Facial nerve management in patients with malignant skull base tumors.

INTRODUCTION: The course of the facial nerve through the cerebellopontine angle, temporal bone, and parotid gland puts the nerve at risk in cases of malignancy. In contrast to Bell's palsy, which presents with acute facial paralysis, malignancies cause gradual or fluctuating weakness. METHODS: We review malignancies affecting the facial nerve, including those involving the temporal bone, parotid gland, and cerebellopontine angle, in addition to metastatic disease. Intraoperative management of the facial nerve and long term management of facial palsy are reviewed. RESULTS: Intraoperative mana

Re-animation and rehabilitation of the paralyzed face in head and neck cancer patients.

Facial nerve paralysis can occasionally result from the treatment of head and neck cancer. The treatment of paralysis is patient specific, and requires an assessment of the remaining nerve segments, musculature, functional deficits, anticipated recovery, and patient factors. When feasible, reinnervation of the remaining musculature can provide the most natural outcome. However, the complex and topographic nature of facial innervation often prevents complete and meaningful movement. In these instances, a wide variety of procedures can be used to combat the functional and cosmetic sequella of fa

Rare instance of gingival enlargement in Klippel-Trenaunay syndrome: a case report.

Klippel-Trenaunay Syndrome is a rare congenital malformation that may include port-wine stain, soft tissue and bony hypertrophy, and venous malformations and lymphatic abnormalities. Although it usually involves the limbs, it may also rarely involve the head, neck, and orofacial regions. Despite its rarity, Klippel-Trenaunay Syndrome should be considered in the differential diagnosis of gingival enlargement. The condition can be easily recognized clinically, but further investigations including imaging studies have to be carried out in order to better understand the nature of the lesion. This

Pulsating exophthalmos associated with neurofibromatosis.

A patient with pulsating exophthalmos is presented. He had facial asymmetry, multiple cutaneous café-au-lait spots and a subcutaneous nodule, biopsy of which showed features consistent with neurofibromatosis. Radiographs of the orbit showed a deficiency of the superolateral orbital wall, a feature previously recognized as part of neurofibromatosis. This feature probably occurs as a result of mesodermal dysplasia. The differential diagnosis and management are discussed.

Conventional tomography and computed tomography-are both methods necessary for evaluation of facial lesions?

108 patients with verified traumatic, inflammatory and neoplastic lesions in the facial region and the pharyngeal space were evaluated both with conventional tomography and CT. Subsequently an analysis was made as to whether both methods were complementary or at least partially replaceable by each other. In traumatic lesions transverse axial CT combined with frontal tomography yielded optimal information. For the diagnosis of inflammatory and neoplastic lesions CT was superior to tomography with the exception of lateral tomograms which contributed additional information to diseases of the sphe

Treatment of facial vascular lesions with intense pulsed light.

BACKGROUND: Various lasers, particularly the flashlamp-pulsed dye laser, have been proven to be effective in the treatment of facial vascular lesions. Nevertheless, the post-treatment side effects, such as pronounced purpura and changes in pigmentation, have been a matter of concern to patients. OBJECTIVE: To test the efficacy of an alternative treatment option that uses intense pulsed light to provide patients with a more tolerable post-treatment outcome. METHODS: A total of 200 patients were treated with an intense pulsed light source (PhotoDerm VL) using various treatment parameters. The

Facial paralysis due to benign parotid tumors.

On rare occasions, facial paralysis associated with a parotid tumor need not denote malignancy. We present two cases in which, contrary to appropriate conventional wisdom, facial paralysis resulted from benign mixed tumors. Each patient presented over 8 years following primary surgical excision. In neither patient was a mass palpable, and facial paralysis was the sole sign of recurrent disease. Each patient had been followed up for several months with a presumptive diagnosis of Bell's palsy prior to discovery of recurrent tumor by radiologic imaging. In each case, at operation the tumor was fo

Maxillofacial deformities in neurofibromatosis.

Four patients with neurofibromatosis are described: all displayed the feature of an increased depth of the sigmoid notch probably due to an expansive growing neurofibroma of the inferior alveolar nerve. One patient had a local neurofibroma with an invasive growth pattern and only this patient shows hyperplasia of the bone. The suggestion is made, contrary to what is found in the literature, that only the invasive growth type gives rise to hyperplasia and this type is always concomitant with a localized form of neurofibromatosis.

Facial myokymia in the course of a pontine tumor.

A patient with facial myokymia, suffering from an infiltrating grade II astrocytoma originating within the pons with vegetations invading the cerebellopontine angle, was studied clinically and electromyographically from 1973 to 1979. The myokymias started in the muscles of the right side of the face and later spread to the left side. The latter point is worth noting because the myokymias of the left side were detectable by EMG but not by inspection. They preceded clinical and instrumental evidence of a contralateral spread of tumoral damage to the brainstem. From the physiopathogenetic angle t

[Retrograde perineural extension of a metastatic recurrence of a frontal skin tumor].

Perineural metastatic spread of malignant tumours has been described for a long time, but has only recently been demonstrated by medical imaging. MRI, guided by the clinical findings, with fine sections on T1-weighted sequences without and with gadolinium, allows the diagnosis of this type of spread. The tumours most frequently responsible are squamous cell carcinomas and cylindromas. The trigeminal and facial nerves are the most frequently affected. Recognition of such lesions is essential in the initial staging of a malignant tumour of the face, as it alters the therapeutic strategy.

Navigating the void: outcomes and adaptations during parotid surgery in the absence of posterior belly of digastric muscle.

It is well known that the digastric posterior belly is one of the essential landmarks for facial nerve identification during parotid surgery. While there were multiple reports about variations of the digastric anterior belly, only a few anatomical variations of the posterior belly of the digastric muscle have been described. In this article, we describe an anatomical variation of the posterior belly of digastric muscle found during superficial parotidectomy of a patient with pleomorphic adenoma. This anatomical variation also led to an anatomical variation in the position of the facial nerve. To

[Clinical retrospective analysis of 19 cases of intraparotid facial nerve schwannoma].

PURPOSE: With literatures review, this retrospective study was aimed to analyze the clinic characteristics and treatment of intraparotid facial nerve schwannoma (IFNS) treated in our department. METHODS: All cases treated in the Department of Oral and Maxillofacial Surgery and diagnosed as IFNS pathologically, from 2000-2007, were reviewed. The data of clinical symptoms, preoperative radio graphic imagings, preoperative diagnosis, tumor size and treatment method were collected and analyzed. RESULTS: Totally, 19 cases of IFNS were collected and reviewed. 3 of 19 (15.8%) were diagnosed as IFNS

Pleomorphic adenoma involving the stylomastoid foramen.

A rare case of an intratemporal pleomorphic adenoma is presented and the management of such a tumour is discussed. Some anatomical aspects of the facial nerve, pertinent to the pathophysiology of facial paralysis are outlined. This case demonstrates that tumour extension into the temporal bone can be resected successfully at initial surgery with excellent facial nerve functional outcome. We advocate exploration of the fallopian canal to be carried out at primary surgery and be performed by a surgeon familiar with the surgical anatomy of the intratemporal segment of the facial nerve. This appro
