Interplay between Elevated Bile Acids and Reduced Vitamin D Levels in Fontan Circulation: A Double-Edged Sword.

PURPOSE OF REVIEW: The Fontan procedure, a palliative surgery for single-ventricle congenital heart disease, often leads to multisystem complications. Elevated bile acids and reduced vitamin D3 levels, frequently observed in this population, may share a harmful bidirectional relationship. This review examines their interplay and therapeutic relevance. RECENT FINDINGS: Fontan-associated liver disease and potential gut dysbiosis contribute to altered bile acid metabolism, with elevated circulating secondary bile acids impairing cardiovascular, hepatic, musculoskeletal, and immune functions. Rec…

Biventricular repair versus Fontan completion for patients with d- or l-transposition of the great arteries with ventricular septal defect and left ventricular outflow tract obstruction.

OBJECTIVES: D-transposition of the great arteries and l-transposition of the great arteries with ventricular septal defect and left ventricu … ortality and functional outcomes. Biventricular patients have a greater risk of reintervention. The Fontan procedure is a viable option when anatomic risk factors preclude biventricular repair.

Challenges of univentricular physiology in heterotaxy.

Patients with heterotaxy syndrome exhibit an extensive constellation of congenital cardiac malformations, making these patients a challenging group to manage surgically. Many of these patients' hearts do not lend themselves to separation of the pulmonary and systemic circulations except by some modification of the Fontan procedure. Palliative procedures early in life are directed at creating a satisfactory balance of pulmonary and systemic blood flow and at the same time ensuring unobstructed pulmonary venous return. Early conversion from parallel pulmonary and systemic circulations to superio…

The utility of 4D flow MRI and computational fluid dynamics in developing the fontan model enhances presurgical planning and ensures better outcomes: the first case report in Thailand.

The Fontan procedure is a surgical palliation for patients with single-ventricle congenital heart defects. Optimal hemodynamic planning is required to minimize energy loss and systemic venous pressure. This study integrated four-dimensional (4D) flow magnetic resonance imaging (MRI) and computational fluid dynamics (CFD) to evaluate three Fontan connection configurations in a twelve-year-old patient, aiming to identify the optimal design for presurgical planning. Model 2, which involved an extracardiac conduit anastomosed to the pulmonary artery with the main pulmonary artery stump excised, de…

Results of the modified Fontan operation in adults.

The results of the modified Fontan procedure were examined in 77 patients 18 years of age and older who underwent operation at this institution between October 1973 and December 1986. This series represents 16% of the patients undergoing the modified Fontan operation during that period. Of the 77 patients (46 men and 31 women), 29 had tricuspid atresia, 30 had double-inlet ventricle and 18 had other complex lesions. There were 5 (6%) hospital deaths, compared with 67 (17%) for patients of all ages undergoing the Fontan operation during the same period. Patient age 18 years and older was not a …

Interstage attrition between bidirectional Glenn and Fontan palliation in children with hypoplastic left heart syndrome.

OBJECTIVE: With improving operative mortality for staged palliation of hypoplastic left heart syndrome, interstage death accounts for an increasing proportion of hypoplastic left heart syndrome mortality. We investigated risk factors for death or cardiac transplantation during the interstage period between bidirectional Glenn and Fontan procedures in children with hypoplastic left heart syndrome. METHODS: Patients with hypoplastic left heart syndrome who underwent bidirectional Glenn between August 1995 and June 2007 were screened. Standard risk patients, defined by having been discharged aft…

Type and Frequency of Arrhythmias Status Post Hybrid Procedure at 6 Years of Age.

The hybrid pathway is an alternative surgical route for patients with hypoplastic left heart syndrome (HLHS) and patients with single-ventricle systemic obstructive physiology. Limited data exist on post-Fontan arrhythmias in these patients. This study aimed to assess the type and frequency of arrhythmias post-Fontan in hybrid pathway patients and compare them to the Single Ventricle Reconstruction Trial (SVRT). A retrospective chart review was performed on all patients with single-ventricle anatomy who underwent the hybrid procedure and subsequently completed the Fontan surgery. Exclusion cri

Clinically significant posture-related oxygen saturation variability in Fontan circulation: prevalence and determinants.

