Subacute encephalopathy with seizures in alcoholics (SESA) syndrome: a subtype of focal non-convulsive status epilepticus.

Focal status epilepticus is often associated with intracranial infections, neoplasms or vascular aetiologies. Subacute encephalopathy with seizures in alcoholics (SESA) syndrome is a clinically under-recognised disorder characterised by encephalopathy in the context of seizure and specific focal EEG abnormalities. Our case involved a male in his 50s with a history of alcohol use disorder, who presented with left-sided twitching and encephalopathy, found to be in a focal status. An EEG revealed abnormal discharges in the right frontoparietal region. MRI of the brain displayed signal changes in

Role of sexually-transmitted infections in the structural and functional reorganization of the prostate.

Intracellular reorganization of secretory epitheliocytes in the main, intermediate, and periurethral prostatic glands was studied in chronic prostatitis under conditions of sexually-transmitted infections. The destructive and autophagic processes in the secretory epitheliocytes were stimulated by persistence of microorganisms, Mycoplasmataceae (mainly mycoplasmas and ureaplasmas) and Chlamydia trachomatis, in the prostatic terminal compartments, epithelial layer, and epitheliocytes. Significant intracellular reorganization of smooth-muscle cells was found: focal destruction of ultrastructures

Focal aberrations indicate EYA2 and hsa-miR-375 as oncogene and tumor suppressor in cervical carcinogenesis.

Cervical cancer results from persistent infection with high-risk human papillomavirus (hrHPV). Common genetic aberrations in cervical (pre)cancers encompass large genomic regions with numerous genes, hampering identification of driver genes. This study aimed to identify genes functionally involved in HPV-mediated transformation by analysis of focal aberrations (<3 Mb) in high-grade cervical intraepithelial neoplasia (hgCIN). Focal chromosomal aberrations were determined in high-resolution array comparative genomic hybridization data of 60 hgCIN. Genes located within focal aberrations were vali

Prevention and treatment strategy in pregnant women with group B streptococcal infection.

Group B streptococcus (GBS; Streptococcus agalactiae) are encapsulated gram-positive cocci belonging to Lancefield group B, that frequently … pneumonia (2%), puerperal sepsis (2%), and bacteremia without a focal site (31%). It also can cause focal infections such as pneumonia, meningitis, and endocarditis, albeit rarely. Invasive maternal infection with GBS is associated with pregnancy loss and preterm delivery. Prior to the widespread use of maternal intrapartum chemoprophylaxis, maternal colonization …

Successful colonoscopic approach in a child with intussusception associated with enterohemorrhagic Escherichia coli O157 infection.

The pathogenesis of intussusception caused by enterohemorrhagic Escherichia coli (E. coli) O157 infection is unknown. In our case, colonoscopy was useful for confirming O157 infection. The intussusception was caused by focally damaged edematous mucosa in the cecum. This case helped in elucidating the pathogenesis of the disease.

[Computed tomographic semiotics of respiratory tuberculosis in HIV-infected patients].

OBJECTIVE: to study the computed tomographic (CT) semiotics of respiratory tuberculosis in HIV-infected patients in relation to the degree of immunosuppression. SUBJECT AND METHODS: The study enrolled 74 patients with verified respiratory tuberculosis in the presence of HIV infection. According to the degree of immunosuppression and the Centers for Disease Control (CDC) and Prevention classification (Atlanta, USA, 1993), the patients were divided into 3 groups: (1) CD4 > or = 500 cells/microl (n = 10); 2) CD4 200-499 cells/microl (n = 28); (3) CD4 <200 cells/microl (n = 36). RESULTS: With sp

Multiple intracranial space-occupying lesions in a renal transplant recipient from an area endemic for tuberculosis (TB): TB vs. toxoplasmosis.

Renal transplant recipients may present with intracranial space-occupying lesions (SOLs) due to infections as well as a post-transplant lymphoproliferative disorder (PTLD). Here, we discuss a renal transplant recipient who presented with neurologic symptoms and magnetic resonance imaging (MRI) of the brain showed multiple focal SOLs. Tuberculosis (TB), toxoplasmosis, nocardiosis, fungal infections, and PTLD were considered in the differential diagnosis. MRI spectroscopy was suggestive of an infectious cause, such as toxoplasmosis or TB. Serologic tests using Toxoplasma were negative. A brain b

Enterococcus faecalis in blood cultures-a prospective study on the role of persistent bacteremia.

