DNRLCNN: A CNN Framework for Identifying MiRNA-Disease Associations Using Latent Feature Matrix Extraction with Positive Samples.

Emerging evidence indicates that miRNAs have strong relationships with many human diseases. Investigating the associations will contribute to elucidating the activities of miRNAs and pathogenesis mechanisms, and providing new opportunities for disease diagnosis and drug discovery. Therefore, it is of significance to identify potential associations between miRNAs and diseases. The existing databases about the miRNA-disease associations (MDAs) only provide the known MDAs, which can be regarded as positive samples. However, the unknown MDAs are not sufficient to regard as reliable negative sample

Scalable Reconstruction of SARS-CoV-2 Phylogeny with Recurrent Mutations.

This article presents a novel scalable character-based phylogeny algorithm for dense viral sequencing data called SPHERE (Scalable PHylogEny with REcurrent mutations). The algorithm is based on an evolutionary model where recurrent mutations are allowed, but backward mutations are prohibited. The algorithm creates rooted character-based phylogeny trees, wherein all leaves and internal nodes are labeled by observed taxa. We show that SPHERE phylogeny is more stable than Nextstrain's, and that it accurately infers known transmission links from the early pandemic. SPHERE is a fast algorithm that

Computing the Rearrangement Distance of Natural Genomes.

The computation of genomic distances has been a very active field of computational comparative genomics over the past 25 years. Substantial results include the polynomial-time computability of the inversion distance by Hannenhalli and Pevzner in 1995 and the introduction of the double cut and join distance by Yancopoulos et al. in 2005. Both results, however, rely on the assumption that the genomes under comparison contain the same set of unique markers (syntenic genomic regions, sometimes also referred to as genes). In 2015, Shao et al. relax this condition by allowing for duplicate markers i

Evolutionary algorithm for vehicle driving cycle generation.

Modeling transit bus emissions and fuel economy requires a large amount of experimental data over wide ranges of operational conditions. Chassis dynamometer tests are typically performed using representative driving cycles defined based on vehicle instantaneous speed as sequences of "microtrips", which are intervals between consecutive vehicle stops. Overall significant parameters of the driving cycle, such as average speed, stops per mile, kinetic intensity, and others, are used as independent variables in the modeling process. Performing tests at all the necessary combinations of parameters

From Biomarkers to Better Outcomes: Precision Medicine in Modern Internal Medicine.

Internal medicine has traditionally focused on population-based approaches, utilizing diagnostic algorithms and treatment thresholds designed for the "average" patient. While beneficial, this one-size-fits-all model often leads to treatment failures. Precision medicine addresses this limitation by tailoring medical care to an individual's unique molecular, genetic, and clinical profile. Driven by advancements in multi-omics platforms, novel biomarkers, artificial intelligence (AI), and pharmacogenomics, precision medicine is transforming modern internal medicine from oncology into routine clin

Skeletal dysplasias: A radiographic approach and review of common non-lethal skeletal dysplasias.

Skeletal dysplasias are not uncommon entities and a radiologist is likely to encounter a suspected case of dysplasia in his practice. The correct and early diagnosis of dysplasia is important for management of complications and for future genetic counselling. While there is an exhaustive classification system on dysplasias, it is important to be familiar with the radiological features of common dysplasias. In this article, we enumerate a radiographic approach to skeletal dysplasias, describe the essential as well as differentiating features of common non-lethal skeletal dysplasias and conclude

Failed intubation and failed oxygenation in a child.

A 'can't intubate, can't oxygenate' scenario in a child is fortunately extremely rare. We report a case of this life-threatening event in a four-year-old boy suffering from a rare genetic disorder, fibrodysplasia ossificans progressiva. He presented for manipulation of his dislocated jaw and was identified preoperatively as having a difficult airway. Despite extensive preparation, a catastrophic loss of airway control occurred minutes after induction of general anaesthesia, necessitating a life saving emergency tracheostomy. This report highlights the small evidence base and lack of definitive

Machine learning-driven multi-objective optimization of straight-blade paddle mixers for efficient mixing in melt-cast explosives systems.

