuroendocrine activation, immune modulation, neurotrophic plasticity, hippocampal change, and network rewiring. We propose the postictal systems window hypothesis. The organizing principle is seizure-induced postictal recoup…
