BACKGROUND: One goal of classifying fractures at the base of the fifth metatarsal is to reliably identify which fracture patterns have an increased risk of delayed union and nonunion and therefore merit consideration for operative fixation. The most common classification system of proximal fifth metatarsal fractures is the three-part Lawrence and Botte scheme, which has been shown to have poor interobserver and intraobserver reliability and can be ambiguous in terms of clinical treatment and prognosis. In particular, the assignment of the middle classification (Type II) has unclear implication

While soccer playing a 19 year old man suffered from acute onset of severe pain in the anterior part of the iliac crest. A CT-scan confirmed the unilateral avulsion of the anterior superior iliac spine. Due to only minimal fragment dislocation and still open apophysis we decided to treat this injury conservatively with crutches and partial weight bearing. There were no complications and already three weeks after the accident the young man was nearly painfree. Avulsion of the anterior superior iliac spine most commonly occur in adolescents. The fracture is often missed and treated as muscle inj

The tarsal navicular plays an integral role in hind-foot motion and gait, and is the keystone of the foot's medial longitudinal arch. As such, injuries to the navicular can be devastating. Acute avulsion, tuberosity, and body fractures have been described. Fractures of the body result from high-energy trauma and are often seen in conjunction with additional ipsilateral foot injuries. Plain radiographs are the gold standard for diagnosis, with computed tomography helpful in the presence of intra-articular fracture extension. Non-operative treatment is reserved for avulsion injuries and non-disp

PURPOSE: The pathogenetic mechanism, progression, and instability in geriatric bilateral fragility fractures of the sacrum (BFFSs) remain poorly understood. This study investigated the hypothesis of sequential BFFS progression by analysing X-ray, computed tomography (CT), and magnetic resonance imaging (MRI) datasets.
METHODS: Imaging data from 78 cases were retrospectively analysed. Fractures were categorized using the CT-based Fragility Fractures of the Pelvis classification. MRI datasets were analysed to detect relevant fracture location information. The longitudinal sacral fracture was gr

Patella sleeve-fractures are a rare but significant injury, mostly affecting children aged 8-12. These avulsion fractures involve cartilage, periosteum, and bone, usually caused by forceful quadriceps contractions during knee flexion. Diagnosis can be challenging due to subtle radiographic changes, often leading to delayed treatment. Management depends on displacement severity: immobilisation versus surgery to restore extensor mechanism function. Early diagnosis and proper care are crucial to prevent complications like instability and functional impairment, as argued in this case.

INTRODUCTION: Tibial spine avulsion injury, tibial eminence injury, tibial spine fracture, and anterior cruciate ligament (ACL) avulsion are multiple terms that express the same pathological condition. It can be encountered both in the pediatric and adult population. A wide array of surgical techniques have been proposed to manage displaced tibial spine avulsions. Anyway, insufficient evidence is currently available to prefer one fixation technique over another, and a gold-standard arthroscopy-based technique is still missing. In this article, we describe a mini-invasive, safe and user-friendl

This article describes a case of an acute anterior inferior iliac spine apophyseal avulsion fracture in an adolescent athlete progressing to secondary symptomatic femoroacetabular impingement from an inferiorly displaced malunion and its arthroscopic management. A 13-year-old boy with an acute minimally displaced avulsion fracture of the anterior inferior iliac spine apophysis had initial symptomatic improvement with conservative treatment and a 3-month symptom-free period but then developed flexion-induced deep anterior groin pain and mechanical symptoms. Radiographs confirmed an inferiorly d

BACKGROUND: Avulsion fractures of the sublime tubercle of the ulna are a cause of medial elbow pain and instability in overhead athletes.
PURPOSE: To compare outcomes after sublime tubercle avulsion fracture managed as a fracture (with cast immobilization) versus a soft tissue injury (without immobilization and with early range of motion [ROM]) to determine how to achieve the best outcomes for these injuries in adolescent throwing athletes.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: A multicenter retrospective review using an institutional database query of radiology reports

We present an unusual and complex case of a 16-year-old adolescent male who injured his right knee and sustained combined avulsion injuries of posterior cruciate ligament (PCL) at the tibial insertion site, iliotibial band at lateral tibial condyle, and lateral collateral ligament (LCL) at femoral insertion site akin to osteoligamentous posterolateral corner injury. Anatomical reduction of the femoral LCL avulsion fragment was performed and fixed with a two 4-mm partially threaded cancellous screw. Iliotibial band avulsion was buttressed using Ellis t-plate and fixed with two 4-mm partially th

Bifocal disruption of the knee extensor mechanism is a rare injury. Bifocal proximal and distal avulsion fracture of the patella tendon is extremely rare in adults. Here, the authors report the case of an 84-year-old male patient who developed simultaneous avulsion fracture of the patella tendon from the inferior pole of the patella and the tibial tuberosity. Open reduction and internal fixation were performed, and at 12 months postoperatively, an excellent functional result was obtained.