Enterococcus faecalis can cause infective endocarditis and other complicated infections. We prospectively investigate the incidence of persistent bacteremia with E. faecalis. Of 50 episodes with monomicrobial E. faecalis bacteremia the control blood culture after 48 to 72 hours was positive in 5 episodes (10%) of which 4 had a complicated focal infection.

No Such Thing as Chronic Q Fever.

Modern diagnostic methods enable clinicians to look beyond a diagnosis of chronic Q fever and discern whether patients instead have persistent focalized Coxiella burnetii infection(s). Use of these methods and development of criteria to define and treat such infections, especially cardiovascular infections, will improve the prognosis for patients previously thought to have chronic Q fever.

[The spatial-temporal structure of fluctuations in tick-borne encephalitis morbidity in the Maritime Territory].

The annual dynamics of the epidemic process in tick-borne encephalitis and its spread in the Maritime Territory, endemic for this infection, have been studied. The study has shown that in the central mountainous regions grown with boreal forest, in contrast to the rest of this focal area, a higher morbidity level and more severe outcomes of this infection are observed. This indicates that in those regions more ancient nuclei of the endemic area of this infection with the main stable elements of the natural focus are preserved.

Infected subfural hematoma: three case reports involving gram-negative organisms.

Three case reports of infected subdural hematoma are presented, two with Salmonellae and one with Escherichia. Infection of such hematomas most often occurs during bacteremia, and the area of infection is limited by the existing hematoma membrane. Clincally, fever, headache, nuchal rigidity, and focal neurological signs, especially in a patient with previous head trauma, suggest subdural infection. Treatment consists of drainage and systemic antibiotics. These case reports demonstrate the clinical features of this rarely reported entity, and especially illustrate the need for careful bacteriol

Treatment and Diagnostic Challenges in a Patient with Atypical SARS-CoV-2-Associated Encephalitis Mimicking a Neoplasm: A Case Report.

Background and Clinical Significance: Encephalitis is a rare neurological complication associated with Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection. In rare cases, focal neuroinflammation can manifest as a mass-like parenchymal lesion, creating profound diagnostic and treatment dilemmas by mimicking primary central nervous system neoplasms. Case Presentation: A 34-year-old female presented with cephalalgia, nausea, confusion, facial palsy, and a new onset of focal impaired awareness seizures (FIAS). Brain magnetic resonance imaging (MRI) revealed a prominent hyperinte

Cerebral venous sinus thrombosis in severe malaria.

Cerebral venous sinus thrombosis has been reported to be associated with various systemic illnesses and infections, including severe malaria. We report here a 43 year-old Thai male presenting with fever and seizures. He was diagnosed as and treated for severe falciparum malaria. After gaining consciousness he developed focal neurological signs and evidence of increased intracranial pressure. Magnetic resonance imaging (MRI) and magnetic resonance venography (MRV) of the brain revealed a mid-superior sagittal sinus thrombosis with venous infarction. Investigations for other infections and throm

Salmonella septic arthritis in a patient with acute idiopathic thrombocytopenic purpura treated with steroid.

Salmonella has three clinical presentations: self-limiting gastroenteritis, a systemic syndrome (enteric or typhoid fever), and bacteremia with focal infection. Hematogenous infections can cause focal lesions, but unusual manifestations occur more often when predisposing factors such as T cell defect, hemolytic disorders (sickle cell disease, malaria) or trauma are present. Salmonella tend to invade bones and joints. There is no mention of acute idiopathic (immune) thrombocytopenic purpura as a predisposing factor for salmonella septic arthritis; however there are reports about the importance …

How do we manage oral infections in allogeneic stem cell transplantation and other severely immunocompromised patients?

The oral cavity is among the sites in the body most susceptible to infections. In the immunocompetent population these are most frequently localized odontogenic infections caused by bacteria. In severely immunocompromised patients, such as those undergoing allogeneic stem cell transplantation, fungal and viral infections become especially prominent. Infections in this population can present in an unusual fashion, can spread rapidly to other organs in the body, and are more frequently resistant to therapies. This article discusses the current knowledge of the most frequent presentations of infe

Anaerobic infections in children.

Anaerobic bacteria commonly cause infection in children. Anaerobes are the most predominant components of the normal human skin and mucous membrane bacterial flora, and are therefore a common cause of bacterial infections of endogenous origin. Because of their fastidious nature, they are difficult to isolate from infectious sites and are often overlooked. Anaerobic infections can occur in all body sites, including the central nervous system, oral cavity, head and neck, chest, abdomen, pelvis, skin, and soft tissues. Anaerobic bacteria colonize the newborn after delivery and have been recovered
