Developmental susceptibility of neurons to transient tetrahydrobiopterin insufficiency and antenatal hypoxia-ischemia in fetal rabbits.

Tetrahydrobiopterin (BH4) is important for normal brain development as congenital BH4 deficiencies manifest movement disorders at various childhood ages. BH4 transitions from very low levels in fetal brains to higher "adult" levels postnatally, with the highest levels in the thalamus. Maternal supplementation with the BH4 precursor sepiapterin reduces postnatal motor deficits and perinatal deaths after 40-min fetal hypoxia-ischemia (HI) at 70% gestation, suggesting that brain BH4 is important in improving function after HI. We tested the hypothesis that the intrinsically low concentrations of

The correlation in antepartum fetal test between full fetal biophysical profile (FBP) and rapid biophysical profile (rBPP).

OBJECTIVE: To determine the correlation between the rapid biophysical profile (rBPP), the combination of amniotic fluid index (AFI), and sound-provoked fetal movement (SPFM) detected by ultrasound, and the full biophysical profile (FBP) in terms of abnormal and normal result. MATERIAL AND METHOD: A prospective study was performed in 200 singleton pregnancies with no fetal anomalies between 30-42 weeks of gestation indicated for non-stress test (NST). All participants received both the standard (FBP) and the new rBPP examinations. Abnormal fetal test was defined as having a score of < or = 6 f…

The effect of mechanical forces on in utero lung growth in congenital diaphragmatic hernia.

Lung development is a complex process of lung growth and maturation. Although lung maturation appears to be principally regulated by hormonal and biochemical factors, lung growth appears to depend more on physical factors such as lung liquid volume, fetal breathing movements, the size of the intrathoracic space, and the amniotic fluid volume. This article focuses on these physical factors. Specific emphasis is given to the normal mechanisms responsible for lung growth, as well as the consequences of disruption of these normal mechanisms.

Prenatal diagnosis of Pena-Shokeir syndrome phenotype by ultrasonography and MR imaging.

Pena-Shokeir syndrome phenotype is characterized by neurogenic arthrogryposis, facial anomalies, polyhydramnios and lung hypoplasia. Prenatal US is crucial in showing Pena-Shokeir syndrome phenotype in addition to demonstrating reduced fetal movements or akinesia as an underlying aetiological factor as early as the 14th week of gestation. Several reports of prenatal diagnosis of Pena-Shokeir syndrome phenotype by US have been published. In this report, MRI findings providing prenatal diagnosis are presented.

The kick chart in high-risk pregnancies: a two-year experience in Zimbabwe.

A kick chart has been recorded by 200 high-risk pregnancies during their antepartum hospitalization. Except for 19 mothers who presented with an unreliable count, this study suggests a good correlation between an abnormal kick chart and an increased risk of intrauterine death. The obstetrical outcome was much better among the patients with a normal kick chart compared to those with an abnormal count. This easy, cheap, low-technology test could be useful in developing countries when monitoring of fetal well-being is required.

Cardiotocographic and Doppler ultrasonographic findings in a fetus with brain death syndrome.

OBJECTIVE: The diagnosis of brain death syndrome by cardiotocography (CTG) and Doppler ultrasonography (US) is reported in a fetus at 35 weeks of gestation. CASE REPORT: A 23-year-old, gravida 2, para 0, woman was referred to our hospital because of the absence of fetal movements. CTG showed fixed fetal heart rate (FHR) pattern. A detailed Doppler US examination of the fetus showed extensive cystic lesions of both cerebral hemispheres, polyhydramnios, total absence of neuromuscular parameters of biophysical profile (BPP) and the cessation of cerebral blood flow. Umbilical cord artery blood ga…

The natural history of fetal long QT syndrome.

INTRODUCTION: Fetal magnetocardiography (fMCG), the magnetic analog of ECG, has provided invaluable insight into the mechanisms of fetal arr … red and compared to published normals. We correlated fetal heart rate (FHR) patterns and effects of fetal movement on FHR and rhythm using actocardiography. RESULTS: Thirty-nine fetuses were studied at a mean of 28 (19-38) weeks of gestation. All had structurally normal hearts. One was on amiodarone for suspected supraventricular tachycardia and hydrops. Five h …

A case of dyskinetic cerebral palsy resembling post-anoxic action myoclonus.

Myoclonus after brain anoxia is more commonly observed in adults. Perinatal anoxia, however, on rare occasions causes myoclonus as the main neurologic abnormality. In this paper we described a case of cerebral palsy complicated by chorea and tremulous movements of the foot in which there were no risk factors except perinatal asphyxia. The patient had unique symptoms, action myoclonus associated with the movements of hyperkinésie volitionnelle. This case illustrated two points. One is that abnormal involuntary movements attributed to perinatal brain insults had changed as the brain matured. The

A monitored study of major physical activities in the perinatal calf.

