Implementation of an enhanced recovery after surgery (ERAS) program in patients undergoing cytoreductive surgery and hyperthermic intraperitoneal chemotherapy: study protocol for a prospective multicenter interventional trial (EPICH study).

OBJECTIVES: This study aims to evaluate the clinical impact of introducing an Enhanced Recovery After Surgery (ERAS) protocol in the management of patients undergoing cytoreductive surgery (CRS), with or without hyperthermic intraperitoneal chemotherapy (HIPEC). By addressing a population at high risk of postoperative complications and delayed recovery, the study seeks to determine whether ERAS can improve short-term outcomes, optimize perioperative care, and promote faster and safer recovery in a standardized, evidence-based manner across multiple centers. METHODS: The EPICH study is a multi…

Multimodal prehabilitation combined with perioperative enhanced recovery after surgery care for older patients undergoing spinal fusion surgery in China: protocol for a multicentre randomised controlled trial (PRACTICE trial).

INTRODUCTION: Besides the comorbid risk factors shared by older and younger patients, older individuals may also experience malnutrition, as well as cognitive or functional impairments. The accumulation of frailty and various geriatric syndromes in older individuals results in decreased physiological reserves, which makes the recovery process after spine surgery particularly challenging. Theoretically, combining the presurgery optimisation provided by a multimodal prehabilitation programme with the reduction of surgical stress provided by an enhanced recovery after surgery (ERAS) programme cou…

Topical Ascorbic Acid for Corneal Endothelial Cell Density Enhancement, Protection, and Recovery in Fuchs Dystrophy After Complicated Cataract Surgery.

We report a case highlighting the potential of topical ascorbic acid (AA) as an adjunctive strategy for supporting corneal endothelial stability in Fuchs endothelial corneal dystrophy (FECD) after complicated cataract surgery. A 74-year-old man with bilateral FECD presented with progressive vision loss. Baseline endothelial cell density (ECD) was 1,996 cells/mm2 in the right eye and 1,988 cells/mm2 in the left eye. After 15 months of AA treatment, ECD increased to 3,079 and 3,008 cells/mm2, respectively. The left eye underwent uncomplicated cataract surgery with minimal ECD loss (9.61%). The r

Texture-optimized early enteral nutrition enhances gastrointestinal recovery and caloric intake after general surgery: A randomized controlled trial.

BACKGROUND & AIMS: Early oral feeding is recommended in enhanced recovery protocols; however, the impact of diet texture on postoperative gastrointestinal recovery remains insufficiently studied. We evaluated whether a standardized semi-solid, nutritionally complete postoperative diet improves gastrointestinal recovery and caloric delivery compared with conventional stepwise feeding after general surgery. METHODS: In this single-center, prospective, single-blind randomized controlled trial, 120 patients undergoing noncardiothoracic surgery under general anesthesia were assigned (1:1) to recei

Dexmedetomidine enhances recovery after gastrointestinal cancer surgery by protecting the endothelial glycocalyx: A randomized, double-blind, placebo-controlled study.

BACKGROUND: The vascular endothelial glycocalyx (VEG) plays a critical role in maintaining vascular barrier integrity, regulating inflammation, and ensuring microcirculatory homeostasis. Surgical stress and systemic inflammation can disrupt the glycocalyx, leading to endothelial dysfunction, impaired microcirculation, and adverse postoperative outcomes. dexmedetomidine (DEX), an α2-adrenergic agonist with anti-inflammatory and organ-protective properties, has been suggested in preclinical and clinical studies to mitigate glycocalyx degradation, yet evidence in gastrointestinal cancer surgery r

Guided meditation as an adjunct to enhance postoperative recovery after cardiac surgery: study protocol for a prospective randomized controlled feasibility trial.

BACKGROUND: Cardiac surgical procedures are associated with postoperative neurological complications such as cognitive decline and delirium, which can complicate recovery and impair quality of life. Perioperative depression and anxiety may be associated with increased mortality after cardiac surgeries. Surgical prehabilitation is an emerging concept that includes preoperative interventions to potentially reduce postoperative complications. While most current prehabilitation interventions focus on optimizing physical health, mind-body interventions are an area of growing interest. Preoperative

Preoperative vestibular ablation with gentamicin and vestibular 'prehab' enhance postoperative recovery after surgery for pontine angle tumours--first report.

CONCLUSIONS: Preoperative gentamicin in combination with vestibular 'prehab' offers a possibility to reduce postoperative malaise and speed up recovery and may be used for patients undergoing such surgery when there is remaining vestibular function. OBJECTIVES: Removal of pontine angle tumours in a patient with remaining vestibular function causes symptoms of acute vestibular loss. A simultaneous cerebellar lesion can cause a combined vestibule-cerebellar lesion. PATIENTS AND METHODS: Twelve patients with pontine angle tumours but with near normal vestibular function were treated with intrat
