The complexity of a gunshot wound to the hand with segmental bone loss and adjacent joint disruption presents a unique challenge for the reconstructive surgeon. There are several options for posttraumatic reconstruction of hand joint defects, ranging from arthrodesis, implants, and autologous arthroplasty. Despite the abundance of literature regarding guidelines for isolated osteoplasty and arthroplasty, there are only case reports describing management of bone and joint defects, all within the setting of cancer resection. This case report presents a 24-year-old, right-hand dominant man with a

An analysis and evaluation of 241Am in the whole body of a donor to the U.S. Transuranium Registry (USTR) is presented in five parts. The USTR donor's pertinent medical history, autopsy findings and antemortem evaluations of intake and systemic burden are described in Parts I and II. The donor was a 49-yr-old male Caucasian radiochemist who died of metastic melanoma in 1979. His work with actinide elements began in 1952, and the greatest potential for intake of 241Am was when he used an unsealed 241Am source in his doctoral research (1952-54). The first indication that an intake had occurred w

The medical records of 13 cases (16 limbs) diagnosed with ulnar club hand in Beijing Jishuitan Hospital between 1966 and 2016 were reviewed. The radiological characteristics of upper limb bones, the shape and function of shoulder, elbow, forearm, wrist, and hand were recorded. The surgical options include radial wedge osteotomy, ulnar anlage excision, release of syndactyly or narrowed first web, and release of camptodactyly were performed to correct deformities. The subjective evaluation of patients or their families was recorded. Thirteen patients with 16 affected limbs were identified with u

The management of arteriovenous malformations (AVMs) of the hand remains challenging. When radical excision results in large defects of both soft tissue and vessels, flow-through flap transfer is useful; however, flow-through flap options for hand and digit reconstructions are limited. Herein, we describe the use of a superficial circumflex iliac artery perforator (SCIP) flow-through flap after excision of an AVM of the hand. A 44-year-old female patient with an AVM of the hand required simultaneous reconstruction of soft tissue, vascular, and bone defects after radical excision of vascular le

Mycetoma, or maduromycosis, is a chronic granulomatous infection involving the skin, subcutaneous tissues, and bones, most commonly affecting the lower extremities. Hand involvement is rare and often leads to diagnostic delays due to its atypical presentation. Mycetoma is classified as actinomycetoma (bacterial) or eumycetoma (fungal), each requiring distinct management approaches. We report a rare case of actinomycetoma affecting the right thumb in an 81-year-old man, initially misdiagnosed as a giant cell tumor. Magnetic resonance imaging revealed a well-defined subcutaneous mass without bon

Several accessory bones can be found around the wrist, which may pose a diagnostic challenge. Hereby, we report a unique case of a traumatic fracture of an aberrant os styloideum in a 66-year-old male. The patient presented with a swollen dominant right hand and a hematoma on its anterior and posterior aspects, as a result of a fall on an outstretched hand. The diagnosis was based on a CT scan, which showed a fractured accessory bone between the distal portion of the capitate bone and the base of the third metacarpus. Due to its typical location, the ossicle was identified as os styloideum. Th

We describe a case of lead poisoning in a worker after hand and forearm trauma with fracture of radius and multiple fractures of metacarpal bones and hand phalanges and tissue infiltration of lead oxide (PbO) paste. Orthopedic surgery was immediately performed. After 20 days the patient had abdominal colic pain episodes and severe stipsis and blood lead level (BLL) was 60 mcg/mL with urinary lead level (ULL) of 238 mcg/24 h. After mobilization test with calcium disodium edetate were observed a high increase of BLL (180 mcg/dL) and UBL (17,000 mcg/24h). An initial anemia was observed and became

This article reviews the underlying anatomy of trigger finger and thumb (fibrous digital pulleys, sesamoid bones), flexor tenosynovitis, de Quervain's syndrome, Dupuytren's contracture, some hand deformities in rheumatoid arthritis, the carpal tunnel syndrome and the ulnar nerve compression at Guyon's canal. Some important syndromes and structures have not been included but such are the nature of these seminars. Rather than being complete, we aim at creating a system in which clinical cases are used to highlight the pertinent anatomy and, in the most important part of the seminar, these pertin

Results of grip reconstruction in two patients after severe crush-avulsion injuries of the hand were presented. Total destruction affected the first or the first and the second ray of the hand which was accompanied by soft tissue loss, finger amputations and complex bone fractures with vessels and nerves damage. Soft tissue reconstructions were performed using various methods. Pedicled groin flap followed by nerves reconstruction and tenolysis were done in one patient. In the second case soft tissues were rebuilt with distally pedicled adipo-cutaneous forearm flap supplied by distal radial art

