Evaluation of Musculoskeletal Re-Injury Occurrence in Previously Concussed National Football League Athletes.

INTRODUCTION: Increased risk of musculoskeletal (MSK) injury post-concussion has been reported in collegiate athletes, yet it is unknown if professional football athletes are at the same risk of secondary injury. The objective of this study was to determine if the risk of MSK injury in National Football League (NFL) athletes increases after concussion. METHODS: NFL injury reports from 2013 to 2017 were collected from public websites. Concussed athletes (n=91) were equally matched to a non-injured control and an athlete with an incident of musculoskeletal (MSK) injury. RESULTS: Following thei…

Relating on-field youth football head impacts to pneumatic ram laboratory testing procedures.

A youth-specific football helmet testing standard has been proposed to address the physical and biomechanical differences between adult and youth football players. This study sought to relate the proposed youth standard-defined laboratory impacts to on-field head impacts collected from youth football players. Head impact data from 112 youth football players (ages 9-14) were collected through the use of helmet-mounted accelerometer arrays. These head impacts were filtered to only include those that resided in corridors near prescribed National Operating Committee on Standards for Athletic Equip…

Slow blood-to-brain transport underlies enduring barrier dysfunction in American football players.

Repetitive mild traumatic brain injury in American football players has garnered increasing public attention following reports of chronic traumatic encephalopathy, a progressive tauopathy. While the mechanisms underlying repetitive mild traumatic brain injury-induced neurodegeneration are unknown and antemortem diagnostic tests are not available, neuropathology studies suggest a pathogenic role for microvascular injury, specifically blood-brain barrier dysfunction. Thus, our main objective was to demonstrate the effectiveness of a modified dynamic contrast-enhanced MRI approach we have develop…

Association of Increased Serum S100B Levels With High School Football Subconcussive Head Impacts.

Astrocyte-enriched marker, S100B, shows promise for gauging the severity of acute brain trauma, and understanding subconcussive effects will advance its utility in tracking real-time acute brain damage. The aim of the study was to investigate whether serum S100B elevations were associated with frequency and magnitude of subconcussive head impacts in adolescents. This prospective cohort study of 17 high-school football players consisted of the following 12 time points: pre-season baseline, 5 in-season pre-post games, and post-season. A sensor-installed mouthguard recorded the number of head imp…

Repetitive Head Impacts in Youth Football: Description and Relationship to White Matter Structure.

BACKGROUND: Few studies have examined white matter with diffusion tensor imaging in 8- to 12-year-old collision sport (CS) athletes. HYPOTHESIS: Youth CS athletes will demonstrate change in brain fractional anisotropy (FA) after a season of CS compared with an age-matched noncollision sport (NCS) cohort, and the number, magnitude, and location of hits will correlate with changes in the brain determined via FA for CS athletes. STUDY DESIGN: Prospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: Thirty-five 8- to 12-year-old males in a youth tackle football league (CS) and 12 males f…

Do American Youth Football Players Intentionally Use Their Heads for High-Magnitude Impacts?

BACKGROUND: Concern for head injuries is widespread and has been reported by the media to be the number one cause of decreased participation in football among the American youth population. Identifying player mechanisms associated with intentional, or purposeful, head impacts should provide critical data for rule modifications, educational programs, and equipment design. PURPOSE: To investigate the frequency of intentional and unintentional head impacts and to examine the player mechanisms associated with intentional high-magnitude head impacts by comparing the impact mechanism distributions …

Clinical Features and isokinetic Parameters in Assessing Injury Risk in elite Football Players.

Football players frequently face the occurrence of non-contact injuries. Although there are likely multiple factors that contribute to increased risk of non-contact injury, it remains a challenge to correlate all these factors. However, it is not clear how much of individual training abilities may interfere in these events. As such, the primary aim of the present study was to determine whether the reduction of functional performance of the thigh in the isokinetic knee tests, anthropometric and morbid history can establish risk factors for lower-limb musculoskeletal injuries throughout the seas…

Knee Ultrasonography to Determine Risk for Noncontact Injuries in Collegiate American Football Players.

Knee ultrasonography has been used effectively as a screening tool for determining risk for knee injuries in athletes. Ultrasonography may be a valuable screening tool for relative risk of noncontact knee injuries that occur over a typical playing career in collegiate American football players. In this prospective longitudinal study, we evaluated American football players (n = 48) in an academic institution affiliated with the National Collegiate Athletic Association Division I athletic program. Players underwent comprehensive ultrasonography of both knees prior to beginning their collegiate c…
