Postoperative reflux gastritis.

The chief clinical features of forty-nine patients with the syndrome of reflux "alkaline" gastritis were epigastric pain, bilious vomiting, anemia, and the dumping syndrome. Separation of the symptoms of this syndrome from the symptoms of a multitude of other postgastrectomy syndromes is difficult, being complicated by a high incidence of emotional instability in these patients. Endoscopy remains the mainstay in diagnosis; among the characteristic endoscopic features are adherent mucus, edema, mucosal friability, and erosions, most severe on the gastric aspect of the stoma. The surgical treatm

Comparisons in Postoperative Endoscopic Findings and Postoperative Weight Change Between Delta-Shaped Anastomosis and Circular-Stapled Anastomosis in Laparoscopy-Assisted Distal Gastrectomy With B-I Reconstruction.

BACKGROUND: Laparoscopy-assisted distal gastrectomy (LADG) with Billroth I (B-I) reconstruction is frequently performed for gastric cancer. However, the difference between the circular stapler technique (CS) and delta-shaped anastomosis (DA) remains unclear, especially regarding the postoperative endoscopic physiological findings. METHODS: Three hundred and one patients including 150 CS patients and 151 DA patients during LADG with B-I reconstruction between 2013 and 2019 at Saitama Medical University International Medical Center were chosen as study subjects. Postoperative endoscopic finding

Short-term outcomes of gastric cancer patients with versus without duodenal stump reinforcement after laparoscopic gastrectomy.

BACKGROUND: Duodenal stump leakage (DSL) is a fairly uncommon but dangerous complication after gastrectomy with Billroth II (B-II) or Roux-en-Y (R-Y) reconstruction. The significance of reinforcing the duodenal stump to prevent DSL during laparoscopic gastrectomy is still under debate. We aimed to explore the clinical impact of duodenal stump reinforcement after laparoscopic gastrectomy with duodenal stump. METHODS: We retrospectively collected consecutive patients with gastric cancer from our center between June 2019 and September 2023. Patients who underwent laparoscopic distal or total gas

Billroth-II modified with hinged anti-peristaltic afferent loop versus Roux-en-Y reconstruction after distal gastrectomy for gastric cancer: A propensity-score match analysis.

BACKGROUND: This study aimed to evaluate the effectiveness of modified Billroth-II with a hinged anti-peristaltic afferent loop by comparing it with the Roux-en-Y method. METHODS: We retrospectively analyzed 344 patients with gastric cancer who underwent distal gastrectomy between 2016 and 2021. Propensity score matching was conducted to balance baseline characteristics. RESULTS: After propensity score matching, there were 117 patients in each group. The Billroth-II group was significantly better regarding operating time (184.7 vs 225.3 minutes), postoperative hospital stays (7.9 vs 9.2 days

[State of duodenal patency in patients with postgastrectomy syndromes].

Источник пометил публикацию как отозванную

It was analyzed the examination and treatment results of 100 patients who underwent resection of stomach by Billroth-I in case of peptic ulcer. Chronic disorders of duodenal patency were diagnosed in 86% of patients. The main role of chronic disorders of duodenal patency in postgastrectomy syndromes development was proved. There were a combination of reflux gastritis with dumping syndrome in 66.3% of patients, a combination of reflux gastritis with recurrent ulcer in 8.1% of patients. Correction of chronic disorders of duodenal patency is necessary stage in conservative and surgical treatment

The role of duodenogastric reflux in formation of precarcinogenic gastric lesions--an experimental study.

INTRODUCTION: Duodenogastric reflux, commonly encountered as an aftermath of gastroenteroanastomosis, with or without gastric resection (Billroth I, Billroth II), vagotomy and pyloroplastic surgery, is known to cause inflammatory-dystrophic-metaplastic lesions of gastric mucosa. Our objective was to determine the effects of surgery-induced duodenogastric reflux on the development of precarcinogenic lesions or carcinoma in correlation with the reflux duration. MATERIAL AND METHODS: The experiment was performed on three groups of Wistar rats with 1) Billroth II-induced reflux surgery, 2) resect

[A case of afferent loop syndrome with acute cholangitis developed after percutaneous transhepatic cholangioscopic lithotripsy for treatment of choledocholithiasis in a patient who underwent Billroth II gastrectomy].

Afferent loop syndrome is a rare complication which can occur in patients with Billroth II gastrectomy. Bile and pancreatic juice is congested at afferent loop in the syndrome. This syndrome can progress rapidly to necrosis, perforation, or severe sepsis, and therefore early diagnosis and swift surgical intervention is important. But, cases of endoscopic or percutaneous transhepatic drainage have been reported when surgical management was inappropriate to proceed. We report a case of afferent loop syndrome accompanying acute cholangitis developed after percutaneous transhepatic cholangioscopic

Management of esophageal atresia with a tracheoesophageal fistula complicated by gastric perforation.

