Is there a threshold that triggers cortical arousals in obstructive sleep apnea.

STUDY OBJECTIVES: To determine whether there is a consistent epiglottic pressure value that predicts respiratory arousal from sleep. METHODS: Thirty-one patients with obstructive sleep apnea underwent overnight polysomnography while instrumented with an epiglottic catheter to measure airway pressures. Nadir epiglottic pressures during respiration events (obstructive apneas/hypopneas) terminated with or without arousals were compared. The events were selected by two methods, (1) 20 events with/without arousals were randomly selected, and (2) Events were sampled in pairs (one terminated with ar

Hypoplasia of the epiglottis in a middle-aged man.

Hypoplasia of the epiglottis diagnosed at adulthood is extremely rare. Most patients with epiglottic hypoplasia have presented life-threatening symptoms of respiratory distress and severe aspiration in infancy or early childhood. This is a case of congenital epiglottic hypoplasia found in a 42-year-old man complaining of mild hoarseness and throat discomfort. Strobolaryngoscopy and CT scans revealed a unique anomaly with isolated epiglottic hypoplasia and normal appearance of other laryngeal structures. This case is discussed with a review of the literature on this rare occurrence.

Adult vallecular cyst: thirteen-year experience.

OBJECTIVE: To assess the characteristics of adult vallecular cyst. STUDY DESIGN AND SETTINGS: A retrospective chart review from a university affiliated hospital. SUBJECTS AND METHODS: Clinical manifestations and airway management of 38 consecutive adult patients with vallecular cyst admitted between 1992 and 2004 were studied. RESULTS: Two distinct groups were identified: infected (n = 24) and noninfected (n = 14). Twenty-two (91.7%) patients of the former group had acute epiglottitis with an abrupt onset culminating in abscess formation in 19 (79.2%) and airway compromise in 9 (37.5%) comp…

Re-writing the laryngeal anatomy.

BACKGROUND: The larynx is a small but highly specialized structure, exhibiting notable sex-related variations. Previous morphometric studies often focused on isolated features, such as the thyroid angle, limiting understanding of overall laryngeal anatomy. MATERIALS AND METHODS: We retrospectively analyzed 157 high-resolution neck computed tomography (CT) scans (67 males, 90 females; age range: 21-89 years) obtained for indications unrelated to laryngeal pathology. Morphometric measurements included laryngeal length, dimensions of the thyroid, cricoid, and arytenoid cartilages, glottic length

The mineralised epiglottis: a narrative review illustrated by an index case.

PURPOSE: Epiglottic mineralisation is an uncommon anatomical variant whose embryological, histological, epidemiological, imaging and clinical implications are incompletely synthesised in the literature. We provide a narrative review of these topics, illustrated by an index case of almost complete epiglottic mineralisation identified on CT angiography. METHODS: A case of incidentally detected epiglottic mineralisation on retrospective head-and-neck CT angiography was analysed using multiplanar reformations, maximum-intensity projections, bone-window images, and three-dimensional reconstruction

A reappraisal of the radiologic findings of acute inflammation of the epiglottis and supraglottic structures in adults.

PURPOSE: To evaluate the radiologic characteristic of acute inflammation of the epiglottis and supraglottic structures in adults. METHODS: The clinical and radiographic findings in 27 adult patients with epiglottitis (average age, 43 years; range, 28 to 81 years) were compared with those of a control group of asymptomatic subjects (n = 15; average age, 48 years; range, 24 to 79 years). Unusual clinical aspects in the current series included two patients who were positive for human immunodeficiency virus, 1 with acquired immunodeficiency syndrome and Kaposi sarcoma, 3 with emphysematous epiglo…

Fetal development of the human epiglottis revisited: appearance of GFAP-positive mesenchymal cells and fibrous connections with other laryngeal and lingual structures.

The fetal anatomy of the human epiglottis has not yet been fully described. We investigated the histology (paraffin-embedding) of 18 mid-term fetuses at 7-25 weeks of gestation (three fetuses each at 7, 9, 12, 15, 20 and 25 weeks). A mesenchymal condensation of the epiglottic cartilage appears posterior and somewhat superior to the hyoid body at 9 weeks, but at 12 and 15 weeks, the root or inferior part descends to the level of the thyroid cartilage. The covering epithelium stains much darker with hematoxylin than other pharyngeal epithelia. After 20 weeks, the epiglottis again protrudes super

[Supraglottitis in adults at a single center experience 1989-2009].

Acute epiglottitis in children has almost vanished since the start of Hib vaccinations. Supraglottitis often develops in adults more slowly than in children. We reviewed all patients at Tampere University Hospital over the age of 18, who had been recorded with a diagnosis of epiglottitis or supraglottitis upon discharge from the hospital between 1989 and 2009. The most common symptoms were sore throat and pain on swallowing. Streptococcus was the most common causative agent. Most of the 308 patients had received conservative treatment. Supraglottitis should be remembered as possible diagnosis …

Emergency evaluation and management of the sore throat.

Although sore throat is a very common complaint in the emergency department and is often associated with benign conditions, such as pharyngitis, it can be the early onset of something much more serious and even life threatening. A thorough history and examination are vital to the correct diagnosis but a high index of suspicion must be maintained to diagnose less common but serious pathology, such as epiglottitis and retropharyngeal abscess. Airway management is paramount to survival, and aggressive means should be taken early when epiglottitis is considered.

