Gianotti-Crosti syndrome: a case report in an adult.

Gianotti-Crosti syndrome (GCS) is a viral-associated eruption that most commonly occurs in children aged 15 months to 2 years. It consists of monomorphic red-brown to pink papules and vesicles distributed symmetrically on the cheeks, extensor surface of the extremities, and buttocks. The eruption usually spontaneously resolves over the course of 10 to 60 days. We report the rare case of GCS in an adult. An otherwise healthy 20-year-old woman presented with a pruritic eruption of 2 weeks' duration on the dorsal aspect of her hands and feet, elbows, and knees. The patient received oral corticost

Mycoplasma pneumoniae infection presenting as bullous papular purpuric gloves and socks syndrome: novel association and review of the literature.

Papular purpuric gloves and socks syndrome (PPGSS) is a self-limited, often febrile illness with symmetric edema and erythema of the hands and feet; papular, petechial, and purpuric acral dermatosis; and mucosal lesions in children and young adults. Most of the cases of PPGSS have been reported to be caused by parvovirus B19 and other viruses. This study describes a case resulting from Mycoplasma pneumoniae infection in an adolescent male and reviews the literature.

New antifungal therapies for the treatment of onychomycosis.

Onychomycosis is a common disorder of nails caused by diverse spectrum of fungi ranging from dermatophytes to nondermatophyte molds and yeasts. Presently, oral and topical therapies are the mainstay of treatment with oral therapies yielding somewhat better results. Though various treatment options are available to treat onychomycosis, search for a convenient, cost effective agent with high and long-lasting cure rates is continuing. Currently, many new therapies for onychomycosis are under investigation which includes new formulations of azoles, improvement of existing topical treatments throug

Recent progress on the topical therapy of onychomycosis.

Onychomycosis is a fungal infection of the fingernails and toenails that results in thickening, discoloration, splitting of the nails and lifting of the nail from the nail bed. The disease is caused by dermatophytes and has a high incidence within the general population, especially among older individuals. Present treatment options include both oral and topical drugs, with oral therapies giving better outcomes; however, neither of these treatment options provides high cure rates that are durable. The difficulty in treating onychomycosis results from the deep-seated nature of the infection with

Acropustulosis repens.

BACKGROUND: Pustular eruptions of the extremities of the fingers and toes (acropustulosis) have been grouped under the single term "Hallopeau's acrodermatitis continua", which is a severe disease, with uninterrupted course, and successive eruptions that may become generalized. However, there is a form of acropustulosis with few lesions, evolving with remissions and relapses, with a benign course. It is necessary to separate these two forms of acropustulosis. METHODS: I had the opportunity to observe six patients with a mitigated form of acropustulosis that does not fit into the diagnostic cri

Tuberous sclerosis complex associated with dyschromatosis universalis hereditaria.

Tuberous sclerosis is an autosomal dominant disease due to mutations in two genetic loci, characterized by hamartoma formation in the skin, nervous system, heart, kidney and other organs. Dyschromatosis universalis hereditaria is an autosomal dominant genodermatosis, characterized by small hyperpigmented and hypopigmented macules, uniformly distributed over the entire body. The face is rarely involved and the palms, soles and mucous membranes are usually spared. We report a case of tuberous sclerosis with dyschromatosis universalis hereditaria, with hyperpigmented and hypopigmented macules aff

Occupational contact dermatitis from a grease.

BACKGROUND: Contact allergy to grease is rare and often not even suspected. We investigated such a case in which the detected allergen was the stabilizer in the grease, which is rarely found as an allergen. OBJECTIVE: Thin-layer chromatography (TLC) was used in a novel way and helped detect the allergen. METHODS: Patch testing with our standard series, a metal-working series, the different substances individually, the grease in serial dilution and extracts of personal objects, the TLC plate. Gas chromatography-mass spectrometry was also used. RESULTS: Test results indicated contact allergy

Acute allergic contact dermatitis due to para-phenylenediamine after temporary henna painting.

The use of temporary natural henna painting for body adornment and hair dyeing is very common in several countries of the Indian subcontinent, Middle East, and North Africa, and the fad is spreading in other parts of the world. Several cases of para-phenylenediamine (PPD) contaminated, temporary traditional/natural henna induced sensitization and acute allergic reaction have been reported, along with occasional serious long term and rare fatal consequences. We report here a 17-year-old girl with blisters over her hands of five-days duration that appeared within 72 hours of applying a temporary

Photoallergic contact dermatitis due to combined UVB (4-methylbenzylidene camphor/octyl methoxycinnamate) and UVA (benzophenone-3/butyl methoxydibenzoylmethane) absorber sensitization.