PURPOSE: Gastric perforations generally develop in neonates with esophageal atresia (EA) and a tracheoesophageal fistula (TEF), requiring preoperative mechanical ventilation. To the best of our knowledge, spontaneous gastric perforation in patients who have not been treated with mechanical ventilation has not been described in the literature. There is also no current consensus or treatment protocol available for the management of these patients. METHODS: Over a period of 6 years, six patients with EA and TEF presented with gastric perforation at our center. We studied the clinical presentatio

A sodium hyaluronate carboxymethylcellulose bioresorbable membrane prevents postoperative small-bowel adhesive obstruction after distal gastrectomy.

PURPOSE: It is predictable that since distal gastrectomy (DG) with Billroth I anastomosis involves no procedures caudal to transverse colon, the effects of the surgical wound are the main cause of adhesive obstruction. Thus, it is an appropriate operation to test the efficiency of a synthetic absorbable adhesion barrier (Seprafilm). METHODS: The subjects were 282 patients diagnosed with gastric cancer who underwent open DG with Billroth I anastomosis between 2001 and August, 2005. Seprafilm was not used in any patients operated on before April, 2003 (n = 169), but it was used in all patients

ERCP afterRoux-en-Y reconstruction can be carried out using an oblique-viewing endoscope with an overtube.

BACKGROUND: We report that an oblique-viewing endoscope facilitates endoscopic retrograde cholangiopancreatography (ERCP) in Billroth II reconstruction. With this endoscope, we carried out ERCP in Roux-en-Y reconstruction. METHODS: Fifteen patients with Roux-en-Y reconstruction were enrolled. Eleven of these patients had undergone gastrectomy, while Whipple's operation or choledochectomy had each been carried out in two patients. Among the 11 post-gastrectomy patients, eight had bile duct stones, and there was one case each of pancreatic abscess with chronic pancreatitis, bile duct obstructio

[Diagnosis and combined treatment of esophageal chemical burns in children].

The experience of management of 426 children with chemical burns of the esophagus and the stomach has been analyzed. The necessity of earlier (up to 3 hours) lavage not only of the stomach, but also of the esophagus, is substantiated. It is considered advisable to carry out diagnostic fibrogastroesophagoscopy not later than 1 day after the trauma and to give up early prophylactic bouginage of the esophagus. The latfer rather stimulates sear formation than prevents it. The developed scheme of treatment was used in 256 patients with resultant decrease of postburn stenoses of the esophagus 3 time

Technical complications of Roux-en-Y gastrojejunostomy.

The Roux-en-Y gastrojejunostomy has become an increasingly popular technique for gastrointestinal tract reconstruction since it is purported to obviate many of the classic complications of the Billroth II gastrojejunostomy. In a review of over 900 Roux-en-Y gastrojejunostomies, seven patients with complications mimicking those seen with Billroth II reconstruction were identified. These complications included duodenal stump blowout, proximal blind loop or afferent loop syndrome, and bile-reflux gastritis. These complications resulted from technical problems in construction of the Roux-en-Y. Onc

Intestinal blind pouch- and blind loop- syndrome in children operated previously for congenital duodenal obstruction.

A follow-up study of 27 children operated for congenital duodenal obstruction (CDO) in the years 1953--71 is presented. Nine children belonged to the intrinsic and 18 children to the extrinsic group of CDO. A total of 7 retrocolic, isoperistaltic, side-to-side duodeno-jejunostomy, 7 Ladd's operation, 8 duodenolysis, 2 reduction of midgut volvulus, 2 duodenostomy a.m. Morton and one gastro-jejunostomy were performed at the age of 1 day--15 years. The clinical and radiological examinations were performed 3--21 years (mean 10 years 2 months) after these operations. In 3 cases there was a moderate

Clinical outcomes of endoscopic retrograde cholangiopancreatography after Billroth II anastomosis: a comparison of gastroscope and duodenoscope.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) in patients with Billroth II anastomosis is challenging due to post-surgical anatomical alterations. This study aims to compare the clinical outcomes of using a duodenoscope and a cap-assisted gastroscope in these patients. METHODS: Seventy-nine patients with Billroth II anastomosis and a naïve papilla were included in the study. ERCP was performed using either a cap-assisted gastroscope (n = 45) or a duodenoscope (n = 34). The primary outcome was the cannulation success rates, while secondary outcomes included clinical success