Holt-Oram syndrome is an autosomal dominant disorder characterized by heart defects in combination with characteristic upper-limb abnormalities. A woman with no family history of genetic diseases underwent prenatal sonography at 25 weeks' menstrual age to screen for fetal anomalies. Sonography revealed abnormalities in the upper limbs and heart. The limb abnormalities included bilateral absence of radii and thumbs: the left hand had no carpal or metacarpal bones, and each of the 4 fingers on that hand had only 1 phalangeal bone. Cardiac malformations included an atrial septal defect and Ebstei

Enchondromata are the most frequent benign tumours of the bones of the hand. From 1973 to 1990 77 patients were treated at the trauma- and hand surgery department of the Westfälische Wilhelms-University in Münster because of a histologically confirmed enchondroma. Those 52 patients (32 female, 20 male, average age 32 years), who were operated on until 1986 were reviewed. After an average history of 16 months the diagnosis was made most often because of a painful swelling. In 15% a spontaneous fracture was seen and in 17% the enchondroma was diagnosed by chance. The enchondroma was merely exsti

In recent years stable internal fixation of hand fractures has become more commonly used though the majority of fractures of the hand bones are still treated successfully by other orthopedic means. We feel that in selected fractures internal fixation achieves better results than conservative treatment. Operative methods with less traumatic skin incisions for the fixation of multiple metacarpal fractures are proposed. A retrograde screw fixation technique is presented which permitts application of compression in very small bone fragments, which risk to be crashed by standard screwing. Results o…

INTRODUCTION AND IMPORTANCE: Giant cell tumor (GCT) of bone rarely involves the metacarpal bones. Secondary aneurysmal bone cyst (ABC) changes arising in association with GCT create distinct diagnostic and therapeutic challenges in this uncommon skeletal location.
CASE PRESENTATION: A 21-year-old right-hand-dominant female presented with progressive right fourth metacarpal pain and swelling following minor trauma. Radiographs and magnetic resonance imaging (MRI) demonstrated an expansile osteolytic lesion with cortical thinning and internal fluid-fluid levels suggesting GCT with secondary ABC

Balistic transfixing hand traumas require a complex reconstruction management. Stabilization and reconstruction of the injuried tissues need a double skin-paddle coverage. We present an original case report of a double skin-paddle scapular/parascapular free flap used to cover a through and through gunshot injury of the right hand. A 14-years-old patient presents a severe and extensive wound with full-thickness palmar and dorsal skin defects, dislocation of the carpal bones, median nerve and flexor tendons losses. Distal vascularization is maintained by the deep palmar arch system supplied by t

Aneurysmal bone cyst is a rare, rapidly growing, and destructive benign bone tumor that even more rarely involves the bones of the hand. Various treatment options for aneurysmal bone cyst have been reported in the literature, but controversy exists regarding optimal treatment. Due to its rarity in the hand, no evidence-based treatment regimen has been established. A 21-year-old man presented with a history of pain and local swelling over his fifth metacarpal of 5 months' duration. Physical and radiographic examination of the hand was consistent with aneurysmal bone cyst. After biopsy, patholog

PURPOSE: To assess particular features of fetal hand evaluation using three-dimensional ultrasound, and to determine whether any advantage is offered by three-dimensional over two-dimensional ultrasound in fetal hand evaluation.
METHODS: Three-dimensional fetal data were collected prospectively from 44 hands in 40 fetuses from a predominantly high-risk patient population. All had a concurrent two-dimensional ultrasound examination of the hands. Diagnoses from two- and three-dimensional ultrasound examinations were compared and additional information from the three-dimensional volume assessmen

INTRODUCTION: Simple bone cyst or unicameral bone cyst (UBC) are benign cystic lesions commonly found in femur and humerus. However hand is a very rare site of occurrence. Treatment described for UBC of hand commonly involves curettage and bone grafting.
CASE REPORT: A 7 year old right hand dominant girl presented to us with chief complaints of pain and swelling in right 4th metacarpal since 2 month. On imaging, plain radiographs of right hand showed expansile lytic lesion on Metaphyseal-diaphyseal region of 4th metacarpal with pathological fracture. MRI showed cystic lesions with internal lo

This study was motivated by the serious impact that cumulative trauma disorders (CTDs) of the upper extremities have on the meat packing industry. To date, no quantitative data have been gathered on the kinematics of hand and wrist motion required in bone-trimming jobs in the red-meat packing industry and how these motions are related to the risk of CTDs. The wrist motion of bone-trimming workers from a medium-sized plant was measured, and the kinematic data were compared to manufacturing industry's preliminary wrist motion benchmarks from industrial workers who performed hand-intensive, repet

Chondrosarcoma of the hand is a rare disease, but is one of the more common malignancies of the hand. Biopsies and imaging are a fundamental step in determining correct diagnosis, grading and selection for best treatment. We describe the case of a 77-year-old male complaining of a painless swelling in the proximal phalanx of the third ray of left hand. A biopsy was performed and the histology revealed a G2 chondrosarcoma. The patient underwent III ray amputation with metacarpal bone disarticulation and sacrifice of the radial digit nerve of the fourth ray. Definitive histology revealed grade 3
