Home Management Versus Primary Care Physician Follow-up of Patients With Distal Radius Buckle Fractures: A Randomized Controlled Trial.

STUDY OBJECTIVE: In patients with a distal radius buckle fracture, we determine whether home removal of a splint and physician follow-up as needed (home management) is noninferior to primary care physician follow-up in 1 to 2 weeks with respect to functional recovery. We also compare groups with respect to health care and patient-level costs. METHODS: This was a noninferiority randomized controlled trial conducted at a tertiary care children's hospital. Eligible patients were randomized to home management versus primary care physician follow-up and received telephone contact at 3 and 6 weeks

A literature-based intervention for older people living with dementia.

BACKGROUND: While several studies have explored the impact of literature and reading on mental health, there has been relatively little work done on how a literature-based intervention might impact on the behaviours of those living with dementia. The present report addresses the effect that a specific literature-based intervention - Get into Reading, designed and practised by national charity The Reader Organisation - might have on the health and well-being of people living with dementia. AIMS: This present study arises out of a service evaluation that specifically assessed to what extent the

Comparison of the effectiveness of home visits and telephone follow-up on the self-efficacy of patients having un-dergone coronary artery bypass graft surgery (CABG) and the burden of their family caregivers: A randomized con-trolled trial.

OBJECTIVES: This study aimed to compare home visits and telephone follow-up effectiveness on patients' self-efficacy undergoing Coro-nary Artery Bypass Graft Surgery -CABG- and caregivers' burden. METHODS: In this randomized clinical trial, 114 patients undergoing CABG were assigned to the three groups of home visits, telephone follow-up, and control based on the stratified block randomization. The self-management program of the home visit group included four face-to-face 60-minute training sessions once a week, and for the telephone follow-up group, four 30-minute telephone counseling sessio

Home Visiting and Use of Infant Health Care: A Randomized Clinical Trial.

BACKGROUND AND OBJECTIVES: Evaluations of home visiting models have shown that they can reduce children's health care use in the first year of life. Models that exclusively use nurses as home visitors may cost more and be infeasible given nursing shortages in some locations. The goal of this study was to test whether a universal home visiting model employing a nurse-parent educator team as home visitors reduces health care use in the first year of life. METHODS: This study was a randomized controlled clinical trial of an intensive home visiting program delivered in homes of primary caregivers

Frailty and sleep quality in elderly living in old age homes of Kathmandu Valley: a cross-sectional study.

BACKGROUND: Frailty is a clinical syndrome characterized by age-related decline in strength, endurance, and physiological function that increases vulnerability to adverse health outcomes. Sleep disturbances are also common among older adults, particularly in long-term care settings, and may contribute to worsening frailty and reduced quality of life. However, limited evidence exists regarding the relationship between frailty and sleep quality among institutionalized elderly populations in Nepal. METHODOLOGY: A cross-sectional study was conducted among 219 individuals aged ≥60 years residing i

Impact of Infection Prevention Measures on Patients With Dementia in Care Facilities During the COVID-19 Pandemic.

AIM: The coronavirus disease 2019 (COVID-19) pandemic has profoundly impacted people with dementia (PWD) residing in care facilities, where stringent infection prevention measures are implemented. While these measures aim to reduce virus transmission, they may inadvertently exacerbate cognitive and physical decline in PWD. This study examines the relationship between the types and the number of COVID-19 infection prevention measures and their adverse effects on PWD in Special Elderly Nursing Homes (SENHs) and Group Homes (GHs) in Japan. METHODS: A cross-sectional survey was conducted between …

Quality of care through the eyes of residents with Huntington's disease living in a nursing home: A qualitative explorative study.

Background: Moving to a nursing home is often unavoidable for late-stage patients with Huntington's disease (HD). Specialized care is needed, adjusted to the specific needs and characteristics of this usually young group within the nursing home. Despite this, there are no specific, validated methods for assessing the experienced quality of care (QoC) by HD nursing home residents. Objective: We used a qualitative, phenomenological approach to gain insight into the perspectives of HD residents and identify elements for assessing QoC in HD specialized nursing homes. Methods: Fifteen advanced-stag

Laundry detergents and surfactants-induced eosinophilic airway inflammation by increasing IL-33 expression and activating ILC2s.

