Multiorgan female pelvic prolapse: pelvic organ prolapse suspension (P.O.P.S.) stapled transanal rectale resection (S.T.A.R.R.): new surgical tecniques and results.

AIM: To demonstrate the surgical treatment validity and the post-operative complication decrease. MATERIAL OF STUDY: Seventythree women who underwent P.O.P.S. + S.T.A.R.R. treatment, follow-up one year. RESULTS: We observed an important reduction or a completely disappearance about pre-operative signs and symptoms. DISCUSSION: We are aware that the proposed technique, if taken into account by urogynecologists, will raise several arguments and will raise many doubts and perplexities. For this reason we wanted develop a follow-up sufficiently long and many case studies with data to support ou

Lumbosacral spondylodiscitis: an unusual complication of abdominal sacrocolpopexy.

Lumbosacral spondylodiscitis, an unusual complication of abdominal sacrocolpopexy using synthetic polypropylene mesh is reported. A young sexually active female with stage IV pelvic organ prolapse was managed with abdominal hysterectomy and sacrocolpopexy. Cervical dysplasia demanded hysterectomy and sacrocolpopexy was done to achieve good long-term results. Mesh exposure was noticed in the early post-operative period which initially responded to conservative management. Eight weeks later, the patient reported with severe pain in lower back restricting her physical movements and ambulation. Fu

Polypropylene mesh vs. site-specific repair in the treatment of anterior vaginal wall prolapse: preliminary results of a randomized clinical trial.

OBJECTIVE: Pelvic organ prolapse is a disorder caused by the imbalance between the forces responsible for supporting the pelvic organs in their normal position and those that tend to expel them from the pelvis. Anterior vaginal wall prolapse, known as cystocele, is the most common form of prolapse and can result from lesions in different topographies of the endopelvic fascia. Currently, a woman has an 11% risk of being submitted to a surgical procedure to correct pelvic floor disorder, and a 29% chance of being reoperated due to failure in the first surgery. METHODS: A prospective randomized

Expert opinion: vaginal aplasia: creation of a neovagina following the Creatsas vaginoplasty.

Vaginal aplasia is an uncommon congenital anomaly which is usually diagnosed as part of the Mayer-Rokitansky-Kuster-Hauser syndrome. The appropriate management of the anomaly includes the creation of a neovagina, and various surgical or non-surgical techniques have been described. Among them the Creatsas modification of Williams vaginoplasty is an operative technique which involves the following surgical steps: Under general anesthesia, and after cutting the hymen with the use of diathermy, a U-shaped incision is made in the vulva extending across the perineum and up to the medial side of the

Central perineal hernia mimicking rectocele: a case report.

BACKGROUND: A perineal hernia is a very rare clinical finding. Three forms are distinguished: anterior, posterior, and central. Diagnosis of the last one is difficult, and sometimes, it is falsely named a posterior rectocele. AIM: This work presents a successfully treated case of central perineal hernia and makes a brief summary of existent literature on the problem. PRESENTATION OF THE CASE: We report of a 67-year-old female patient with a symptomatic central pelvic floor hernia. After radiological confirmation of the diagnosis, a transperitoneal approach was chosen to reposition the protru

Laparoscopic microsurgical anastomosis of the blocked, solitary post-ectopic Fallopian tube: case report.

The surgical options for the management of ectopic pregnancy include linear salpingotomy or salpingectomy. If salpingotomy is performed, subsequent tubal blockage may result and if this occurs in a solitary Fallopian tube then assisted reproductive technology is recommended as the treatment of choice. We describe a case report detailing the application of laparoscopic microsurgical tubal reanastomosis in two patients with post-ectopic blockage in a solitary Fallopian tube. Both patients conceived post-operatively and subsequently delivered term pregnancies. Laparoscopic microsurgery offers an

[Treatment approach for vesicogenital fistula. Retrospective analysis of our data].

The vesicogenital fistula are abnormal communications between female genitalia and urinary bladder. We recorded all the vesicogenital fistula diagnosed since 1986, analyzing aetiology, treatment applied, complications and results. Total number of fistula have been 20 (18 vesicovaginal and 2 vesicouterine). The distribution in vesicovaginal fistula was iatrogenic in 15 cases (83%) and tumoral in 3 cases (17%). Vesicouterine fistula were due to cesarean. The initial treatment of the iatrogenic fistula was conservative using foley catheter. Twenty percent of the patients were cured with this trea

The disappointing quality of published studies on operative techniques for rectovaginal fistulas: a blueprint for a prospective multi-institutional study.

