[Juvenile epiphyseolysis of the femoral head--coxa vara development and treatment].

Children's coxa vara is based on epiphyseolysis of the proximal femoral epiphysis due to relative overload of a primarily healthy metaphysis, or to a "normal" load on diseased structures. These give passively way in the direction of varus and retroversion of the head in the presence of fatigue fractures. Etiologically the idiopathic C. vara infantum can be differentiated from the symptomatic forms with known causes. Broadening of the epiphyseal line, spur formation and cystic bone resorption of the medial-caudad portions of the metaphysis are early x-ray signs. When the process involves the wh

Hazards of internal fixation in the treatment of slipped capital femoral epiphysis.

We reviewed the records of 202 patients (308 hips) in whom a slipped capital femoral epiphysis had been fixed with pins or screws. A serious complication that was directly related to the use of internal fixation developed in eighty hips (26 per cent). The rate of complications in the 202 patients was 40 per cent. In thirty-six (18 per cent) of the 202 patients, an additional procedure was done to correct a pin-related complication. Forty-one hip joints had been penetrated by a pin. Other complications included avascular necrosis (fourteen hips), chondrolysis (nine), fracture (one), infection (

Thyroid hormone levels in patients with slipped capital femoral epiphysis.

Twelve patients with slipped capital femoral epiphysis and 12 age- and sex-matched controls were prospectively studied. Their height, weight, skeletal maturation, and thyroid status were assessed. The duration of symptoms and severity of the slip was recorded in the test group. There was no correlation between duration of symptoms and severity of the slip or delay in making the diagnosis and severity. Although 50 percent of patients with slipped capital femoral epiphysis were over the 90th percentile for weight, there was no overall significant difference in weight, height, or skeletal maturat

A computer model of slipped capital femoral epiphysis. Causes of fixation error and chondrolysis.

Penetration of the joint by metal pins can occur when treating slipped capital femoral epiphysis. Predisposing factors are present in the usual surgical methods. These include x-ray detail loss secondary to the pelvic projection and failure to obtain specific x-ray orientation to the axis of the capital epiphysis. One operative strategy to reduce these predispositions is presented. A probable mechanism of how chondrolysis can occur without joint penetration following extra capsular osteotomy is described with a theoretical remedy. Computer disease modeling and treatment simulation are the basi

[Anaesthesia in a patient susceptible to malignant hyperthermia (author's transl)].

A 13 year old girl who had suffered from an abortive form of malignant hyperthermia during tonsillectomy eight years before was scheduled for orthopaedic surgery. Dantrolene sodium, 3 mg/kg orally, was given prophylactically the day before surgery; preanaesthetic medication consisted of Thalamonal, a fixed combination of droperidol and fentanyl; anaesthesia was induced with methohexitone and maintained as neurolept anaesthesia with fentanyl and droperidol; tubocurarine was administered for tracheal intubation and intraoperative neuromuscular blockade. Using this anaesthetic regimen no adverse

Chondrolysis following slipped capital femoral epiphysis.

Chondrolysis of the hip following slipped capital femoral epiphysis (SCFE) is reviewed and 3 cases are presented. The roentgenographic findings and early symptoms may serve to warn the radiologist and clinician of the onset of chondrolysis. Chondrolysis without SCFE is rare. Unfortunately the etiology is not well understood, and often the clinical course cannot be altered.

[Traumatic separation of the epiphysis of the lower end of the femur].

The authors have treated 28 cases of traumatic separation of epiphysis of the lower end of the femur. They have noted the particular severity of the initial trauma. The results were less satisfactory when the child was younger and when the plate was crashed. However, 4 cases of fractures in breech delivery had rather good results. The authors think that a good reduction is essential, either close reduction fixed by Kirchner's wires or open reduction when the former is impossible.

Sugioka's Modified Hungria-Kramer intertrochanteric osteotomy in the treatment of severe slipped capital femoral epiphysis.

