Binocular and Fellow Eye Acuity Deficits in Amblyopia: Impact of Fixation Instability and Sensory Factors.

Amblyopia, a neurodevelopmental disorder, is commonly assessed through amblyopic eye visual acuity (VA) deficits, but recent studies also highlight abnormalities in the fellow eye. This study quantified binocular and fellow/dominant eye VA in individuals with amblyopia and strabismus without amblyopia and examined factors influencing these measures, including fixation eye movement (FEM) abnormalities. Identifying which subsets of patients-such as those with nystagmus, concurrent strabismus, or greater fixation instability-exhibit more pronounced deficits in binocular visual acuity and binocula

Pulsed Electromagnetic Fields for Cervical Spine Fusion in Patients with Risk Factors for Pseudarthrosis.

BACKGROUND: Certain demographics and/or risk factors contribute to complications following cervical spinal surgery including pseudarthrosis, prolonged pain, and reduced quality of life (QoL). Pulsed electromagnetic field (PEMF) stimulation is a non-invasive therapy that may enhance fusion success in at-risk patients. OBJECTIVE: To evaluate the safety and efficacy of post-operative adjunctive PEMF therapy following cervical spinal surgery in subjects at risk for pseudarthrosis. METHODS: This prospective, multicenter study investigated PEMF as an adjunctive therapy to cervical spinal fusion pr

Sudden cardiac death in patients with myocarditis: Evaluation, risk stratification, and management.

Myocarditis is a major cause of sudden cardiac death (SCD) and dilated cardiomyopathy (DCM) in young adults. Cardiac magnetic resonance is the established tool for the diagnosis of myocarditis, and late gadolinium enhancement detected on cardiac magnetic resonance imaging is the strongest independent predictor of SCD, all-cause mortality, and cardiac mortality. Several other factors have been associated with SCD or cardiac transplantation including New York Heart Association functional class III/IV, reduced left ventricular ejection fraction <35%, and right ventricular ejection fraction ≤45%.

Frequency and Predictors of Ultrasound-Detected Extracranial Vertebral Artery Stenosis in a Sample of Non-Western Population.

OBJECTIVE: Extracranial vertebral artery (VA) stenosis represents an important aetiology of ischaemic vascular events in the posterior circulation. Nevertheless, it remains insufficiently investigated. Ethnic differences might be related to VA stenosis. In this regard, Egyptian studies are scarce. We aimed to investigate the frequency and predictors of extracranial VA stenosis in a sample of Egyptian patients. METHODS: We enrolled consecutive patients who were referred to our neurovascular ultrasonography laboratory for routine clinical indications. Extracranial VA stenosis was diagnosed and

Ventricular Arrhythmic Events After Transcatheter Pulmonary Valve Replacement in Adults with Repaired Tetralogy of Fallot.

Arrhythmias are a major cause of morbidity and mortality in repaired Tetralogy of Fallot (rTOF). However, predicting those at risk for life-threatening ventricular arrhythmias (VA) remains difficult. Many centers approach risk assessment at the time of surgical pulmonary valve intervention. Increasing numbers of patients have undergone transcatheter pulmonary valve replacement (TPVR), yet there are no studies evaluating VA in rTOF undergoing TPVR and the approach to risk assessment for these patients. A single center retrospective study was performed. The institutional interventional database

Impact of Risk Factor Combinations on Severe COVID-19 Outcomes Among Outpatients.

INTRODUCTION: Several risk factors for progression to severe COVID-19 have been identified, but whether multiple concomitant risk factors (risk stacking) further increase risk remains unknown. We sought to identify outpatient populations at the highest risk for severe COVID-19 using risk factor stacking. METHODS: We conducted a retrospective cohort study of Veterans Affairs (VA) outpatient adults (≥ 18 years) at high risk for severe COVID-19 with a positive SARS-CoV-2 test or COVID-19 diagnosis between April 1, 2022 and August 21, 2024. Severe COVID-19 was defined as COVID-19-related hospital

Comparison of Vascular Aging Models on the Basis of Different Vascular Indicators for Cardiovascular Events Prediction: A Chinese Community-Based Cohort Study.