BACKGROUND: Treatment of rectovaginal fistulas is difficult, and many surgical interventions have been developed. The best surgical intervention for the closure of these fistulas is still unclear. OBJECTIVE: A systematic review was performed reporting the outcomes of different surgical techniques for rectovaginal fistulas. DATA SOURCES: Medline (PubMed, Ovid), Embase (Ovid), and The Cochrane Library databases were searched for eligible articles as well as the references of these articles. STUDY SELECTION: Two independent reviewers analyzed the search results for eligible articles based on t

Effects of wound closure on wound healing in gynecologic surgery: a systematic literature review.

A systematic review was undertaken using the Cochrane and PubMed databases to answer the question of how wound closure affects wound healing after gynecologic surgery. Leaving the vaginal vault open after vaginal and abdominal hysterectomy is as safe as closing it. When closing the vaginal vault, there is no difference between sutures and staples. Nonclosure of the peritoneum is a safe method after vaginal and abdominal hysterectomy. After laparotomy there is no difference between continuous and interrupted sutures regarding wound infection and/or dehiscence. After vertical midline incisions i

Mesh-related infections after pelvic organ prolapse repair surgery.

The use of vaginal meshes has been an advance in the surgical management of women with pelvic organ prolapse. We reviewed the literature to synthesize the evidence regarding the infectious complications related to this new type of foreign body. We searched PubMed, current contents, and references of initially identified relevant articles and extracted data regarding the incidence, clinical manifestation, and management of vaginal mesh-related infections. The incidence of mesh-related infections and erosion ranged from 0 to 8%, and 0 to 33%, respectively, in the published studies. Various facto

Perioperative iron deficiency and anaemia in scheduled gynaecological surgery: An update based on findings from the PERIOPES and CARENFER studies: Iron deficiency in gynaecological surgery.

Major gynaecological surgery is a significant risk factor for intra and postoperative blood loss. Effective iron deficiency (ID) and anaemia management is critical for ensuring patient safety. The aim of this update was to take an in-depth look at two recently published studies focusing on the assessment and management of ID and anaemia in subgroups of patients undergoing gynaecological surgery from the CARENFER PBM (2023) and PERIOPES (2023 and 2024) studies. Among the 6999 patients included in the three studies, 354 involved gynaecological procedures. Within this cohort, the prevalence of pr

Use of the Martius Flap for Tethered Vagina Syndrome After a Single-Incision Mini-Sling Procedure.

Tethered vagina syndrome is an iatrogenic, but poorly defined condition that usually develops after surgical procedures performed on the anterior vaginal wall. Loss of elasticity as a result of scarring in the bladder neck-urethra is blamed for its pathogenesis. Patients often present with typical, massive uncontrollable urine loss and have specific examination findings. Having awareness about the diagnosis is important for its management. Surgical restoration and maintenance of bladder neck elasticity are essential for treatment. In our case, we presented the surgical approach we applied to t

[Innovative prolapse surgery without mesh implants].

BACKGROUND: In many countries, such as France, England, USA, Canada, Australia, and New Zealand, alloplastic material in prolapse surgery has been paused due to the US Food and Drug Administration (FDA) warning, and restricted in other countries like the Netherlands and Sweden. For Europe and thus Germany, the SCENIHR report allows alloplastic material to be used for prolapse repair after recurrence and in other special situations. QUESTION: Which established and innovative prolapse surgeries without alloplastic material are currently available? METHODS: A literature search was carried out o

Uterine prolapse: impact of the condition and practical advice.

Pelvic-organ prolapse is where organs such as the uterus move out of place; in some cases, they can protrude out of the body. It occurs when muscles extending from the pubic bone to the coccyx or the levator ani muscles become weak or are injured. Although it is not life-threatening, the condition can be life limiting, adversely affecting physiological processes, function and quality of life, with patients reporting years of pain, discomfort, humiliation and embarrassment. Uterine prolapse is often under-reported as women may feel resigned to a condition that is viewed as part of getting old o