Using doppler ultrasound and manual palpation per rectum, 10 heifers in late gestation were examined twice daily to determine fetal activity, posture and position. Three heifers calved early but seven were monitored over their final week. After birth the calves were watched to estimate their vitality. Much variation in activity occurred but there was a significant (P = 0.012) reduction 11 to 23 hours before birth. After bouts of activity an 'alerted' posture was struck with the head raised and forelimbs normally extended; pronation and engagement then occurred. These fetal movements were not n…

The effect of magnesium sulfate on the biophysical profile of normal term fetuses.

Forty term pregnant women with singleton breech gestations admitted for external cephalic version underwent biophysical profile testing before any fluid infusion or medication. After magnesium sulfate had been infused for contraction prophylaxis, the maternal serum magnesium level was measured and a second biophysical profile was performed. The mean (+/- SD) serum magnesium was 5.1 +/- 1.0 mg/dL. The biophysical profile score decreased significantly, reflecting a decrease in fetal breathing activity. In patients with therapeutic maternal serum magnesium levels, loss of any component of the bio

Pregnancy-Induced Thrombotic Thrombocytopenic Purpura Complicated by Atypical Posterior Reversible Encephalopathy Syndrome and Acute Pancreatitis: A Rare Presentation.

Pregnancy is a documented trigger for thrombotic thrombocytopenic purpura (TTP), a rapidly progressing condition that is often misdiagnosed as HELLP (hemolysis, elevated liver enzymes, low platelets) syndrome. Here, we present the case of a 28-year-old pregnant female, gravida 2 para 1, who was admitted for complaints of seizures, vomiting, fever, decreased perception of fetal movements, and visual impairment. She was found to have posterior reversible encephalopathy syndrome, acute kidney injury, and acute pancreatitis, along with her diagnosis of TTP. She improved with plasma exchange and re…

The neural and vascular effects of killed Su-Streptococcus pyogenes (OK-432) in preterm fetal sheep.

Fetal exposure to inflammatory mediators is associated with a greater risk of brain injury and may cause endothelial dysfunction; however, nearly all the evidence is derived from gram-negative bacteria. Intrapleural injections of OK-432, a killed Su-strain of Streptococcus pyogenes, has been used to treat fetal chylothorax. In this study, we evaluated the neural and cardiovascular effects of OK-432 in preterm fetal sheep (104 +/- 1 days, term 147 days). OK-432 (0.1 mg, n = 6) or saline vehicle (n = 7) was infused in the fetal pleura, and fetuses were monitored for 7 days. Blood samples were ta

Intraplacental choriocarcinoma in a term pregnancy with massive fetomaternal hemorrhage: a case report.

BACKGROUND: Intraplacental choriocarcinoma (IC) is a rare type of gestational choriocarcinoma that occurs in the placenta, with only a small number of cases reported. The symptoms of IC are nonspecific, and the diagnosis is often incidental, typically made during routine placental pathological examination. CASE PRESENTATION: We report a case of IC in a 30-year-old Chinese woman presenting with intrauterine fetal distress and fetomaternal hemorrhage. The patient was admitted to our hospital at 40 weeks of gestation due to decreased fetal movements. An emergency cesarean section was performed, …

Placental chorioangioma associated with polyhydramnios and hydrops fetalis.

A 27-year-old multigravida woman was noted on routine growth scan at 27 weeks gestation to have a central placental hypoechoic area measuring 6.7×6.0×4.4 cm. A subsequent magnetic resonance scan confirmed a solid mass in the placenta lying anteriorly; therefore, a preliminary diagnosis of giant placental chorioangioma was made. A repeat ultrasound scan at 30 weeks gestation indicated that the mass had increased, with the presence of polyhydramnios. The patient experienced reduced fetal movements at 31 weeks gestation. There was persistent fetal tachycardia at 33 weeks gestation, and consequent…

Complete hydatidiform mole and live fetus in a singleton pregnancy with confined placental mosaicism and fetomaternal hemorrhage: a case report.

BACKGROUND: Coexistence of complete mole and a live fetus is uncommon (1:22,000-100,000), more so with euploidy. CASE: We present a case of a molar pregnancy with a euploid fetus who had close fetal evaluation for second trimester bleeding. The patient presented at 29 weeks' pregnancy with decreased fetal movements, a result of fetomaternal hemorrhage. She underwent cesarean section and delivered a live infant. By close follow-up and a multidisciplinary approach, the appropriate diagnosis and a favorable outcome were achieved. Both mother and the child at 5 years of age are doing well. CONCL…