In the production of melt-cast explosives, efficient mixing in straight-blade paddle mixers is critical to product performance. Taking the classical trinitrotoluene-cyclotrimethylenetrinitramine (TNT-RDX) system as an example, this paper presents a machine learning-based multi-objective optimization algorithm that addresses the conflicting requirements of maximizing mean flow velocity and minimizing maximum pressure difference during mixing. By training random forest regression models on historical experimental and simulation data, the algorithm accurately predicts mixing outcomes from structu

Classification of unilateral vocal fold paralysis by endoscopic digital high-speed recordings and inversion of a biomechanical model.

Hoarseness in unilateral vocal fold paralysis is mainly due to irregular vocal fold vibrations caused by asymmetries within the larynx physiology. By means of a digital high-speed camera vocal fold oscillations can be observed in real-time. It is possible to extract the irregular vocal fold oscillations from the high-speed recordings using appropriate image processing techniques. An inversion procedure is developed which adjusts the parameters of a biomechanical model of the vocal folds to reproduce the irregular vocal fold oscillations. Within the inversion procedure a first parameter approxi

The impact of self-reported ethnicity versus genetic ancestry on phenotypic characteristics of polycystic ovary syndrome (PCOS).

CONTEXT: It is well established that ethnicity is associated with the phenotype of polycystic ovary syndrome (PCOS). Self-reported ethnicity was shown to be an inaccurate proxy for ethnic origin in other disease traits, and it remains unclear how in PCOS patients self-reported ethnicity compares with a biological proxy such as genetic ancestry. OBJECTIVE: We compared the impact of self-reported ethnicity versus genetic ancestry on PCOS and tested which of these 2 classifications better predicts the variability in phenotypic characteristics of PCOS. PATIENTS: A total of 1499 PCOS patients fro

Estimating the sensitivity of a genetic test using gene-carrier probability estimates and its application in genetic counselling.

BACKGROUND: The estimation of the sensitivity of a genetic test is of practical importance. If a 'gold standard' (an exact test) is not available, an estimate of the probability of being a gene carrier may be useful for an individual. This information could be based on individual pedigree and family-history data, and a known genetic model for the disease of interest. METHODS: We develop a maximum-likelihood estimate of the sensitivity of a genetic test that may be applied in a situation without a 'gold standard', in which an estimate of the probability of being a gene carrier for an individua

Genetic Testing in Prospective Kidney Donors: Current Evidence and Clinical Practice in Australia.

Genetic testing is increasingly transitioning from being a research tool to standard clinical practice, but guidance for integrating genomics into living kidney donor assessment remains limited. This review provides an evidence-based framework for genetic testing in prospective kidney donors within clinical practice in Australia. Kidney donor genetic assessment requires first establishing a genetic diagnosis in the transplant recipient. Identifying which patients with chronic kidney disease warrant genetic testing depends on clinical phenotype, extrarenal manifestations, age of onset, and, cru

Exploring genetic drivers of unexplained venous thromboembolism through whole-exome sequencing.

Venous thromboembolism (VTE) is a complex multifactorial disorder in which the contribution of genetic and non-genetic risk factors is well recognized, yet standard thrombophilia testing is unable to explain a substantial proportion of cases. To explore additional genetic contributors, we performed whole-exome sequencing (WES) in 35 patients with recurrent and/or severe VTE in whom routine diagnostic evaluation failed to identify risk factors. Variants were first screened against established thrombophilia-associated loci using clinical classification tools, while the remaining variants were pr

Causal Transformer for Learning Embeddings from Structured Medical History Records and Multi-Source Data Integration for Complex Disease Risk Prediction.