BACKGROUND: Patients with Fontan circulation often have low baseline oxygen saturation (SpO2), and posture-related SpO2 variability is somet … in 2 patients. Patients with significant variability were older and had a longer duration since the Fontan procedure. Veno-venous collaterals (VVCs) were more prevalent in these patients, whereas pulmonary arteriovenous malformations (PAVMs) were not more prevalent. Five patients with significant variability had neither VVCs nor PAVMs. In multivariable analysis, V …

Interventional Cardiovascular Magnetic Resonance in Children Undergoing Pre-Operative Fontan Evaluation: One-Stop Shop.

BACKGROUND: Interventional cardiovascular magnetic resonance (iCMR) provides cardiovascular magnetic resonance (CMR) and cath-derived hemodynamic assessment under the same anesthesia. In this study, we describe the role of iCMR in children with single-ventricle lesions before Fontan palliation. METHODS: iCMR-guided right and left heart catheterization were performed using balanced steady-state free precession (bSSFP) or T1 overlay sequences to visualize the gadolinium-filled balloon-tipped catheter in the Phillips Ingenia 1.5 T system scanner. iCMR procedure was defined as successful if all p

Risk Factors For Unfavorable Outcomes After Bidirectional Cavopulmonary Anastomosis.

BACKGROUND: Bidirectional cavopulmonary anastomosis (BCPA) is an important preliminary step toward the Fontan procedure; thus, understanding of risk factors for morbidity and mortality after BCPA may ultimately promote improved rates of success with Fontan completion and general survival. This study evaluated survival and predictors of unfavorable outcomes in patients after BCPA. METHODS: Clinical data of 157 patients who underwent BCPA from 2003 to 2015 at a single center were retrospectively analyzed. RESULTS: Three-year and nine-year survival after BCPA were 87.1% ± 2.8% and 85.8% ± 2.9%,…

Transcatheter Fontan completion: Creation of an extracardiac Fontan.

Patients with functionally univentricular hearts are usually palliated surgically. There have been several reports of successful attempts to complete the Fontan procedure without surgery. The pathways created at the time of the preconditioning were largely reminiscent of the lateral tunnel Fontan. However, this approach is still confidentially limited to a small number of centers. In 2013, we designed a circuit that mimics the actual surgical technique of extracardiac total cavopulmonary connection to allow for transcatheter completion in an animal study. A polytetrafluoroethylene conduit was …

Advances in Diagnostic and Interventional Catheterization in Adults with Fontan Circulation.

Over the past five decades, the Fontan procedure has been developed to improve the life expectancy of patients with congenital heart defects characterized by a functionally single ventricle. The Fontan circulation aims at redirecting systemic venous return to the pulmonary circulation in the absence of an impelling subpulmonary ventricle, which makes this physiology quite fragile and leads to several long-term complications. Despite the importance of hemodynamic assessment through cardiac catheterization in the management and follow-up of these patients, a thorough understanding of the ultimat…

The so-called "one-and-a-half" ventricular repair: where are we after 40 years?

OBJECTIVES: The indications, timing, and results of the so-called "one-and-a-half ventricle repair", as a surgical alternative to the creation of the Fontan circulation, or high-risk biventricular repair, currently remain nebulous. We aimed to clarify these issues. METHODS: We reviewed a total of 201 investigations, assessing selection of candidates, the need for atrial septal fenestration, the fate of an unligated azygos vein and free pulmonary regurgitation, the concerns regarding reverse pulsatile flow in the superior caval vein, the growth potential and function of the subpulmonary ventri

Consequences of right ventricle-to-pulmonary artery shunt at the first stage for the Fontan operation.

BACKGROUND: The evidence of the advantageous physiology associated with right ventricle-to-pulmonary artery (RV-PA) shunt in the early postoperative period after the Norwood procedure for hypoplastic left heart syndrome has been recently widely reported. We investigated the late consequences of this modification from the perspective of the third-stage palliation, the Fontan operation. METHODS: Between September 1995 and November 2006, a consecutive series of 50 children with hypoplastic left heart syndrome from a single institution underwent a fenestrated Fontan operation (lateral tunnel tech
