BACKGROUND: Gynecomastia is enlargement of the male breast. Although treatment is not indicated in most cases, aesthetic reconstructive surgery is commonly performed for psychological reasons. The goals in surgical treatment are to restore the breast contour with minimal scar and to protect areolar anatomy and sensation. Thirty-four HIV-positive men with gynecomastia were seen in an HIV outclinic during a 20-month period (incidence of 2.4 cases/100 patients receiving HAART per year). It developed mainly in subjects having good immunologic and virologic status, after an average of 3 years of HAART. No hormone abnormalities were found, or association with specific drugs. Although initially unilateral, more than half of cases progressed to bilateral gynecomastia. Spontaneous resolution occurred in most subjects with 12 months without modifying therapy.

Gynecomastia was treated surgically by Webster's method in 23 patients during the period from 1983 to 1989. This surgical procedure was used if gynecomastia was classified from stage 1 to stage 2B. Cosmetic reasons were the major reasons for operation in 70% of the patients. The most common late postoperative sequelae were inverted areolae and hypaesthesiae of the areolar region. An occult Leydig cell tumor is described in a patient who presented with gynecomastia. Diagnosis was established by means of testicular ultrasonography and selective testicular venous sampling. The pathophysiology and management of this rare disease entity are discussed.

Four men with metastatic nonseminomatous testicular cancer, who had received combination chemotherapy, experienced transient gynecomastia. Serum hormone levels were measured during and after the resolution of the gynecomastia. These levels indicated toxic effects on Leydig's cells and germinal epithelium concurrent with raised serum estradiol levels in the absence of tumor. The gynecomastia may have been caused by Leydig's cell dysfunction or refeeding after substantial weight loss. This benign form of gynecomastia must be recognized to avoid unnecessary investigation and treatment.

Gynecomastia is a common finding that is present in up to 57% of men. It is caused by proliferation of the mammary glands, which leads to the development of dense subareolar tissue. OBJECTIVES: An uncommon case of clinically occult Leydig cell tumor of the testis is described. The literature is reviewed with special reference to the difficulty encountered in making the diagnosis.
METHODS/RESULTS: The clinical data of a patient with occult Leydig cell tumor of the testis, whose only remarkable feature was a long history of gynecomastia, are presented.
CONCLUSIONS: A hormonal study and scrotal ultrasound evaluation are essential in the diagnosis of this tumor type and should be performed even in the absence of testicular anomalies on palpation.

We report here a new case of feminizing adrenocortical carcinoma revealed by a bilateral gynecomastia in a 50 year old man. The malignant character was already suspected on clinical and hormonal data. It was confirmed by the pathology. The op'DDD treatment of three weeks was of too short duration to allow any opinion on its efficacy.

A case of post-chemotherapy gynecomastia is reported in a 40 years old male patient with a tumor of seminomatous germinal cells with no evidence of tumoral reappearance or augmentation of the chorionic gonadotropin hormone. The existence of primary hypogonadism was tested. A differential diagnosis was performed and possible physiopathological mechanisms of the gynecomastia were analyzed in a patient previously treated for testicular cancer.

Gynecomastia is a reaction that is not usually associated with calcium channel blocker therapy.