In a 71-year-old male Caucasian patient with persistent eczema on light-exposed skin, photocontact allergy was demonstrated to the UV filter substances 4-methylbenzylidene camphor (UVB), octyl methoxycinnamate (UVB), benzophenone-3 (UVA) and butyl methoxydibenzoylmethane (UVA) present in sunscreen products used by the patient over several years. A significantly reduced UVB sensitivity of 25 mJ/cm2 in this patient (normal minimal erythema dose in our laboratory = 70-130 mJ/cm2) was considered an early indication of a persistent light reaction. Topical anti-inflammatory treatment over 2 weeks to

[Chronic sclerodermiform syndrome disclosing subcutaneous T-cell lymphoma].

INTRODUCTION: Subcutaneous tissue is an uncommon primary localization for T-cell lymphomas. Panniculitis with recurrent papulonodules are reported in most cases. CASE REPORT: We report a case in which small-cell pleiomorphic non-Hodgkin lymphoma developed initially in subcutaneous tissue followed by secondary systemic extension. The first clinical manifestation was a sclerodermiform syndrome involving the four limbs. DISCUSSION: This type of lymphoma appears to have a polymorphic clinical presentation. Physicians should be aware of this type of lymphoma and use immunohistological techniques

Onychomycosis caused by Scytalidium dimidiatum.

We report a case of onychomycosis caused by Scytalidium dimidiatum (syn., Hendersonula toruloidea) in a patient who did not live in an endemic area. This nondermatophyte mold may produce disease indistinguishable from dermatophyte fungi, but it does not respond to current systemic antimycotic therapy. Distal subungual onychomycosis, lateral onycholysis followed by lateral nail plate invasion, and chronic paronychia are common nail presentations.

[Ulcers of the tongue, pityriasis lichenoides and primary parvovirus B19 infection].

INTRODUCTION: We report a case of parapsoriasis en gouttes (or pityriasis lichenoides) which presents two peculiarities. First, the patient had lingual ulcerations and second, the eruption appeared during a seroconversion for Parvovirus B19. OBSERVATION: A 25 old woman presented a first episode of characteristic parapsoriasis en gouttes associated with purpuric palmoplantar lesions and lingual ulcerations, reaching deep muscular in histology. DISCUSSION: This observation of parapsoriasis en gouttes, peculiar because of lingual ulcerations, is mostly interesting because of its association wit

Recurrent pyogenic granulomas: a form of lobular capillary hemangioma.

Two cases of pyogenic granuloma recurred despite what was originally thought to be a sufficient surgical excision. The granulomas presented clinically as polypoid structures, one on the finger and one on the chin. On histologic examination, all showed polypoid configuration with lobules of small vessels and endothelial cell proliferation. The recurrent lesions also showed similar lobules, but interspersed between the collagen bundles in the deeper dermis. Vascular tumors such as the lobular pyogenic granulomas, intravenous capillary hemangiomas, and acquired tufted angiomas were compared to ou

[Osteoarticular manifestations of pustulosis palmaris et plantaris].

In 6 patients with clinically and histologically proven pustulosis palmaris et plantaris the disease was complicated by osteo-articular manifestations. Five patients presented with articular symptoms and one with aseptic subacute osteomyelitis. The distribution of these bone and joint disorders was different from that of Sonozaki's "pustulotic arthro-osteitis": in contrast with the latter, the anterior chest was inconstantly involved whereas the spine, sacro-iliac joints and peripheral articulations were more frequently affected. Although the B27 antigen is usually absent, it seems possible to

Studies on experimental exanthema in rabbits by means of recurrent systemic sensitization.

To explore the relationship between the tonsils and pustulosis palmaris et plantaris, the author performed several experiments. Since, in the pustulosis palmaris et plantaris abscess, many pleomorphic leukocytes are seen histologically, in order to provoke this condition, sensitized lymphocytes isolated from sensitized rabbits were transferred intravenously. To localize the site of immunological reaction on the skin, extracts were used from cutaneous abscesses elicited by intracutaneous injection of BCE, CA, and PPD, emulsified with FCA. The results were as follows: (1) exanthema was demonstra