From May 1990 to November 1997, 24 cases of severe slipped capital femoral epiphysis were treated by an osteotomy that is a modification of the Hungria-Kramer intertrochanteric osteotomy proposed by Sugioka (Hungria-Kramer-Sugioka osteotomy or HKS osteotomy). The degree of displacement as seen on the frog-leg lateral radiograph of the proximal femur was measured according to the deviation of the longitudinal axis of the epiphysis from the center line of the neck (Fish classification). All hips were considered as grade III and underwent HKS osteotomy. Sugioka's radiographic study (true AP view

Which Radiographic Measurements Best Predict Contralateral Slip in Patients Who Undergo Pinning for Unilateral Slipped Capital Femoral Epiphysis?

BACKGROUND: Managing the contralateral hip in patients with unilateral slipped capital femoral epiphysis (SCFE) involves balancing the risks of prophylactic pinning against the potential for a subsequent slip. This study aimed to evaluate the role of radiographic methods in predicting contralateral slips in patients with unilateral SCFE. METHODS: We assessed frog-leg radiographs from 312 patients with unilateral SCFE, measuring the Billings, Southwick, Posterior Sloping Angle (PSA), and tilt angles. Seventy patients (22%) experienced contralateral slips, with a median follow-up of 42 months (

[Contralateral preventive screwing in proximal femoral epiphysiolysis].

PURPOSE: The incidence of bilaterality in slipped capital femoral epiphysis (SCFE) ranges, in the literature, from 19 per cent to 80 per cent. The role of contralateral pinning is to prevent late slipping of the femoral epiphysis and its complications. The purpose of this study is to assess the usefulness of routine preventive contralateral pinning in SCFE, and to evaluate its complications. MATERIAL AND METHODS: We reviewed retrospectively 74 consecutive patients treated for unilateral SCFE by associated routine preventive contralateral pinning. The age at surgery ranged from 10 years and 6

Morphologic Development of the Posterior Sloping Angle of the Proximal Femoral Epiphysis in Children: What Is the Peak Risk Age for Slipping?

OBJECTIVE: The purpose of this study was to describe the developmental patterns of the posterior sloping angle (PSA) of the proximal femoral epiphyseal growth plate (PFEP) in a cohort of healthy Chinese children aged 5 to 14 years. METHODS: Pelvic frog-leg lateral radiographs of 400 healthy children (n=800 hips) were retrospectively analyzed. The cohort included 215 males and 185 females with a mean age of 9.5 years (5 to 14). PSA measurements of the PFEP were obtained bilaterally from frog leg lateral radiographs. Subjects were equally divided into 10 subgroups of 40 patients each (80 hips p

Primary cam morphology; bump, burden or bog-standard? A concept analysis.

BACKGROUND: Cam morphology, a distinct bony morphology of the hip, is prevalent in many athletes, and a risk factor for hip-related pain and osteoarthritis. Secondary cam morphology, due to existing or previous hip disease (eg, Legg-Calve-Perthes disease), is well-described. Cam morphology not clearly associated with a disease is a challenging concept for clinicians, scientists and patients. We propose this morphology, which likely develops during skeletal maturation as a physiological response to load, should be referred to as primary cam morphology. The aim of this study was to introduce and

Primary healing of physeal separation under rigid fixation.

We observed the healing process under rigid external fixation after Salter-Harris type-1 or type-2 physeal separation at the proximal tibia in immature rabbits. Metaphyseal vessels grew across the gap with little delay; the site of separation then came to lie in the metaphysis and was bridged by endochondral ossification. Union was achieved within two days in all rabbits. Progression of endochondral ossification repaired the separated physis, thus showing 'primary healing of physeal separation'. This depends on accurate reduction and stable fixation to allow the survival of vessels across the

Evolution of slipped capital femoral epiphysis after in situ screw fixation at a mean 11 years' follow-up: a 222 case series.

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) can lead to hip impingement, more or less rapidly depending on initial slippage severity and on surgical technique. Various surgical options are applicable, including in situ fixation (ISF). The aim of the present study was to look for long-term signs of radiological impingement in hips treated for SCFE by IFS, in order to identify a slip threshold beyond which impingement more regularly appears. MATERIAL AND METHODS: A multicenter retrospective study assessed the clinical and radiological evolution of patients operated on by ISF for SCFE
