Primary fusion of fracture dislocations of central carpometacarpal joints.

Six patients with dislocations or fracture dislocations of at least the second and third carpometacarpal joints are presented. Closed reduction was attempted in all cases. Two dislocations with minimal periarticular fractures were stable and were treated with cast immobilization. Four fracture dislocations were unstable and were treated with primary arthrodesis of the injured joints. One patient was lost to followup and the other 5 were observed for an average of 24 months. Establishment of a stable or rigid central carpometacarpal joint produced an excellent result in 4 patients and a satisfa…

[TMJ morphology after condylar dislocation fractures in childhood].

In a CT- and NMR-study, condylar remodeling after conservative treatment of condylar dislocation fractures in growing individuals was evaluated. The regenerated condyles displayed great similarities. Two condylar parts could be differentiated--one with and one without muscular attachment. The medial part represents the former small fragment and the lateral part a bony outgrowth. One of the patients studied demonstrated a bifid condyle. NMR-records were remarkable in that each of the three patients presented a lateral pterygoid muscle with the two heads oriented one above the other rather than

Bilateral posterior hip dislocations with femoral head fractures.

An unusual case of bilateral posterior fracture-dislocation of the hip (Pipkin Type IV) occurred in a 63-year-old man with Paget's disease of the pelvis. Other injuries included a displaced humeral shaft fracture and patellar ligament disruption. Bilateral cemented total hip arthroplasty was performed to avoid the need for prolonged immobilization. Postoperative low-dose irradiation was used because of the risk of heterotopic ossification.

CT patterns of facet fracture dislocations in the thoracolumbar region.

Thoracolumbar facets are not as commonly dislocated as are those of the cervical spine. It is, however, crucial to make an early and accurate diagnosis of thoracolumbar facet dislocation since the injury may be unstable and require reduction and internal fixation. This paper presents three major CT patterns of thoracolumbar facet fracture dislocation. The first represents anterior subluxation of the vertebral body with anteriorly locked facets. The second is a lateral vertebral body subluxation with laterally locked facets. The third is an acute kyphosis with little vertebral body subluxation …

Fracture of the neck of the femur associated with posterior dislocation of the hip.

Two patients are described, each with a fracture-dislocation of the hip combined with a fracture of the neck of the same femur. Open reduction combined with internal fixation was performed in both cases. Eight years later one patient had developed avascular necrosis of the femoral head; no signs of avascular necrosis or associated arthritis have appeared in the other patient after four years. A plea is made for considering this more conservative type of operation for these serious injuries before resorting to total hip replacement.

Delayed presentation of posterior fracture dislocation of the hip in a child.

A case of delayed presentation of posterior fracture dislocation of the hip in a child is reported. Only four similar adult cases have been described in the English medical literature. This case is even more perplexing than those already reported in thAT AP and lateral X-rays of the affected hip were performed on initial presentation and were normal. The mechanism of such a phenomenon, explanations for failure to detect such an injury and methods of avoiding this complication are presented.

Unstable fracture-dislocations of the forearm (Monteggia and Galeazzi lesions).

Forty-nine Monteggia and forty-seven Galeazzi lesions were treated over a twenty-five-year period. I used Bado's criteria to evaluate the results in the Monteggia lesions. In all of the children in the series either closed or open reduction yielded good results, while the results of treatment of the Monteggia fractures in the adults in the study varied. The best results were obtained in Type-I lesions treated by open anatomical reduction, internal stabilization of the ulnar fracture, and closed reduction of the radial head. Factors leading to poor results in Type-I lesions were failure to obta

[Cervical spine. II. Fractures and dislocations (author's transl)].

Clinical and radiologic investigations in spinal fractures and dislocations must allow assessment of modifications in osteoarticular and neural functions. With respect to osteoarticular function it is necessary to identify the presence of both temporary or permanent instability and impaired static equilibrium. Regarding neural functions we must not only evaluate the neurologic damage but also residual stenosis of the spinal canal and sometimes of the intervertebral foramina. Compression induced by such stenosis is best demonstrated on métrizanide myelography. Management is currently aimed at b

Fractures and dislocations encountered by the general surgeon: general principles.

The definitive treatment selected for the care of a fracture is dependent upon what will offer the best cosmesis and function for the individual patient. Closed reductions and immobilization which require the joints to be in abnormal positions may offer an anatomic reduction but after the fracture is healed, joint function may be poor. Consequently, immobilization in the functional position with early motion and acceptance of some deformity is often better in terms of the ultimate use of the extremity. Traction treatment which requires prolonged recumbency is dangerous in the elderly because o

Current concepts in management of cervical spine fractures and dislocations.

In order to manage the patient with a cervical spine injury, one must be familiar with a diagnostic classification of these injuries and be able to apply this classification to the individual patient. When a cooperative effort is made between orthopedists and neurosurgeons, we have found that most cervical spine injuries can be managed successfully without operative means using Cheshire's classification for predicting late instability. We have been able to identify (1) those patients who would not substantially benefit from surgical intervention; (2) certain types of injuries where relative in