A case of congenital aplasia of the epiglottis in an adult.

Aplasia or hypoplasia of the epiglottis in an adult is rarely reported congenital anomaly of the larynx. Most patients with epiglottic aplasia have presented fatal symptom of respiratory distress and severe aspiration in infancy or childhood. We present a case of congenital absence of the epiglottis found in a 33-year-old woman complaining of recurrent lingual tonsillitis. Laryngoscopic finding and CT scan revealed an isolated epiglottic aplasia and normal appearance of other laryngeal structure. This is the first description of physiologic evaluation in an epiglottic anomaly to find the compe

[A Christmas straw goat astray].

A 22-year-old otherwise healthy man presented to our clinic with suspected acute epiglottitis. The patient had a 1-week history of pain in the throat and fever for the latest 24 hours. During the physical examination, the patient mentioned that he had been eating a Christmas straw goat at a party one week previously. Direct fiberoptic laryngoscopy showed a red and swollen lingual surface of the epiglottis, but no foreign bodies were identified. After inhalation of adrenalin, a 4 cm straw became visible in the epiglottic vallecula. Mimic of acute epiglottitis from a straw in the vallecula has t…

Granulomatosis with polyangiitis involving the epiglottis.

Granulomatosis with polyangiitis (GPA) frequently involves the upper respiratory tracts, but involvement of the epiglottis is extremely rare. This report describes a patient initially presenting with dysphagia and increasing stridor due to epiglottitis. Bronchoscopy showed swelling of the epiglottis with partly whitish nodular lesions, with biopsy specimens showing neutrophil infiltration and necrosis. Chest computed tomography showed multiple nodular consolidations in the bilateral lung parenchyma, and histological findings were consistent with vasculitis. The patient was diagnosed with GPA a…

Dysphagia due to diffuse idiopathic skeletal hyperostosis: an analysis of five cases.

We report five cases of dysphagia caused by diffuse idiopathic skeletal hyperostosis or Forestier's disease of the cervical spine observed by clinical evaluations, X-ray and videofluoroscopy. Dysphagia in particular was present for solid foods in all patients. Three patients showed bolus aspiration into the airways, mainly in the post-swallowing phase and one patient with preserved epiglottic tilt showed post-swallowing penetration. The physiopathogenetic mechanisms affecting swallowing dynamics were mechanical compression of the pharyngeal lumen associated with abnormal epiglottic tilt, incom

Ten years' experience of Haemophilus influenzae infection at Birmingham Children's Hospital.

A survey of serious infections due to Haemophilus influenzae, in the years 1973-1984, was made at the Birmingham Children's Hospital. Eight-three cases were recorded, comprising 42 of meningitis, 18 of septicaemia, 16 of epiglottitis and seven of septic arthritis or osteomyelitis. The mortality rate for meningitis and septicaemia was 5%, but no child with epiglottitis or septic arthritis died. The data are compared with those of American studies. The incidence of severe haemophilus infection in the inner city area of Birmingham was estimated to be at least 1 in 950 children during the period o…

Immunization against Haemophilus influenzae type B fails to prevent orbital and facial cellulitis: results of a 25-year study among military children.

Vaccines against Haemophilus influenzae type B (HI) and Streptococcus pneumoniae (SP) have dramatically reduced the incidence of bacterial meningitis (due to both HI and SP) and epiglottitis (due to HI) in childhood. The effects of these vaccines on other conditions, however, are less clear. We report an analysis of the effect of serial deployment of various HI and SP vaccines over a 25-year period, involving an examination of over half a million pediatric hospitalizations occurring in Army hospitals worldwide. We show that, in marked contrast to the reduction in the number of meningitis and e…

Anesthesia treatment in cases of infant epiglottis cyst emergency extirpation operations.

Congenital epiglottic cyst is a rare embryonic disease. As a congenital laryngeal mucocele, its clinical manifestations include repeated sudden dyspneic respiration and even suffocation accompanied by laryngeal stridor after birth. During food intake, bucking and vomiting is a key feature. Delay in diagnosis and treatment of the disease affects growth and the development of neonatorum leading to suffocation and death. This study was designed to investigate the safety of anesthesia in infants with congenital epiglottic cyst during operation to reduce the occurrence of its complications. The tre

Purpura Fulminans as a Rare Manifestation of Invasive Haemophilus influenzae Disease: A Case Report.

Haemophilus influenzae (Hi) is a bacterium usually found in the upper respiratory tract of humans. Though it is recognized as a naturally occurring element in the human bacterial reservoir, Hi infections have the potential to be severe and even fatal, particularly when they result in conditions such as meningitis or epiglottitis. Because of this, Hi invasive infections are considered a reportable disease in Portugal. We report a case of a 58-year-old female, chronically adrenally suppressed on long-term steroids, who developed an invasive Hi type b infection that led to purpura fulminans and m…

Approaching the airway in prehospital emergency is a common and potentially life-saving practice.

Management by the environment is complex, which means a much higher percentage of difficult airways than in a regulated environment such as the operating room. Failure or prolonged attempt to tracheal intubation is associated with unfavorable outcomes and serious complications. Acute epiglottitis is a life-threatening disorder, classified as a medical emergency within the diseases of the upper respiratory airway and characterized by its sudden and deadly evolution if rapid intubation is not achieved to allow oxygenation of the patient. We describe a 36-year-old male patient with stridor, dyspn…
