A network-based meta-population approach to model Rift Valley fever epidemics.

Rift Valley fever virus (RVFV) has been expanding its geographical distribution with important implications for both human and animal health. The emergence of Rift Valley fever (RVF) in the Middle East, and its continuing presence in many areas of Africa, has negatively impacted both medical and veterinary infrastructures and human morbidity, mortality, and economic endpoints. Furthermore, worldwide attention should be directed towards the broader infection dynamics of RVFV, because suitable host, vector and environmental conditions for additional epidemics likely exist on other continents; in…

Unspecified gastroenteritis illness and deaths in the elderly associated with norovirus epidemics.

BACKGROUND: New variant strains of norovirus have emerged worldwide in recent years, evolving by mutation much like influenza viruses. These strains have been associated with a notable increase in the number of annual norovirus outbreaks. However, the impact of such increased norovirus activity on morbidity and mortality is not clear because norovirus infection is rarely specifically registered. METHODS: We studied trends of gastroenteritis with unspecified cause in medical registrations (ie, general practitioner [GP] visits, hospitalizations, and deaths) and their association with known temp

[Environmental drivers of emergence and spreading of Vibrio epidemics in South America].

Vibrio cholerae and V. parahaemolyticus are the two Vibrio species with a major impact on human health. Diseases caused by both pathogens ar … in 1997. These constituted exceptional experiments to evaluate the relationships between the Vibrio epidemics and changes in the environment. The epidemic radiations of V. cholerae and V. parahaemolyticus constitute to clear examples supporting the oceanic dispersion of pathogenic vibrios and have enabled the identification of El Niño events as a potential …

Initial clinical management of symptomatic adult patients during influenza A (H1N1) epidemics.

BACKGROUND: Pandemic influenza A (H1N1) has emerged rapidly in Argentina since May 2009. Preliminary comparisons with seasonal influenza suggest that H1N1 disproportionately affects younger patients, generally causing mild disease, but in a minority of cases can be lethal. OBJECTIVE: The aim of this study was to develop a clinical tool for the initial management of patients with influenza-like syndrome, within the context of the novel H1N1 virus epidemic, to detect patients who need further investigation (e.g., chest X-ray study) for the diagnosis of pneumonia. METHODS: We prospectively stud

Religious and cultural traits in HIV/AIDS epidemics in sub-Saharan Africa.

BACKGROUND: The pandemic of HIV/AIDS in sub-Saharan Africa and the rise of epidemics in Asia to the previously unforeseen level are likely to have global social, economic, and political impacts. In this emergency, it is vital to reappraise the weight of powerful religious and cultural factors in spreading the disease. The role of Islam in shaping values, norms, and public policies in North African states is to be appreciated for the lowest HIV prevalence in their populations. Yet, the place of religion in prevention of the disease diffusion is not fully understood nor worldwide acknowledged by…

Unacceptably high mortality related to measles epidemics in Niger, Nigeria, and Chad.

BACKGROUND: Despite the comprehensive World Health Organization (WHO)/United Nations Children's Fund (UNICEF) measles mortality-reduction strategy and the Measles Initiative, a partnership of international organizations supporting measles mortality reduction in Africa, certain high-burden countries continue to face recurrent epidemics. To our knowledge, few recent studies have documented measles mortality in sub-Saharan Africa. The objective of our study was to investigate measles mortality in three recent epidemics in Niamey (Niger), N'Djamena (Chad), and Adamawa State (Nigeria). METHODS AND…

Balancing selection at the prion protein gene consistent with prehistoric kurulike epidemics.

Kuru is an acquired prion disease largely restricted to the Fore linguistic group of the Papua New Guinea Highlands, which was transmitted during endocannibalistic feasts. Heterozygosity for a common polymorphism in the human prion protein gene (PRNP) confers relative resistance to prion diseases. Elderly survivors of the kuru epidemic, who had multiple exposures at mortuary feasts, are, in marked contrast to younger unexposed Fore, predominantly PRNP 129 heterozygotes. Kuru imposed strong balancing selection on the Fore, essentially eliminating PRNP 129 homozygotes. Worldwide PRNP haplotype d

[Epidemics and the evolution of medicine in Iran during the XIX century].