BACKGROUND: Vascular aging (VA) is a promising concept for cardiovascular risk assessment, but the optimal vascular indicator for vascular age calculation remains uncertain. This study aimed to compare VA indices on the basis of different vascular indicators for predicting cardiovascular events. METHODS: This prospective study included 3561 participants from a Chinese community-based cohort. Four distinct VA indices including brachial-ankle pulse wave velocity (baPWV), carotid intima-media thickness, carotid plaque, or ankle-brachial index, and conventional risk factors were calculated to pre

Reappraisal of electrocardiographic criteria for localization of idiopathic outflow region ventricular arrhythmias.

BACKGROUND: Electrocardiographic (ECG) criteria have been proposed to localize the site of origin of outflow region ventricular arrhythmias (VAs). Many factors influence the QRS morphology of VAs and may limit the accuracy of these criteria. OBJECTIVE: The purpose of this study was to assess the accuracy of ECG criteria that differentiate right from left outflow region VAs and localize VAs within the aortic sinus of Valsalva (ASV). METHODS: One hundred one patients (mean age 52 ± 16 years; 55 [54%] women) undergoing catheter ablation of right ventricular outflow tract (RVOT) or ASV VAs with

[The value of laparoscopy in the diagnosis and treatment of non-palpable testicular cryptorchism].

Laparoscopy is useful in both the diagnosis and the management of impalpable testes. Intra-abdominal testicles can be removed laparoscopically if atrophic or can be partly devascularized by spermatic vessel clipping if apparently normal. Assessment of testicular revascularization would be desirable prior to subsequent orchidopexy. A second-stage vasal-based orchidopexy can then be performed once adequate testicular reperfusion via the deferential pedicle is believed to have occurred. We have used both diagnostic and therapeutic laparoscopy in the management of 103 non-palpable testes over a pe

[Mutation in the cystic fibrosis transmembrane-regulator gene in bilateral congenital ductus deferens aplasia].

A 28-year-old man and his 27-year-old wife were investigated for infertility of 3 1/2 years' duration. There was azoospermia caused by bilateral aplasia of the vas deferens, and therefore it was planned to aspirate spermatozoa from the epididymis for the purpose of in-vitro fertilisation. As part of the diagnostic workup the man was investigated for those mutations of the cystic fibrosis transmembrane regulator (CFTR) gene which occur with undue frequency in association with aplasia of the vas deferens. Deoxyribonucleic acid (DNA) analysis revealed a typical three base deletion (delta F 508).

Role of ECMO and percutaneous mechanical thrombectomy in the management of acute pulmonary embolism.

Interventional treatment for hemodynamically unstable pulmonary embolism (PE) is now common due to the fact that unstable PE carries a significant 30-day mortality. Unstable PE is characterized by acute right ventricular failure, hypotension, and hypoxia, leading to cardiovascular collapse and cardiac arrest. Implementation of veno-arterial extracorporeal membrane oxygenation (VA-ECMO) acutely provides oxygenation support to improve hypoxia and offload the right ventricular (RV) pressure in the belief that rapid reduction of hypoxia and right ventricular (RV) pressure will provide a window for

Usefulness of the CHA2DS2-VASc Score to Predict the Risk of Sudden Cardiac Death and Ventricular Arrhythmias in Patients With Atrial Fibrillation.

Sudden cardiac death (SCD), the most devastating manifestation of ventricular arrhythmias (VAs), is the leading cause of mortality in patients with atrial fibrillation (AF). We hypothesized that the CHA2DS2-VASc score, consisting of age and several clinical risk factors, could be used to estimate the individual risk of SCD/VAs for AF patients. From year 2000 to 2011, 288,181 newly-diagnosed AF patients without antecedent SCD/VAs were identified from "Taiwan National Health Insurance Research Database." During the follow-up of 1,065,751 person-years, 11,166 patients experienced SCD/VAs with an

Predictors of long-term outcome in an elderly group after laminectomy for lumbar stenosis.

OBJECT: This is a retrospective long-term outcome study of results after laminectomy for lumbar spinal stenosis in an elderly group of patients. The study was designed to evaluate possible demographic, comorbidity, and clinical prognosticators for pain reduction and functional improvement in this population. Because the assessment of functional outcome in the elderly is complicated by several specific factors, the use of outcome measurement parameters should be revised and refined. Moreover, despite numerous relevant studies, the results of various techniques remain equivocal, particularly amo
