OBJECTIVE: Previous studies, in which the vestibular aqueduct (VA) was determined using axial CT, have indicated that the VA sizes in patients with Ménière's disease were smaller than those in the subjects of the control group and that 25-35% of ears with Ménière's disease were "non-visible type." In this study, in addition to obtaining the axial size, we measured the VA size along the vertical plane by using vertical multiplanar reconstruction (MPR) images.

Patients with intractable vertigo due to clinical Meniere's disease were offered treatment with Innovar, a neurolept analgesic. The patients were from a single clinical practice, had failed conventional dietary and medical treatment and were eligible on clinical grounds for endolymphatic sac surgery. After a follow-up of 2-8 years, 58% of patients had long-lasting relief of vertigo. Hearing was not affected. OBJECTIVE: To study the development of the bone around the vestibular aqueduct (VA) in Meniere's disease (MD) by CT.
METHOD: The experiment consisted of three groups; normal ear group, non-MD vertigo group and MD group. VA in each group was examined by CT, and the minimum distance between the posterior semicircular canal and the posterior petrous surface where contain the endolymphatic sac.
RESULT: VA visualization rate in MD was low, P-P distance in MD group was shorter than that of normal ear group and non-MD vertigo group.

The direct route to the cerebellopontine (CP) angle through the ear (retrolabyrinthine approach) represents a significant advance in neuro-otologic surgery. After incising the dura between the endolymphatic sac and the lateral venous sinus, a self-retaining retractor is used to collapse the lateral sinus, which provides a wide exposure of the CP angle, with visualization of the 5th through 11th cranial nerves. The etiology of Meniere's disease is unknown and the treatment is empirical and at best controversial. The goals of treatment are hearing preservation and the control of vertigo. Surgery for vertigo is recommended where there is objective evidence of a unilateral inner ear disorder, when medical therapy fails, and when the patient demands definitive therapy.