Avulsion fracture of the tibial tuberosity usually caused by sudden violent contraction of the quadriceps muscles is uncommon and predominantly occurs in adolescents. Illustrative is the case history of a 16-year old boy with an Ogden I-B avulsion fracture successfully treated by open reduction and internal screw fixation. In these cases, anatomic reduction should be the primary goal. In specific cases this can be achieved by closed reduction, however, usually open reduction and fixation is indicated.

The treatment of an anterior cruciate ligament (ACL) avulsion fracture is controversial, especially in skeletally immature patients, because of concerns about physeal damage. To reduce the risk of physeal injury, an arthroscopic technique was performed. A bioabsorbable suture anchor was inserted through anteromedial portals and fixed to a bioabsorbable suture anchor at the center of the fracture bed; it was then passed through the threads at the ACL avulsion fragment and tied with the ACL substance. After this, the avulsion fragment was repaired by an all-inside technique between the distal po

There is a paucity of orthopaedic literature describing avulsion of the ischial tuberosity, but its spontaneous healing does not seem to have been reported to-date. This article describes the case of a young football player who suffered an avulsion fracture of the ischial tuberosity while kicking vigorously next to the ball during a football match. The diagnosis was not made at the time of trauma, and the fracture was found five years later to have healed spontaneously. A review of the literature is provided.

Traumatic avulsion of the triceps brachii tendon is a rare injury. Clinical diagnosis shows that elbow extension against gravity is not possible. Radiologically, the lateral elbow may show the presence of flecks of avulsed osseous material from the olecranon ("flake sign"), which is almost pathognomonic of this lesion. The authors of the present study present a case of avulsion fracture treated with the attachment of bone suture anchors.

We herein present the exceptional case of a patient, who injured a sciatic nerve due to avulsion of proximal hamstring tendon in a motorcycle accident. The 63-year-old man was diagnosed firstly with an incomplete fracture of distal femur. A foot drop on the right side was observed when the full-length cast was removed two months later. The patient was referred to the neurology clinic and was diagnosed with a sciatic nerve lesion at the proximal level of the biceps femoris. Magnetic resonance imaging of the thigh showed a proximal avulsion of hamstring muscles tendon. The patient did not improv

Background The pathogenesis of Kienbock's disease is poorly understood. The coronal fracture line is acknowledged as a poor prognostic marker in the disease. Other fracture types in the Kienbock's wrist have subsequently been identified. Questions/Purposes The aim is to study the fracture morphology of the lunate in a cohort of patients using computed tomography. Methods Patient images were acquired using a set protocol with four-dimensional computed tomography scanning. Images were reviewed by two orthopaedic surgeons and a consultant radiologist. Static and dynamic images were assessed an

We propose an anatomical classification of fractures involving the distal articular surface of the scaphoid. The predominant patterns, in the 37 patients reviewed, were avulsion fractures from the radio-volar tip of the tuberosity and impaction fractures of the radial half of the articular surface. The possible mechanisms of injury are discussed.

Mandibular fractures represent a common consequence of high-energy facial trauma; however, avulsive fractures of the genial tubercle are exceedingly rare and sparsely documented in the literature. These injuries may be easily overlooked, particularly in polytrauma settings, despite their potential impact on airway function and mandibular stability. This report describes a unique case of combined midfacial and mandibular trauma, including an uncommon 2-cm avulsion fracture of the genial tubercle, following a motorcycle accident. A 22-year-old male presented to the emergency department after a m

OBJECTIVE: To provide a comprehensive review of the biomechanics, pathophysiology, and clinical management of atlas fractures.
METHODS: Selected literature review.
RESULTS: Atlas fractures account for 25% of craniocervical injuries, 3% to 13% of cervical spine injuries, and 1% to 3% of all spinal injuries. Motor vehicle accidents account for 80% to 85% of atlas fractures, and the mechanism of injury is axial loading. Isolated atlas fractures are more common; however, 40% to 44% of atlas fractures have concomitant axis fractures. Fractures of isolated anterior or posterior arches are more com