Traditional disease risk prediction models predominantly rely on statistical algorithms and often focus on genetic factors or a limited set of lifestyle factors to estimate the risk of disease onset. Recently, more comprehensive approaches have emerged that integrate genetic factors with additional lifestyle factors (e.g., alcohol intake) and physical features (e.g., body mass index, age) to increase predictive accuracy. Since the onset of complex diseases is often accompanied by the occurrence of comorbidities, incorporating medical history records is a critical yet underexplored avenue for i

Predicting relapse risk in childhood acute lymphoblastic leukaemia.

Intensive multi-agent chemotherapy regimens and the introduction of risk-stratified therapy have substantially improved cure rates for children with acute lymphoblastic leukaemia (ALL). Current risk allocation schemas are imperfect, as some children are classified as lower-risk and treated with less intensive therapy relapse, while others deemed higher-risk are probably over-treated. Most cooperative groups previously used morphological clearance of blasts in blood and marrow during the initial phases of chemotherapy as a primary factor for risk group allocation; however, this has largely been

Algorithmic Approach to Diagnosis of Salivary Gland Neoplasms Based on Morphology and Select Immunostains.

Salivary gland tumors present diagnostic challenges even to the experienced surgical pathologist due to overlap in both histologic and immunohistochemical features. Morphology, immunoprofile, and genetic testing (in some cases) can be used judiciously in an algorithmic approach to narrow the differential diagnosis on a salivary biopsy or resection specimen. The purpose of this review is to present an objective algorithmic approach to assessing salivary gland specimens that utilizes histomorphology, a limited panel of immunohistochemical stains, and, occasionally, genetics to aid in diagnosis.

Evaluation of hepatic cystic lesions.

Hepatic cysts are increasingly found as a mere coincidence on abdominal imaging techniques, such as ultrasonography (USG), computed tomography (CT) and magnetic resonance imaging (MRI). These cysts often present a diagnostic challenge. Therefore, we performed a review of the recent literature and developed an evidence-based diagnostic algorithm to guide clinicians in characterising these lesions. Simple cysts are the most common cystic liver disease, and diagnosis is based on typical USG characteristics. Serodiagnostic tests and microbubble contrast-enhanced ultrasound (CEUS) are invaluable in

Cell-free DNA screening for trisomies 21, 18, and 13 in pregnancies at low and high risk for aneuploidy with genetic confirmation.

BACKGROUND: Cell-free DNA noninvasive prenatal screening for trisomies 21, 18, and 13 has been rapidly adopted into clinical practice. However, previous studies are limited by a lack of follow-up genetic testing to confirm the outcomes and accurately assess test performance, particularly in women at a low risk for aneuploidy. OBJECTIVE: To measure and compare the performance of cell-free DNA screening for trisomies 21, 18, and 13 between women at a low and high risk for aneuploidy in a large, prospective cohort with genetic confirmation of results STUDY DESIGN: This was a multicenter prospect

CACs Recognition of FISH Images Based on Adaptive Mean Teacher Semi-supervised Learning with Domain-Knowledge Pseudo Label.

Circulating genetically abnormal cells (CACs) serve as crucial biomarkers for lung cancer diagnosis. Detecting CACs holds great value for early diagnosis and screening of lung cancer. To aid the identification of CACs, we have incorporated deep learning algorithms into our CACs detection system, specifically developing algorithms for cell segmentation and signal point detection. However, it is noteworthy that deep learning algorithms require extensive data labeling. Consequently, this study introduces a semi-supervised learning algorithm for CACs detection. For the cell segmentation task, a co

Autosomal Dominant Polycystic Kidney Disease: Clinical Assessment of Rapid Progression.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) causes the development of renal cysts and leads to a decline in renal function. Limited guidance exists in clinical practice on the use of tolvaptan. A decision algorithm from the European Renal Association-European Dialysis and Transplant Association (ERA-EDTA) Working Groups of Inherited Kidney Disorders and European Renal Best Practice (WGIKD/ERBP) has been proposed to identify candidates for tolvaptan treatment; however, this algorithm has not been assessed in clinical practice.
