Basic Ring Fixator Configurations for the Foot and Ankle Surgeon.

Circular external fixators are powerful tools for correcting foot and ankle deformities. We summarize the most frequent indications for its use in an orderly fashion, as well as the basic configurations in the treatment of these conditions, including hexapod fixators. Following Ilizarov's principles and the latest experts' recommendations, most foot and ankle deformities can be properly managed, providing safe and accurate correction.

The newborn foot: diagnosis and management of common conditions.

An assessment of the foot should be a part of every newborn physical examination. The newborn foot, although complex in structure, can be examined quickly in any office setting. Many foot deformities are diagnosed immediately after birth, allowing for earlier and often more successful treatment. A neonate with a foot deformity can be a source of anxiety to parents. Adequate knowledge of commonly encountered neonatal foot problems enables the nurse to give appropriate anticipatory guidance to the parents. Foot abnormalities usually occur as isolated findings in an otherwise healthy newborn. How…

Preclinical symptoms of the diabetic foot.

BACKGROUND: The diabetic foot is one of the main complications of diabetes mellitus, with a high risk of minor or major amputation. The preclinical foot lesions of patients without foot complaints were compared with healthy controls and analyzed. METHODS: This study was conducted with 89 diabetic patients from an endocrinology clinic and 35 nondiabetic control patients. The patients were asked about the presence, types, and durations of pedal complaints; acquired and congenital foot deformities; and atrophy. Patient gaits were inspected for any swelling; skin and nail changes were also record…

Foot surgery for adults with Charcot-Marie-Tooth disease.

People with Charcot-Marie-Tooth (CMT) disease often undergo foot and ankle surgery, as foot deformities are common and cause a degree of functional limitations impairing quality of life. Surgical approaches are variable and there are no evidence-based guidelines. A multidisciplinary approach involving neurology, physical therapy and orthopaedic surgery is ideal to provide guidance on when to refer for surgical opinion and when to intervene. This review outlines the range of foot deformities associated with CMT, their clinical assessment, and their conservative and surgical and postoperative ma…

Varus and valgus foot in cerebral palsy and its management.

Varus and valgus foot, often associated with equinus, are common problems in cerebral palsy. These deformities can interfere with standing or walking, aggravate shoe wear, and lead to painful calluses on pressure areas of the foot. The deformities are caused by a basic muscle imbalance. If the posterior or anterior tibial muscles are overactive, the foot will swing into varus. If the peroneal muscles predominate, the foot assumes a valgus position. However, the alignment of the hip and knee play an important role in the production of deformity. Correction of the deformity must be preceded by a

Correction of neglected clubfoot using the Ilizarov external fixator.

BACKGROUND: This study was conducted to evaluate the corrective capability of the Ilizarov external fixator in the treatment of neglected cl … thoses. CONCLUSION: The Ilizarov external fixator allows simultaneous correction of all the severe foot deformities associated with neglected clubfoot with minimal surgery, reducing risks of cutaneous or neurovascular complications and avoiding excessive shortening of the foot. Even in those patients in whom final corrective arthrodesis is necessary, this may be …

[The rheumatic foot and its treatment].

The alterations of form and function of the rheumatoid foot are derived from the pathomechanical conditions. The latter are used as a measure for the treatment. Type and frequency of inflammatory alterations are studied by x-ray examination and new aspects are stressed. The deformities of the rheumatoid foot can be explained conclusively through the interaction of functional, gravitational, inflammatory and anatomical influences. The deformities of the toes are induced to a great extent by the deformities of the proximal part of the foot. The possibilities of surgical treatment are discussed i

The indications and technique for surgical correction of pes cavus with external fixation.

Cavovarus is a deformity commonly associated with a variety of underlying disorders, and treatment of severe cavovarus foot with an underlying progressive disorder is very challenging. Often patients have undergone some prior surgery at least once, with increased potential risk of neurovascular injury and breakdown of soft tissue. In addition, concomitant problems such as torsional malalignment and leg-length discrepancy should also be addressed to prevent recurrence and treatment failure. In this article, indications and an algorithmic approach with various osteotomies for the treatment of ca

Resistant clubfoot deformities managed by Ilizarov distraction histogenesis.

BACKGROUND: Resistant clubfoot deformities of the foot and ankle remain a difficult problem even for the most experienced surgeon. We report a series of neglected resistant clubfoot deformities treated by limited surgery and Ilizarov distraction histogenesis. MATERIALS AND METHODS: Twenty one patients with 27 feet having resistant clubfoot deformities were managed by Ilizarov distraction histogenesis from April 2005 to May 2008. The mean age was 12 years (range 8-20 years). A limited soft tissue dissection like percutaneous Achilles sheath tenotomy and plantar fasciotomy were done. Progressiv…

Effectiveness and safety of elective surgical procedures to improve wound healing and reduce re-ulceration in diabetic patients with foot ulcers.

The objective is to evaluate the effectiveness and safety of surgical off-loading to heal diabetic foot ulcers and prevent ulcer recurrence. Usually, structural foot deformities such as hallux rigidus, hammertoe deformities and equinus of the ankle contribute to abnormal pressure and shear forces and non-healing foot ulcers. Elective surgery to remove the deformity and restore joint mobility has been shown to be safe and effective to improve wound healing of recalcitrant ulcer and to reduce the risk of re-ulceration. Unfortunately, there is very little high-level evidence to help guide patient…
