Vasculo-Behcet's disease mimicking a metastatic neck mass.

This article presents a case of Behcet's Disease (BD) with vascular involvement of the neck, which mimicks a metastatic neck mass in the initial presentation. A 58-year-old man presented with dysphagia, weight loss, bulging on the lateral wall of the left pyriform sinus, and a firm and fixated neck mass suggestive of metastasis. Computed tomography of the neck demonstrated a solid mass, around the bifurcation of the carotid artery together with a pseudoaneurysm of the left external carotid artery. The mass was about four centimeters in diameter and extended to hypopharynx medially. Biopsy from

Resection arthrodesis of the knee in the treatment of tumours--a long-term follow-up.

Limb salvage operations using large custom-made prostheses have become the treatment of choice for juxta-articular tumours at the knee. Resection-arthrodesis has been used since the 1920s and offers a satisfactory long term solution for young active patients. The good functional results after this procedure in 9 patients is presented here. This method should still be considered in selected cases.

[On the method of hyperthermic chemotherapy by regional isolated perfusion for tumors of lower extremities].

From August 1990 to June 1993, 30 patients with osteosarcoma of lower extremities were treated with chemotherapy by hyperthermic regional isolated perfusion. There were 19 male and 11 female with a mean age of 21 (15-28) years. All of the tumors were located in the lower limbs: 20 on the femora, 9 on the tibiae and 1 on the fibula. Chemotherapy was going on for 60 minutes during hyperthermic regional isolated perfusion. Temperature was kept at 42 degrees C in deep soft tissue around the tumor during perfusion. The results showed that local edema of the limbs were reduced observably, tumors wer

Epiphyseal involvement in osteosarcoma.

Plain radiography and magnetic resonance (MR) imaging were used to assess the extent of transphyseal involvement in 15 consecutive patients with long bone osteosarcoma and nonfused epiphyses. The findings were correlated with those from surgical and microscopic pathologic examinations. There were no cases of false-positive findings with either MR imaging or plain radiography. Conventional radiography accurately helped predict transphyseal spread in only nine of 15 cases (60%). Spread to the epiphysis was present in 12 of the 15 cases (80%) and was accurately predicted with MR imaging in all 12

[Clinical features of pathologically confirmed metastatic bone tumors--a report of 390 cases].

BACKGROUND & OBJECTIVE: With the development of diagnostic techniques of imaging and pathology, early diagnosis of metastatic bone tumors has been greatly improved, but the clinical characteristics which are essential for diagnosis are rarely reported. In this article, the clinical features of pathologically confirmed metastatic bone tumors were analyzed for further improvement of early diagnosis and treatment. METHODS: Clinical data of 390 patients with pathologically confirmed metastatic bone tumors, treated from 1980 to 2003 at The First Affiliated Hospital of Sun Yat-sen University, were

Characterization of the Posterior Femoral Cutaneous Nerve and Its Clinical Application for Autologous Breast Reconstruction.

Autologous breast reconstruction (ABR) uses a harvested tissue flap from the abdomen, posterior thigh, or buttocks to rebuild the breast postmastectomy. Identification of nerves for use in autologous sensate breast reconstruction flaps is an important surgical consideration as loss of breast sensation is a common risk of ABR. The posterior femoral cutaneous nerve (PFCN) and its branches supply sensory innervation to skin of the posterior thigh, leg, perineum, and buttocks, creating a feasible candidate for sensate profunda artery perforator (PAP) flaps for reestablishing breast sensation throu

Femoral metastatic pathological fractures, impending and actual fractures - A patient survival study.

INTRODUCTION: The skeleton is a common site for metastases. Prostate, breast, lung, renal and thyroid carcinomas account for 80 % of the original cancers, with the femur being the most affected long bone. With improved oncological treatments, prolonged patient survival leads to an increased prevalence of osseous metastases. This study examines the impact of preventive surgery for impending femoral pathological fracture (IFF), versus treatment of pathological femur fracture (PFF) on patient mortality and morbidity. METHODS: Retrospective cohort of 174 patients undergoing surgery due to femoral

Risk Factors Affecting Complications of Access Site in Vascular Intervention through Common Femoral Artery.

BACKGROUNDS: Traditionally, vascular interventions have been performed through the femoral artery. AIMS: The purpose of this study was to evaluate risk factors affecting access-site complications in patients with hepatocellular carcinoma or peripheral arterial disease in lower extremity who underwent vascular intervention by accessing the common femoral artery (CFA). PATIENTS AND METHODS: From December 2015 to November 2018, 287 patients underwent transarterial chemoembolization (TACE) or peripheral vascular intervention with ultrasound (US)-guided CFA access. Standard 18-gauge (G) access wa

Femoral fracture in primary soft-tissue sarcoma of the thigh treated with radiation therapy: indications for prophylactic intramedullary nail.