INTRODUCTION: Epidemiological studies demonstrated that cleaning work and frequent use of cleaning products are risk factors for asthma. Laundry detergents have been reported to have epithelial barrier-opening effects. However, whether laundry detergents directly induce airway inflammation and its mechanisms in vivo remain to be elucidated. METHODS: Two commercial laundry detergents and two commonly used surfactants for cleaning and cosmetics (sodium lauryl sulfate and sodium dodecyl benzene sulfonate) were intranasally administered to mice. Lungs were analyzed using flow cytometry, histology

Weight management for adults with mobility related disabilities: Rationale and design for an 18-month randomized trial.

Adults with mobility related disabilities (MRDs) represent an underserved group with a high prevalence of overweight/obesity and limited options for weight management. We previously demonstrated clinically meaningful 12-month weight loss in adults with MRDs (-6.2%, 36% ≥5% of baseline weight) using an enhanced Stop Light Diet (eSLD) delivered using at home face-to-face behavioral sessions and optional physical activity. However, the costs/logistics associated with intervention delivery by individual home visits limits the potential for scaling and implementation of this approach. Thus, we will

Self-management tasks to improve mobility and reduce fall risk are not leading to lower research participation in older adults.

PURPOSE OF STUDY: The first aim is to evaluate, in a sub-study, the recruitment process of the Senior Step Study, which was an intervention study on the self-management of mobility and fall risk; the second aim is to explore the reasons mentioned by older people, from three different settings, for (not) participating. METHODS: Subjects were community-dwelling older persons, residents of homes for the elderly, and older persons regularly visiting community centres. The effectiveness of different recruitment procedures was analysed for each setting separately. We also analysed reasons for accep

Environmental surveillance and molecular characterization of Legionella in tropical Singapore.

Legionnaires' disease is often acquired by inhalation of legionellae from a contaminated environmental source. In recent years, Singapore has seen an increase in the use of aerosol-generating fixtures such as mist fans and spa pools. Poorly maintained and designed water fixtures could pose a public health threat to the community. In this study, we provided an update on the prevalence of Legionella in mist fans (N=28), household water heaters with storage tanks (N=19) and instantaneous heaters (N=30); and extended the survey to spa pools (N=29) and aerosol-generating fixtures in nursing homes (

Types of infectious outbreaks and their impact in elderly care facilities: a review of the literature.

BACKGROUND: infectious outbreaks in long-term care facilities (LTCFs) tend to have a significant impact on infection rates and mortality rates of the residents. OBJECTIVES: this review aimed to update the information on pathogens identified in such outbreaks and to try to explore indicators that reflect the impact of outbreaks among residents and health care workers (HCWs). METHODS: MEDLINE (1966-2008) was used to identify outbreaks using the following thesaurus terms: 'Cross-Infection', 'Disease Outbreaks', 'Urinary-Tract Infections' and 'Blood-Borne Pathogens'. Elderly care facilities were

Convergence of clinical staff ratings and research ratings to assess patients with schizophrenia in nursing homes.

OBJECTIVE: A large number of older patients with serious mental illnesses, including schizophrenia, now reside in nursing homes or similar residences, yet little is known about assessments, services, or outcomes for these patients. The Minimum Data Set (MDS) is a mandatory assessment instrument for nursing care facilities, and although it has been well studied in the general nursing home population, little is known about its validity in assessing schizophrenia, which was the purpose of this study. METHODS: A group of 77 patients with schizophrenia had been recruited as part of a longitudinal

Early-life adversity before age 5 is associated with reduced cardiometabolic health characterized by increased arterial stiffness and inflammation in adolescence.

OBJECTIVE: Cardiometabolic disease may have roots in early-life adversity (ELA). Most studies linking harsh conditions in the first year of life are either retrospective or follow individuals who continue to live in harsh conditions. The present study identified the impact of ELA, operationalized as institutional care limited to infancy and early childhood, on cardiometabolic health in adolescence. METHOD: Participants were 190 youth aged 12-21, half adopted internationally from institutional care (previously institutionalized [PI]) into highly resourced homes and a comparison group of youth