In a description of a historical case in 19th Century Persia, this article endeavours to show how changes in medical thought occurred. It ar … dical thought, together with the awakening of socio-political consciousness regarding the spread of epidemics, were the main factors clearing the path to a natural integration of Western medicine in 19th century Iran.

Molecular characterisation of type 1 polioviruses associated with epidemics in South Africa.

The molecular epidemiology of wild-type 1 polioviruses isolated in South Africa during 2 major poliomyelitis epidemics in the 1980s and during the pre- and inter-epidemic periods was investigated by partial sequence analysis across the VP1/2A junction. Poliovirus-specific primers were used to amplify and subsequently sequence the region of interest. Viruses belonging to different genotypes were found to have been responsible for the 2 outbreaks. The Gazankulu outbreak in 1982 was caused by a poliovirus genotype which was unique to South Africa and which circulated endemically throughout much o…

Weather factors in the prediction of western equine encephalitis epidemics in Manitoba.

Cases of western equine encephalitis in horses in 1987 in western USA and Manitoba, Canada were examined by backward trajectory analysis of … . tarsalis in this area. Weather factors involved in outbreaks from 1975-88 were analysed to see if epidemics in Manitoba (23 or more cases in horses) could be predicted. The conditions for epidemics could be defined as follows: (a) the number of cases in horses in USA was 98 or more, (b) winds were southerly with speeds 45 kmh-1 or higher, and (c) counts o …

"Measles, mumps and mud": childhood epidemics at the turn of the century.

Childhood epidemics were a commonly accepted part of life in the two decades around the turn of the century. Official health and education records portray, however, not an apathetic acceptance of epidemics but a public concern with their prevention and control. Education boards referred to outbreaks of measles, mumps, whooping-cough, scarlet fever and diphtheria in explaining poor attendance rates while school inspectors commented on the impact that epidemics had on schoolwork. District health officers closed and disinfected schools to prevent the spread of infection, and outside of schools nu…

Monitoring for multiple malformations in the detection of epidemics of birth defects.

Although most known human teratogens often produce a combination of birth defects in an affected infant, surveillance programs aimed at detecting epidemics of birth defects usually only monitor rates of individual defects. A drawback to this approach is that an increase in the rate of infants affected with a specific combination of defects may lead to little or no increase in the rates of component defects. Using the Poisson distribution, we show that, compared with monitoring for individual defects, monitoring for combinations of two and three defects may require fewer numbers of births to de…

[Treatment of cholera in the adult. Studies during recent epidemics in Apulia].

A rapid and adequate intravenous infusions of saline and alkali in correcting hypovolemic shock, alectrolyte abnormalities, acidosis and renal failure, is the choice treatment in the majority of acute cholera patients. On the basis of the knowledge of chemical composition of the gastrointestinal fluids loss and the definition of exact mechanism of metabolic features observed in acute cholera patients, it has been possible, in the last ten years, to develop and utilize in a large numbers of patients, a more exact and adequate treatment consisting of replacement of water and electrolytes depleti

Dengue epidemics in two distinct periods reveal distinct epidemiological, laboratorial and clinical aspects in a same scenario: analysis of the 2010 and 2013 epidemics in Mato Grosso do Sul, Brazil.

BACKGROUND: Dengue is a major problem in Brazil. Epidemiological and clinical aspects were characterized in patients from two epidemics which occurred in Mato Grosso do Sul, Brazil. METHODS: Dengue cases were classified according to the 2009 WHO criteria, tested by serological and molecular biology tests and analysed for nonstructural protein 1 (NS1) antigenemia. RESULTS: Dengue was confirmed in 78.7% (48/61) and 75.6% (118/156) of the cases studied in 2010 and 2013, respectively. DENV-1 and DENV-2 were the serotypes involved in the 2010 epidemic and DENV-4 in the 2013 one. Most of the cases…