INTRODUCTION: Post-radiation fractures of the femur (PRF) are difficult to treat and are associated with a high risk of delayed union and non-union. We report a series of patients affected by soft tissue sarcoma (STS) of the thigh, treated with limb-sparing surgery and perioperative radiotherapy (RT), to analyse post-radiotherapy femur fracture (FF) rate and its management. MATERIAL AND METHODS: 547 patients treated with surgery and RT for a deep primary STS of the thigh were included. "Periosteal stripping" and "bone tangential resection" were performed in case of tumor invasion. In the case

Thermal damage of the genitofemoral nerve due to radiofrequency ablation of renal cell carcinoma: a potentially avoidable complication.

OBJECTIVE: A technique is presented that may prevent damage to the genitofemoral nerve during percutaneous radiofrequency ablation of renal cell carcinomas. The genitofemoral nerve originates from the upper part of the lumbar plexus and descends laterally along the psoas major muscle, which is anatomically close to the kidney. During radiofrequency ablation, nearby healthy nerve tissue could be damaged by heat conductance. In the described technique, the radiofrequency applicator within the renal tumor is used as a lever to displace the kidney away from the psoas muscle. CONCLUSION: Three cli

Rotationplasty of the lower limb for childhood osteosarcoma of the femur.

BACKGROUND: Surgical management of osteosarcoma of the limb in childhood often involves extensive surgery. Intervention ranges from local resection to amputation depending on the level and the extent of the disease process. However, where the disease is extensive the level of amputation may preclude effective prosthetic use. In these circumstances resection of the diseased segment of the limb, i.e. an intercalary amputation and reconstruction with rotation of the tibial component of the limb can provide function equivalent to a below-knee amputee. METHODS: To examine the role of rotationplast

Case report 650: Grade II chondrosarcoma of the proximal end of the right femur.

A case is presented in which the conventional radiograph was suggestive of avascular necrosis. This impression remained unchanged inspite of an MRI examination that was more suggestive or replacement of marrow. The diagnosis of ischemic necrosis was strongly supported by the patient having previously established avascular necrosis in the opposite hip. The gross pathologic specimen showed only necrotic tumor, which mimicked avascular necrosis on the plain film. The importance of placing proper confidence in the MRI findings is thus emphasized. A classification and brief description of the vario

[Osteoid osteoma of the femur neck with synovitis simulating chronic monoarthritis of the hip].

The authors report a case of subperiosteal osteoid osteoma of the femoral neck in a 14 year-old girl presenting as chronic hip monoarthritis with narrowed coxofemoral joint space and non specific histological synovitis. They analyse the synovitis which is associated with some osteoid osteomas (hip, elbow, knee, ankle bones) emphasizing the "pseudo-arthritis of the hip" picture and insist that all cases of unexplained chronic monoarthritis of the hip should suggest subperiosteal osteoid osteoma of the neck.

[Function-preserving interventions in metastatic fractures].

A report on combined osteosynthesis (plate and bone cement) in 11 metastatic fractures of arms and legs is presented. Fractures in the aged and high risk patients with osteoporosis are other indications for this method. Normal use of the affected limb and mobilization are possible one day after operation. Already existing visceral metastases are no contraindication.

[Femoral artery pseudoaneurysms encountered in orthopedics and traumatology].

BACKGROUND AND PURPOSE: Most published articles regarding orthopedic- and trauma-related femoral artery pseudoaneurysms (FAPs) are case reports in English. Reported cases are often associated with a literature review but actually provide little robust data. We wanted to summarize the current knowledge on diagnostic and therapeutic features of these FAPs. METHODS: A new case of superficial FAP is described followed by a review of the literature. A bibliographic search was performed online (PubMed, ScinceDirect) from 1964 to 2015 using the descriptors "traumatic femoral pseudoaneurysm, orthoped

The incidence of acute cardiorespiratory and vascular dysfunction following intramedullary nail fixation of femoral metastasis.

Intramedullary nail fixation is a common treatment for metastatic tumors of the femur with overt or impending femoral fracture. This procedure sometimes causes severe cardiorespiratory and vascular dysfunction. The clinical relevance of this is not dear. We reviewed 45 operations in 43 patients, where intramedullary nail fixation was used to treat metastatic femoral fractures and impending fractures. We studied the incidence of intraoperative oxygen desaturation and hypotension associated with intramedullary manipulation as markers of cardiorespiratory and vascular dysfunction. Acute oxygen de
